    All these beautiful people, that can walk well always seem to have to use a wheelchair and walkers for no reason at all. What is the purpose of this? Could it be because they want to milk there insurance companies so they can stay for free at The Laurel's Of Huber Heights and to get free meals. I truly don't understand when there are other people that could really use them and they have no extras.

    I have seen these people who don't need wheelchairs or walkers using them as a play toys, racing around in there wheelchairs popping wheelies, and then walk away from there walkers and just walk around like nothing. Also I have seen them also push others around in there wheelchairs. This is just plain ridiculous! When we have our laundry done and it's brought back to us, there is always something missing. I feel laundry employees and or other staff members all taking them home. This is Theft! That's wrong when we pay for our own clothes not them! Leadership should spend more time talking to the residents to see how everything is going, like is there something we can do to make you feel more at home etc. Activities Department should go around to the residents and ask them what kind of activities do you enjoy doing? Taking care of all the residents here at The Laurel's Of Huber Heights is more important making sure everyone is happy being here, except on how much money we are making. If there were no residents here, the business would close. Is it all about making money or having no residents, because a lot of residents are ready to go somewhere else I have never seen a business ran like this in my life. So poorly ran, working only 8 hours a day for Management is not enough time to resolve all the issues that's going on here. Maybe I should just call corporate and let them know what all is going on here. My family and so many other families are so disappointed in the way things are ran here. It needs to change for the better or close it or sale it.

    Extremely disappointed. We removed our family member after five days without getting a release from the facility. Senior staff are incompetent and dishonest. Failed to provide essential medication for FOUR DAYS. Different story from every person we asked - from the "Pharmacy is closed", to "its a holiday", to "I don't know". When we mentioned we wanted to move to another facility (and told them the name and phone # of whom to contact) they slow rolled us, and told us a number of reasons why we couldn't move for several more days. It was all a lie to keep us there longer. Stay away. Dangerous environment.

    Do not believe their Mission Statement. Poor nursing services. No follow-up on medical treatment orders. Son was there for pressure wound, but also had other medical issues. Catherer was placed incorrectly had to be hospitalized and damage was done from nursing staff not being properly trained, and lack of follow-up from nursing supervisor. Short staff for aides and used Pool Nursing staff. All compliants were stated to CMS. All this occurred in 2016, and after seeing a news reporting last week on TV poor management still is continuing. What is it going to take to get improvement in this facility?

    This nursing home is horrible! I have walked in on a family friend covered in filth and food. His oxygen mask was thrown in a drawer; his diaper soaked with his own urine! He has bed sores all over him from sitting and laying in bed too long! Their Stna's were yelling in my face and point in my face. She started digging in the trash can to find a dirty diaper and proceeded to wipe is over my family friend! We took him right out of there he is in a hospital now and is getting cleaned up. I WILL NEVER BRING A FAMILY MEMBER OR A FRIEND HERE EVER AGAIN. This place is awful and disgusting. Plus there are bed bugs!

    I am a current resident of laurels of west carrollton and after being here and numerous times submitting concerns getting meds on time rather it be the nurses here need more and or better training because they don't know proper hand out protocol (time or timeframe which to administer)…documenting that myself and another resident received medications and thus we DID NOT, and these are not narcotics or any form of pain medication what so ever in fact one was insulin and the other a blood thinner….or in a lot of cases passing meds at their (nurses) leisure because the on cell phone, ordering food, godsipping among themselves or simply not having a good day and pass meds when they (Nurses) feel like it! It discuss me! This facility is extremely LOUD contributed by staff yelling and talking awfully load from one end of hall way to the next…in middle of hallway, many times in front of my door where I had to get up out of bed and close my door! How rude and unprofessional…or simply at nursing station entertaining themselves instead of answering call lights of residents…. instead in hear "oh he or she don't want nothing" incredible!!!! I wouldn't recommend the laurels of west carrollton to a soul. I'm highly disappointed.

