    All these beautiful people, that can walk well always seem to have to use a wheelchair and walkers for no reason at all. What is the purpose of this? Could it be because they want to milk there insurance companies so they can stay for free at The Laurel's Of Huber Heights and to get free meals. I truly don't understand when there are other people that could really use them and they have no extras.

    I have seen these people who don't need wheelchairs or walkers using them as a play toys, racing around in there wheelchairs popping wheelies, and then walk away from there walkers and just walk around like nothing. Also I have seen them also push others around in there wheelchairs. This is just plain ridiculous! When we have our laundry done and it's brought back to us, there is always something missing. I feel laundry employees and or other staff members all taking them home. This is Theft! That's wrong when we pay for our own clothes not them! Leadership should spend more time talking to the residents to see how everything is going, like is there something we can do to make you feel more at home etc. Activities Department should go around to the residents and ask them what kind of activities do you enjoy doing? Taking care of all the residents here at The Laurel's Of Huber Heights is more important making sure everyone is happy being here, except on how much money we are making. If there were no residents here, the business would close. Is it all about making money or having no residents, because a lot of residents are ready to go somewhere else I have never seen a business ran like this in my life. So poorly ran, working only 8 hours a day for Management is not enough time to resolve all the issues that's going on here. Maybe I should just call corporate and let them know what all is going on here. My family and so many other families are so disappointed in the way things are ran here. It needs to change for the better or close it or sale it.

    Extremely disappointed. We removed our family member after five days without getting a release from the facility. Senior staff are incompetent and dishonest. Failed to provide essential medication for FOUR DAYS. Different story from every person we asked - from the "Pharmacy is closed", to "its a holiday", to "I don't know". When we mentioned we wanted to move to another facility (and told them the name and phone # of whom to contact) they slow rolled us, and told us a number of reasons why we couldn't move for several more days. It was all a lie to keep us there longer. Stay away. Dangerous environment.

    Do not believe their Mission Statement. Poor nursing services. No follow-up on medical treatment orders. Son was there for pressure wound, but also had other medical issues. Catherer was placed incorrectly had to be hospitalized and damage was done from nursing staff not being properly trained, and lack of follow-up from nursing supervisor. Short staff for aides and used Pool Nursing staff. All compliants were stated to CMS. All this occurred in 2016, and after seeing a news reporting last week on TV poor management still is continuing. What is it going to take to get improvement in this facility?

    This nursing home is horrible! I have walked in on a family friend covered in filth and food. His oxygen mask was thrown in a drawer; his diaper soaked with his own urine! He has bed sores all over him from sitting and laying in bed too long! Their Stna's were yelling in my face and point in my face. She started digging in the trash can to find a dirty diaper and proceeded to wipe is over my family friend! We took him right out of there he is in a hospital now and is getting cleaned up. I WILL NEVER BRING A FAMILY MEMBER OR A FRIEND HERE EVER AGAIN. This place is awful and disgusting. Plus there are bed bugs!

    I am a current resident of laurels of west carrollton and after being here and numerous times submitting concerns getting meds on time rather it be the nurses here need more and or better training because they don't know proper hand out protocol (time or timeframe which to administer)…documenting that myself and another resident received medications and thus we DID NOT, and these are not narcotics or any form of pain medication what so ever in fact one was insulin and the other a blood thinner….or in a lot of cases passing meds at their (nurses) leisure because the on cell phone, ordering food, godsipping among themselves or simply not having a good day and pass meds when they (Nurses) feel like it! It discuss me! This facility is extremely LOUD contributed by staff yelling and talking awfully load from one end of hall way to the next…in middle of hallway, many times in front of my door where I had to get up out of bed and close my door! How rude and unprofessional…or simply at nursing station entertaining themselves instead of answering call lights of residents…. instead in hear "oh he or she don't want nothing" incredible!!!! I wouldn't recommend the laurels of west carrollton to a soul. I'm highly disappointed.

    I would not recommend this nursing home. The majority of staff members do not seem to be patient-focused, nor is staff morale very high at this facility. The food service is the worst, and rehabilitation services (PT and OT) are mediocre at best with limited patient progress for those with the greatest needs. Social services and medical service standards are lacking as well.

    The facility is a beautiful "show place" but lacks in personal care. Resident had to lay in urine and feces for several hours before aids would assist. They send residents to special doctors off site without assistance where they have to sit in waiting areas alone for several hours. Nurses aids very unfriendly to residents and the family and do not explain anything to the residents. Nurses aids do not have good communication on the health conditions of the residents. A smile and good customer service goes a long way. We are treated better from the "Wal-Mart greeters", than here. Some of the RN nurses were good. We were the ones that found a cancer spot on resident, that should have been detected by nursing staff long before.

    The food is disgusting & isn't nutritional. People with a heart condition should never be served food with gravy! They have a tendency to serve everything with gravy which I am sure is too hide the taste of the food. all too often the food is cold,the bread is hard & the meat is so hard you can't chew it!Don't expect to get what you ordered. The staff is nice but, as in most nursing homes the staff patient ratio is way too low!

    Do not use this facility. My mother was treated poorly and not given appropriate comfort measures when she was there. When medical concerns were brought to the attention to the nurse she sputtered and gave excuses.

    Food was brought to the room and left on the dresser. My mother was bed bound so couldn't get out of bed to eat and the staff knew she needed help. Several times when we arrived her food was cold and still sitting on the dresser. We witnessed the STNA's standing over my mother yelling at her that she needed to eat however my mother was laying flat in bed and unable to sit up to eat.

