    Accel at Willow Bend

    Do not choose this facility for your love one. There is absolutely no management in this facility. Because it was normal for patients to wait an hour for someone to take them to the restroom or attend any other need. I quickly determined I could not leave my wife alone we were constantly with her. We had to watch medications because of their consistent errors. The only thing favorable I can say is that the rehab people cared and did a good job. Probably the worst part of the operation is the dining room. The food and service was so bad that patients did not eat the bad food, after meals when I examine plates and so how much they left it was obvious that many of the people went away hungry. They were constantly out of products that the patients needed and wanted. One four-day holiday the kitchen was out of applesauce & yogurt which many patients use to take their pills, nurses on the floor were giving patient pills with jelly packets. The kitchen has published hours of serving but the kitchen pays no attention consistently patients would be in the dining room for breakfast waiting an hour to an hour 1/2 to even get anything to drink. The needs of long-term patients were ignored more than short-term patients. When you walk in the halls bells were constantly ringing making you aware that somebody was in need. I cannot believe that any business can be as poorly managed as this facility.

    My mother recently stayed at the Accel Rehabilitation Hospital in Plano a few weeks and this facility was fantastic in all categories. Then she was transferred to Accel at Willow Bend for final rehab prior to going home. Accel at Willow Bend was a very bad experience for her and her family as described below.

    I tried calling my mother at Willow Bend’s 24/7 phone number 214-501-4672 nine (9) times the first two days she was there and no response after 12 rings for each call. I had to call on her cell phone to speak to her. I contacted Accel Rehabilitation Hospital about this and I was informed they also had a hard time getting someone to answer the phone there.

    After a few days at Willow Bend my mother complained about her leg hurting and maybe having an infection as the wound around her recent leg surgery was very red. The nurse who visited her room told her the leg was fine and looked normal to her. The next day the pain increased and she requested to see the staff doctor who determined it was an infection and started antibiotic IV therapy immediately.

    My mother was also using a feeding tube for food consumption. She had complained several times about pain where the feeding tube was attached to her stomach. The staff thought just feeding her slower would remedy the problem. A couple of days later my sister visited her and requested help for our mother’s severe stomach pain and the staff said the pain was not an issue. My sister called 911 and my mother was transported to an ER who immediately did surgery and replaced the feeding tube to correct the issue and stop the pain. After surgery she was transferred to the Presbyterian Hospital where she stayed until she could be transferred to another rehabilitation facility for full recovery.

    I would suggest anyone considering to use Accel at Willow Bend to reconsider your options.

    Accel at Willow Bend is mismanaged. My father has been there for a week, but we are transferring him to a better place. Be prepared for your loved one to wait up to an hour for a helper to come to the room when the resident requests help.

    Sad but true. The staff is nice,,,,but severely understaffed. Do not go there.

    We really appreciated the care my mom recieved while at Prestonwood Rehab. Of course there were questions along the way, however, they continued to prove to be above board and up front with any concerns that we brought to them. It's a great place with great staff. We would definately recommend them to an tone who needs rehab and is not able to care for themselves.

    I wouldn't recommend this facility to anyone. During a meeting with one of the doctors who told us that he had consulted with family member's previous doctor at another facility before transfer to Prestonwood, it was later found out by our family that he had lied to us and had not discussed the treatment plan and that this Prestonwood doctor took it upon himself to discontinue the IV antibiotic one week early. Our beloved family member passed away, and we will never know if stopping the IV was the cause. It is reprehensible that a doctor lies when a beloved's life is literally at stake.

    My husband has been at Accel at Willow Bend for 4 weeks and he and I have only high praises for this facility. From the hands on administrator, staff, PT/OT personnel, nurses, techs, dining room employees and house keepers, we find all friendly and helpful every day, including the weekends.

    the meals are tasty and nutritional. The whole atmosphere is airy and bright.

    I know several men and women who have returned for other issues 2-3 times.

    We highly recommend Accel at Willow Bend.