    This review may be lengthy, but I assure you, if you have a loved one that is seeking long-term placement or a short rehab stay, you will want to take the time to read it. I have worked full time at Kingston of Miamisburg for 6+ years. I have worked both night shift and day shift.

    First of all, this facility claims to "specialize" in respiratory therapy. This itself is laughable. "We have a respiratory therapist on staff 24/7" is what is marketed to the public. ABSOLUTELY NOT TRUE!! After 11p (and most of the time much earlier, say 930p) and before 7-8a the nurse is the one left to deal with respiratory issues that a lot of times, he/she isn't even qualified to do! And good luck if their nurse is trying to reach someone on the phone for an emergency during these off hours! It seldom happens, but if you ARE lucky enough to get an answer or return call, it's about an hour before anyone reaches the facility. And with an attitude. Trachs get dislodged/pulled out/coughed out all the time. People often times get weaned from their trachs too early. Which just sets up more problems for the patient. This is not to even mention how lazy these respiratory therapists are. Oh my God! You would think someone making $30 per hour would be happy to have such a job. Not all are lazy, but seriously, most are.

    Working short staffed is a common problem anywhere you work. Any field. But in this type of facility, adequate staffing is a MUST! But not at Kingston! Expect the facility to be short staffed nearly every.single.day. Mildly to severely. DAY SHIFT AND NIGHT SHIFT! And please don't expect anyone to come out of their comfy little office to help out. The last I heard someone in the office be asked to come to the dining room and help feed patients? "I'm getting really tired of this!!" No joke folks.

    You also have here at Kingston some great nurses and some really really bad/incompetent/lazy nurses. The latter, I'll go with them. Does your family member have a bed or chair alarm for safety? Better keep an eye on that! Your nurse doesn't want the aggravation of an annoying alarm sounding frequently. Does your loved one have a treatment? Doesn't matter how bad/infected/draining it is. Those don't get changed for days at a time! Don't believe me? Start checking! You need their nurse? Excellent chance they can be found in the smoking area. Doesn't matter if it's an emergency, or the Dr calling, or a pharmacist checking on their medications. Doesn't matter. Not one bit!! At least once an hour around the clock for about 15 minutes per cig. Smokers 24/7. And as far as having an RN on staff at all time? That is also laughable. DOES NOT HAPPEN!

    Let's talk about those friendly and not so friendly nursing assistants. Pretty much the same as the nurses. Some great and some absolutely not. Day shift throws responsibilities off to night shift and night shift reciprocates. So who is getting left out of the equation? Your loved one! Their supposed to get a shower twice per week or more often? Most likely doesn't happen. Bed alarms? Same thing. I've seen many many falls/falls with injuries happen because someone didn't answer a call light soon enough for the patients liking. Some justified, some not. Some folks are just too impatient and refuse to wait for help or doesn't call for help at all.

    But some nursing assistants like to sit and chit chat with other staff/play on their phone/smoke too much to be bothered with that call light. And most of the staff, and I mean MOST, are big time buddies or sleeping with each other so they dare not tell on each other. I really don't think they care enough to anyway. It's all covered up by the buddy system. And by all, I mean all.

    Those little white badges they make us wear? The Versus badge? When the smokers need to light up they just take those off and toss them to the side, usually their desk to make it appear as though they've been hard at work. Management knows all about this too but nothing is ever done, because, u see, it happens on both shifts. And management smokes, also.

    I'll just put his here as a short comment: folks in the admissions office will accept ANYONE into this facility. Appropriate placement or not. Especially if their census is down. Some of these folks that are not appropriate and require one-on-one care (such as Alzheimer's patients/combative/high fall risk, etc) do just that. Take a nurse or nursing assistant off the floor to personally sit with you to keep you safe and off the floor from a fall. Leaving the other residents at risk due to short staffing. But keep that census up $$$.

    The turnover rate? Beyond laughable. I've literally seen hundreds of nursing staff come and go. No joke folks. Nurses, nursing assistants, DON's, Team Lead/Supervisors. And when the State of Ohio comes to the facility for their annual survey? Everything is in tiptop shape. Until they're gone.