    We witnessed other people yelling at the STNA's that dirty linens had been left in their loved one's room for over a week and they were supposed to be washing them at Heartland.

    When we gave specific instructions to the nurse she later acted like we hadn't told her anything.

    My mother got weaker and weaker until hospice was called in. Hospice had a nurse for her at heartland on a daily basis until the hospice nurse told us their shock at the lack of care Heartland gave. They recommended we move mom asap and amazingly enough mom improved health wise and spirit wise almost immediately.

    There were a rare few staff members that truly seemed to care about the patients but overall I wouldn't send my worst enemy to this place for care.

    Again.. don't use this facility.

    I rate this a very poor based on the fact that they let my brother get ill and by the time they got him to the hospital he was already near death with an infection throughout his body. He did not survive. If you put your relative here demand they go to a hospital when you suspect they have not done everything they should be doing to find out what is causing a resident to jump as if in pain, if the resident is running a temperature. X-Rays don't tell anything, so demand further help for your loved ones or lose them like I did.

    I give this nursing home a 3 Star. The manger that runs the nursing home was very polite and helpful. However, some of the nurses were more than condisending when it was decided by our family to take our family member home. It seemed to me that they felt that they were a better judge of what should be done than our family. In fact, one of them told us we could NOT take him home. Further, they told us someone would be coming out to the home to inspect the safety of the home. Again, it was told to us in a rude and condisending way.

    my family member was at HOC for rehab following a CHF episode. The therapy team overworked him, causing him to have a heart attack. They ignored his pleas for help, falsified documents about the incident, and had another family member not run to his aid after hearing distress when she called him, he probably would have died. She couldn't find anyone who would admit it was their job to care for him. He was there for 8 days and lost 12 pounds due to the horrendous food. She demanded a medic be called. The state was called for an investigation, and that is when the falsification about his care was discovered. We had no recourse because it was a "word-against-word" situation. When he was released from the hospital after 4 days to another facility, 4 people were eagerly waiting to move him out so they could move someone else in . Where were these 4 people when my family member was calling for someone to help him? Please, PLEASE do not send someone you love to this facility. It could cost them their life.

    This is my second stay at Heartland of Centerville. I have used the facility both times for rehabilitation and my stay for both times was six weeks. I received caring and successful rehab and excellent care from the nursing staff and aides. At Heartland of Centerville you feel like part of the family and form lasting relationships. If I have to ever go back I will definitely use Heartland of Centerville for their excellent care!

    Tonight we went to visit my fatherinlaw and was in quite shock over what we seen. The date is 12/25/13 and at about 730pm a black female nurse came into his room and proceded to wrestle around with a resident there. She kissed him, told him she loved him and acted like he had her in bondage. SHe shut the door and slightly pulled the curtain and proceded to lap dance him all in the view of my 10 yr old daughter. Was this a previous prositute that is now a nurse? If I had a clue who to report this to I certainly would. Totally inappropriate!

    Our Dad received overall wonderful care. The nursing staff was excellent, always keeping us informed and involving us in his recovery from his surgery. We were very anxious and did not know what to expect. The head. of nursing always made time for us.stopping in to talk with us and check on Dad,he called her his girl. The.therapist were also very good- pushing Dad when he needed it in the end he was thankful. Food was a different story,could have used improvement, we frequently brought food from home..We will return if Dad needs nursing care, and reccomend Heartland Of Centerville to our friends.

    My 85 year old mother was admitted today post-op for broken hip. My sister was there with my mother upon admission. The staff was extremely rough when moving her. When I arrived at 6:30pm, her dinner plate was on her bedside table across her bed. It included cake. My mother is a diabetic. When I asked about glucose monitoring, I was told nothing was ordered. The website touts excellent diabetic care. My sister and I left at 7:30 to go eat. When I came back at 8:30, my mother said she was in pain. I pushed the call light and when the aide came I asked for something for pain. After 20 minutes my mother looked up and said "it hurts". I told her that I knew she was in pain, but they were going to bring her something. She then said "no, do I have to stay on this bedpan all night long?" When I questioned the LPN on duty who told me she was the charge nurse, she immediately took an attitude with me. She said she didn't know that my mother had no idea what a call light was. When I asked for her supervisors name, she said "Yes, I have a supervisor". Horrible, horrible place. I will do what I can to get my mother moved from this facility. I had to choose a minimum of one star because it won't let me submit this review without it.

    My Grandmother lived here for a short while. She was independent living before she had her accident. She had a wound on her leg and also suffered from dementia. The nurses would forget to give her her medication and only would give her parts of it. Her aid did not know her name and did not remember that she had a wound on her leg but only could point to what hallway she was in charge of, The aids do not care about the patients they only care about completing tasks. They lost all of my grandmothers clothes and shoes. Do Not Go Here!

    we were very happy with the care and pleasantly surprised with the level of commitment shown to our mother. she loved the nurses and aides as well as her therapists. they were dedicated to getting mom not only back home taking care of herself as she had befoe but they worked towards mom maintaining her level of independance. we will definately go back if anyone in our family needs nursing care.

    This is a great facility. The therapy department is top notch. I was only there for 30 days and I am back to where I was before surgery. The nurses were compassionate and the aides were very caring as well. I would recommend this rehab center to all. Only area of improvement would be the food. Not as good as the hospital.