    THE FACILITY ITSELF IS REALLY NICE. BUT THE QUALITY OF PATIENT CARE IS AT THE LOWEST LEVEL I HAVE SEEN, YOU MAY NOT SMELL URINE AS YOU ENTER BUT THE PATIENT HAS URINE RUNNING DOWN THEIR LEGS. THEY MAY HAVE CALL BUTTONS BUT THEY ARE NEVER ANSWERED. THEY MAY HAVE MED AIDES BUT THE MEDS ARE NOT ADMINISTERED IN A TIMELY MANNER. THE STAFF HAS NO COMPASSIONATE WHAT SO EVER AND IT SHOWS IN THE LACK OF CARE OF PATIENTS. I DO NOT RECOMMEND THAT YOU TAKE ANYONE THAT YOU CARE FOR TO THIS FACILITY. THEY ARE TREATED WORST THAN YOU WOULD A ROTTEN SACK OF POTATOES.

    Wow! I have just discharged from Prestonwood Rehabilitation & Nursing Center and it was a great experience. Obviously nobody wants to go to a Rehab, but when there is not other choice, this is the one to go to. I broke my hip in a car accident and was at Prestonwood Rehab for 6 weeks. There is no way I could have done it on my own. The staff was wonderful. The rehab therapists were the best. I couldn't have asked for more. The stay was not without a hiccup here or there, but simply asking questions resolved any misunderstanding or problems. It was a blessing for me to find this facility and staff.

    My mother is law was at this facility. With myself and my husband being in the health care profession we scrutinized many places. Prestonwood rehab was exceptional. The care was Bar none!, My mother in law loved the food, and therapy was so kind, and knowledgeable. The doctors were present and would even meet my husband or myself for any questions we may have.

    The RN/ LVN's all were well versed in there area, always available for any questions I may have had. If you are looking for a place that is with a home like environment, clean, friendly with excellent care and good food. This is the place, my mother is law is at home now doing better then ever…Thank you to all the staff…

    Don't let the fancy new facility fool you. Always understaffed, lack of communication between staff members, terrible food, wrong items or missing items on meals, huge turnover of aides, long waits for help when using call button, piled excess towels, pillows, etc. everywhere in the room. Meals served late in mornings and never knew when they would be served. Only good thing about the place is the therapists. They are terrific.

    We took mom to Prestonwood Rehab for rehab after a surgery. Prestonwood sales staff told us about their great 5 star ratings, their high Nurse to Patient ratio, and their intense three hours of rehab per day. On the surface they match up with the very BEST facilities out there! Unfortunately beneath the surface is a very chilling reality. Three days into mom’s stay, the surveillance camera we had installed in the room, CLEARLY shows a Prestonwood staffer coming into the room while mom was out. The Prestonwood employee checks behind her, then she proceeds to open the closet where mom’s personal belonging are kept. The Prestonwood employee emerges with mom’s wallet/purse. She hides the wallet behind the folder she is holding, then proceeds to rifle through it looking for cash, cards, etc. Coins begin dropping, and the Prestonwood Rehab Employee is picking up the money as she continues to calmly check behind her. She stands back up and then reveals the wallet that is behind the folder. It is our mother’s wallet! The Prestonwood Employee soon discovers that the purse has nothing much of value, as we had already removed any valuables from the purse. The Prestonwood Employee is seen placing the wallet back into the closet and she leave the room. Fast forward 15 minutes… this same Prestonwood Employee has come back into the room. She calmly sits down and proceeds to conduct a survey with mom and a relative, as if nothing has transpired. Obviously, this was not the first time for this Prestonwood Predator to do this. This evil person is the worst of the worst! She is a predator who targets our most vulnerable, with no moral conscious.

    We showed the video evidence to the management of Prestonwood Rehab. It turns out that this predator is actually Prestonwood’s “Director of Activities”. We were told this issue would be addressed immediately. We asked Prestonwood Management for complete confidentiality, to insure the safety of our mother. The next day, we come to the room and were surprised to find a sign on the door of the room that states…”AN ELECTRONIC MONITORING DEVICE MONITORING THIS ROOM” (With a picture of a camera). So much for confidentiality!