    Now, I'm sure as you read this your thinking, if you've been there 6+ years. Why haven't you reported these things? Why do you still work there? Or various other questions I may have riled up in you. 1) I have reported all the above mentioned. AND MORE! Numerous times. Doesn't matter. 2) I love the residents.

    Needless to say, I would seriously think about having a loved one stay at Kingston. I don't even refer people to this facility. That's sad.

    I would not recommend this nursing home. The majority of staff members do not seem to be patient-focused, nor is staff morale very high at this facility. The food service is the worst, and rehabilitation services (PT and OT) are mediocre at best with limited patient progress for those with the greatest needs. Social services and medical service standards are lacking as well.

    Ask how many falls, near falls, found on floor, lowered to floor events. Don't just ask for "falls" as in medical-wise words that means witnessed fall to the floor only. What is the nurse to resident ratio on each unit, as well as aide to resident ratio. Have you had any state inspections, including visits due to complaints. Staff is burned out, high turn over rate, no mgmt on site except during bankers hours.

    The facility is a beautiful "show place" but lacks in personal care. Resident had to lay in urine and feces for several hours before aids would assist. They send residents to special doctors off site without assistance where they have to sit in waiting areas alone for several hours. Nurses aids very unfriendly to residents and the family and do not explain anything to the residents. Nurses aids do not have good communication on the health conditions of the residents. A smile and good customer service goes a long way. We are treated better from the "Wal-Mart greeters", than here. Some of the RN nurses were good. We were the ones that found a cancer spot on resident, that should have been detected by nursing staff long before.

    I was in this facility for almost 9 months. Before I entered as a resident, they took me on a tour. Very impressive as they take you through the new part of the building. Both halls had private, semi-private, and rooms that housed 2 patients. They showed me their indoor heated swimming pool and the physical therapy room. Nurses would come up to me with big smiles and say, "Hope you come visit us a while." After the tour and the interview, I called them 2 days later with my decision to accept their offer. When I arrived a liaison met me and guided me to a part of the facility that was not included on the tour. I passed room after room in the new section that had several rooms that were unoccupied. Surely I waited to be led into one of those rooms but we kept going. They took me all the way back to the old section of the facility. As soon as we got in the hallway wing, the stink of feces and urine permeated the hallway and my eyes began to water from the overtaking odors. They led me into a room with an 88 year old man that was deaf as a doorknob and had his television cranked up high. They took me to the bed by the window. They showed me the closet I had to share and the bathroom. By 20 minutes after being there the smell was nauseating me and I was having gag reflexes from the odor. I asked them about the rooms in the new section and they told me those were reserved for short term patients. I was having regrets about moving there. 2 weeks into the stay the man I had to share the room with started waking me up all hours of the night cussing me out and telling me I had to get out of his room. After begging and pleading in tears, they finally moved me to another hall. It wasn't any better. I noticed the aids setting around in the lounge discussing health issues about other patients in front of other patients who were in the room. They bitched about how bad they hated their jobs and how they hated cleaning patients who were incontinent. Come to find out some patients hadn't been cleaned for several hours. Dementia patients were left roaming the halls and going in and out of other patients rooms completely ignored and un-attended. I had one female patient that kept coming in my room all hours of the night waking me up trying to get in my bed. I would hit the call button and sometimes it would take them 1/2 hour to 45 minutes to answer the call. In the meantime I had a female patient in my room, pulling the covers on my bed with me in it. It seemed like I had to put out one fire after another. I never got my meds on time. Many times I had to get up and chase the nurse down who was usually setting in the lounge or outside smoking a cigarette. Let me for warn anyone thinking about putting an elderly loved one in Kingston facility. They only show you the shiny part of the facility but 98% of the time they were put in what I called "The Concentration Camp Halls". One time they put a man in the room with me that had open MRSA. He was suppose to be in an isolated room. They tried to convince me that I was safe. I went online to the CDC website and read what was suppose to be done with a person who had Open Contact MRSA. When I confronted the Nursing Director they told me that was an old practice and was not done anymore. I called the Ombudsman's Office and they sent a person down to see the situation. The facility was definitely in violation of CDC practices for MRSA patients. After that I got put on the dark brown smelly stuff list. Needless to say, I got out of there as quick as I could.