    Just a day or two before this occurred, we had participated in a meeting regarding a rehab plan of action for mom. The management had scheduled a progress meeting in two weeks. A couple of days after our reporting the abuse of this Prestonwood Director, we were called in for a meeting. We were informed by Prestonwood management that they had done all they could for our mother and suggested that we take mom to a different facility immediately. They stated that they could only provide 30 minutes of therapy per day and she needed more. What happened to the THREE Hours of intense therapy. This was obviously retaliation by Prestonwood for finding them out! GOOD NEWS - We moved mom to a REAL Rehab facility and after another week of actual REHAB, we were able to take her home, where she is doing well. Obviously, patients are welcome at Prestonwood Rehab, as long as they do not need rehab, or find Prestonwood Rehab employees abusing your loved one. Avoid Prestonwood Rehab!

    I have lived here for 17 months. We recently got a new administrator, which I am grateful for. The previous administrator was running the facility with money being more of a concern then the residents and the staff. We were having to fight for rights we already have. Privacy was thrown to the wayside. Staff were not knocking before coming in. Our dementia residents were just walking in to our rooms and not leaving. I personally had 25 visits one day, 17 from one resident.

    My mother was a resident at Life Care Center of Plano. She was very close to death and I wasn't sure how long she had to live. She ended up being a resident there for over 3 years. I know that her quality of life was extended by the staff there. As time went by the staff became more and more familiar with my mom's needs.

    I know that I wasn't the easiest person to get along with as I loved my mother very much. I wanted to make sure the staff did the right thing for my mom and they did. I had my share of concerns and they dealt with each one and helping me feel involved and important.

    The nurses and cnas loved my mom very much and I could feel their caring attitude when she did get sick. The kitchen staff always tried to make mom happy with the food that she liked.

    At some point, I knew the inevitable would happen and it did. Mom's strength gave out and it was time for her to go. It was a difficult time to let this happen. The staff were very attentive to my needs at the end of her life and ensured that all ended well. I can't thank the staff enough including management and administration for their caring attitude. My mom and I were both blessed by her stay.

They were all professional, very pleasant and easy to get along with. Thank you to all, very sincerely. It is never pleasant to be away from home but your staff made it easier.

    My mother entered Life Care Center of Plano for rehabilitation after having a liver resection. The Center is nice enough and everyone seemed friendly and concerned at first, but I believe it was just a cover-up for bad business! We chose this place over others because it looks nice and because of it's 5 star rating and the review in US News and World report. However, we did not readmit her to Life Care Center after she left the hospital because we feel that she was not well cared for.

    Mom was in Life Center Plano for a little over 3 weeks and she did not get better, but got progressively worse. When we discharged her from the center she had to go back into the hospital where we were told that she had:

    1)Pneumonia (even though an x-ray taken that morning at the center showed her lungs to be clear). The ER doctor informed us that it looked like she had had Pneumonia for about a week

    2) a urinary tract infection (the center had inserted a catheter 6 days prior because she was taking Lasix)

    3) Cellulitis (we had been trying to get the nursing staff to look at her legs all weekend)

    Also on the morning of her discharge a MALE staffer was sent in to bathe my mom. When I let him know that I did not want him to bathe mom he exchanged residents with a female staffer, but said that he had bathed mom before.

    The therapists seemed to be doing what they were supposed to be doing, although mom was not progressing as fast as we hoped she would.

    The nursing staff seemed indifferent , but would come to mom's room if someone insisted.Unfortunately the weekend staff was not even kind enough to do that; we had to bring to their attention that she was coughing and that her legs were very swollen and red. They did eventually call the Dr for permission to put mom on antibiotics and cough medicine.

    The non-nursing staff seemed friendly enough, the food was good and they did try to have activities planned for the residents.

    From our vantage point it seemed that no one at the center was proactive in making sure that mom was doing okay.

    What we have learned is that you cannot be polite and assume that they are going to do what they should be doing or even what they promise you that they will do. You have to stand up for your loved one and make noise if noise is what it takes.

    Mom is now in another rehab center and we will insist that she get better care. So far so good…….

    The turnover here is ridiculous. My Dad lived here for 6 several months after a stroke. I chose this facility because of medicare.gov reviews. I was FOOLED! The reason for their high reviews is because they only accept low-level care patients… recovering from a knee injury, etc.