    The food is disgusting & isn't nutritional. People with a heart condition should never be served food with gravy! They have a tendency to serve everything with gravy which I am sure is too hide the taste of the food. all too often the food is cold,the bread is hard & the meat is so hard you can't chew it!Don't expect to get what you ordered. The staff is nice but, as in most nursing homes the staff patient ratio is way too low!

    My dad was at Astoria for eleveen months. He is extremely oxygen dependant. Almost everytime I came to visit him his portable tank was empty. For eleveen months, I spoke with the nursing staff, director of nursing, assist director of nursing, and even the administrator on numerous occasssions. I was always told they were checking it. At one point the pulmonary physcians nurse came up with the idea for a timer to be set so they would know when they needed to change it. They still did not set the timer, and nursing staff did not even know what I was talking about. One day, the ADON herself told me the reason his tank was on empty was because they was short staffed!? Needless to say, he has been moved to a facility that so far has not let his tank run empty.

    Do not use this facility. My mother was treated poorly and not given appropriate comfort measures when she was there. When medical concerns were brought to the attention to the nurse she sputtered and gave excuses.

    Food was brought to the room and left on the dresser. My mother was bed bound so couldn't get out of bed to eat and the staff knew she needed help. Several times when we arrived her food was cold and still sitting on the dresser. We witnessed the STNA's standing over my mother yelling at her that she needed to eat however my mother was laying flat in bed and unable to sit up to eat.

    We witnessed other people yelling at the STNA's that dirty linens had been left in their loved one's room for over a week and they were supposed to be washing them at Heartland.

    When we gave specific instructions to the nurse she later acted like we hadn't told her anything.

    My mother got weaker and weaker until hospice was called in. Hospice had a nurse for her at heartland on a daily basis until the hospice nurse told us their shock at the lack of care Heartland gave. They recommended we move mom asap and amazingly enough mom improved health wise and spirit wise almost immediately.

    There were a rare few staff members that truly seemed to care about the patients but overall I wouldn't send my worst enemy to this place for care.

    Again.. don't use this facility.

    I rate this a very poor based on the fact that they let my brother get ill and by the time they got him to the hospital he was already near death with an infection throughout his body. He did not survive. If you put your relative here demand they go to a hospital when you suspect they have not done everything they should be doing to find out what is causing a resident to jump as if in pain, if the resident is running a temperature. X-Rays don't tell anything, so demand further help for your loved ones or lose them like I did.

    I give this nursing home a 3 Star. The manger that runs the nursing home was very polite and helpful. However, some of the nurses were more than condisending when it was decided by our family to take our family member home. It seemed to me that they felt that they were a better judge of what should be done than our family. In fact, one of them told us we could NOT take him home. Further, they told us someone would be coming out to the home to inspect the safety of the home. Again, it was told to us in a rude and condisending way.

    my family member was at HOC for rehab following a CHF episode. The therapy team overworked him, causing him to have a heart attack. They ignored his pleas for help, falsified documents about the incident, and had another family member not run to his aid after hearing distress when she called him, he probably would have died. She couldn't find anyone who would admit it was their job to care for him. He was there for 8 days and lost 12 pounds due to the horrendous food. She demanded a medic be called. The state was called for an investigation, and that is when the falsification about his care was discovered. We had no recourse because it was a "word-against-word" situation. When he was released from the hospital after 4 days to another facility, 4 people were eagerly waiting to move him out so they could move someone else in . Where were these 4 people when my family member was calling for someone to help him? Please, PLEASE do not send someone you love to this facility. It could cost them their life.