    My Dad has paralysis on half his side… this place was supposed to move him to his side every few hours… never happened. He developed LARGE bed sores… he never got oral care, making my dad's comfort level atrocious. The RNs only do paperwork. They rarely do anything with patients. One time my Dad's mouth was so disgusting, I went to the RN (NOT a CNA/LVN) and she said she'd have someone come clean his mouth. An hour later, nothing was done. Furious, I went to the nurses station and demanded the Director of nursing come IMMEDIATELY. At 11pm, he had nothing to say… no reasoning….

    They never had family meetings to discuss treatment… speaking with their DR was ridiculous. She never responded and the RNs, always said she was not available… and she never returned calls. I called the director and demanded a meeting--- it took 2 weeks before I got a meeting.

    When I had enough, and had my Dad discharged… NOBODY was there to discharge him. The nurse couldn't let him go…. they hadn't signed the proper paperwork… The admin in the front was in charge of admissions and discharges…. an ADMIN!!!!! I told both of the admissions about my experience… they both went to a different facility.

    Finally, when the paperwork was signed… they tried to slip in some paperwork that made ME financially responsible for his care there… AND a release of liability! WOW!!!!!!!!

    I've found this to be a good facility. The staff seems to be very caring and the atmosphere is much like a close community. My brother has been at this facility for years and is well taken care of. I would say the only issue I have had is occasionally some personal items disappear from the room, but that is not unusual in this industry.

    My mother was recently a resident for two weeks. I was surprised that the center considered itself a five star. My mother did not get good care. Fell twice becuase she was not checked on, was not eating or drinking adequately and they did not have the proper equipment in her room for her to get out of her bed safely. I ended up taking her out of this place and immediately ended up at the hospital with her. She is being treated for improper nutrition and things that were not attended to at this facility. There are a few good people there (like the social worker, chef, and speech therapist) but not many. Ignore the red call buttons constantly too. Also will not answer the phone at the nurses station when you call.

    My Dad stayed at 3 nursing homes during his illness. This was the WORST. The best was Brookhaven in Carrollton. MY Dad was on total care and got bed sores at this facility. It go so bad I was going to call the Police. I called in the nurses and their response was terribly slow. It took them 30 mins to come look in his mouth and see that it had not been clean in Days (Should happen hourly) THey don't document their failures. It's HORRIBLE. My Dad died right after being moved out and I attribute it to this company. It's horrible. Understaffed. There was a director that was fired when my Dad moved in… and his replacement was gone 6 months later. Very understaffed. Unprofessional. I have to live with having lost my Father because of this place… for the rest of my life.

    My mother spent the last year of her life at Prestonwood rehab, and I am eternally grateful we found the facility. The facility is large and airy, clean and fresh smelling with wonderful modern amenities. After we got Mom established there she asked me "Why didn't you bring me here sooner?". Mom came to Prestonwood for rehab of a broken leg. The administration arraigned for Medicare payment, it did not cost us a dime. Mom stayed in a private room with a lovely view. Mom loved rehab - her whole life she was a loner, I could never get her to participate in any clubs or activities. Suddenly, she had friends and looked forward to getting up in the morning. In her prior nursing home she stayed in her room all day, but at Prestonwood, with the hard surface hallways and friends to visit, Mom was out and about tooling herself around in her wheelchair by her own power. My siblings and I were stunned - she was a new person. When her rehab ended we stayed with Prestonwood, but Mom was sad she would not get to exercise with her friends: her coverage had run out. I brought this up at Mom's staffing, and within a few days Mom was back in the Rehab room - not receiving the same service, but exercising on her own with supervision, and enjoying the company of her new friends. This was above the call of duty for Prestonwood. We had some slips and communication problems, but I was able to go straight to the charge nurse and get the problem solved. There was a difficult communication issue caused by Mom deafness, but the facility wired her room with a special feed that supported communication equipment she needed. The facility director personally supervised the effort. When Mom's time came, the nursing staff gave us exceptionally good advice, balanced, thoughtful and on point. Mom got into hospice care at the facility and passed away with her pain in control. Thank you Prestonwood Rehab.
