    I will be filing a complaint with the health department. My mother received minimal care. I had to ask for everything--several times-- as regards to treatments and meds she was to receive. Most of the time I was answered with "I don't know" or "let me check" and then the staff person would never come back! Finally after asking 3-4 times over several hours I'd get an answer--maybe.She was supposed to be at WB-Lincolnshire for 2-3 weeks after her surgery. I moved her after 2 days when I realized I was giving her more care than the staff and I have no training to do so.

    The excuse I was given when I could get someone o talk to me was "they're in transition" from Wealshire. Not good enough. They shouldn't be open if they're not ready for patients.

    Please do not take your loved one to this facility. What I have experienced at this facility is not acceptable quality measures for anyone to go through. This facility has had quiet a few violations from state surveys in the last year. Not enough staff for shifts and are always short handed and the nursing staff have too many patients in one shift for one and one care that the patient deserves. High turnover rate in management and is never at fault for anything they may do wrong. Please be very careful if choosing this facility for your loved one.

    Many changes from the past reviews. Our family was involved with Brentwood North in Summer of 2014. Items were taken during the night, call button went unanswered for more the 30 minutes. And falls were a problem. Many bruises from the falls and not being watched clearly. No bath or shower for more than 6 days. Food was not fit for people who were trying to improve strength. Many of the staff do NOT speak enough English to communicate with people. Billing problems over and over. Things that were not done were billed or items were double and triple billed. Bedding was not changed daily nor was the room cleaned on a daily basis. I wish I had visited the rehab before sending our family member to the facility. Now there are so many more emotional scars to deal with on top of the health issues. It was such a mess we moved our loved one to another facility not affiliated with Brentwood.

    If I could give this facility zero stars, I absolutely would! This place is horrible, and I'm completely unsure of how it is still operating! The staff is very scarce and doesn't respond when the call button is pressed. We had a few incidents about which we hoped to speak with an administrator in an amicable manner, but the response we received from her was rude, uncaring, and unprofessional. Several times when I walked in unannounced, I found my mom sleeping in feces and urine.

    We have also spoken with other residents' families and their experiences have not been favorable either. Other families also spoke of abuse and neglect, which I can believe, given the rumors. Please do your research (there are plenty of articles online regarding incidents that have occurred here) before deciding on making this place your loved one’s home.

    I'm glad we chose to move my mom from this place to another, but wish we had known about how unclean and unsafe the environment is, how rude the staff is, and what nonsense goes on here earlier.

    Most clinical staff competent and caring, some exceptions, issues addressed when presented to Supervisor. Often understaffed, staff working doubles to cover - burns them out! Administration is disconnected from the mission - encountered rudeness and uncaring attitude. Best advice - visit as often as possible and let your presence be known!

    Claridge Nursing home is the worse place on the planet

    The first night he arrived we were told that he was unable to get out of bed because they wouldn't give him a walker. The reason was that he had to wait 24 hours to be accessed to find out if he needed one. We knew he needed one because he had just spent 5 weeks in the hospital learning to use one and was very capable, The first night he got up and began wandering the halls without a walker. He was and still is very confused and forgetful and wouldn't stay in bed, So he wandered the unfamiliar halls looking for a bathroom. Lining the narrow halls are chairs, people in wheel chairs and people milling about so there are many things to trip over, The staff are rude and incompetent and ignore the patients,especially the ones that are bedridden, The room was filthy.

    I've waited to write this review because I wanted to give the owner an opportunity to remedy the situation before I went ahead and wrote a less than favorable review. And I've done this on more than one, two, or even three occasions. I'm constantly letting her know regarding the situation at her business. she's very diplomatic. she tells me she will look into it and that we'll work together to rectify the situation. However, this hasn't been the case time after time again.

    I've given up. I go everyday to make sure that my father are as well cared for as they can be. I will do some of the cleaning which should be done on a daily basis -- washing mugs for instance.

    Claridge Healthcare Center has a below average registered nurse per resident per day of 29 minutes compared to the Illinois state nursing home average of 53 minutes. They had 30 deficiencies in its past 2 state inspections compared to the IL average of 11 and complaints compared to the IL average of 3. Claridge Healthcare Center had 19 fire safety deficiencies in the past 2 state inspections which is worse than the Illinois state nursing home average of 12.

    While my loved one was in The Grove at the Lake, I watched not only the care my loved one received, but the care of other residents. I noticed that the residents were placed in wheelchairs and placed in the hallway and some allowed to scream and cry without any attention given to them, or placed in their beds in the middle of the day. There is a hispanic nurse who is VERY disrepectful to the CNA'S and barks out orders that she could very well do herself. Very rude Director of Nursing, who is also hispanic, and will stand and argue with you if you disagree with one of her decisions she has made for your loved one.

    The CNA's are excellent, OVERWORKED, UNDERSTAFFED, and this is due to a few of the Nurses who think they are above wiping a dirty bottom or helping a resident get to the washroom to avoid this happening.

    My Mother was moved into a room with a woman who could not speak English and incessantly screamed the same words over and over…needless to say, My Mother went without rest the night she was in the room with her, when I found out about it I went to the Director of Nursing and told her my Mother had to be moved, (she had 2 days left to be released) and her response to me was an argument.

    As long as the present Director of Nursing is in control, and the administrator continues to give her control, I would avoid this place like the plague. My advice to the WONDERFUL CNA's at The Grove at the Lake is get a better job in a better place where you are appreciated for your hard work.

    This is by far the worst place you can have a loved one. My mother was admitted in this facility after a hospital stay for 10 days just to be treated for her UTI (urine track infection). I spend most of my days morning till night with her and I saw how negligent this place was with their nurses and doctors on call. If my mother pushed the buzzer to get help it would take the nurse 1 to 2 hrs to get to her. Doctor's never returned call to check on patients for days…..After 10 days of stay at the highland Park Nursing $ Rehab JUST for a UTI due to doctors not responding and staff not being responsively my mother passed away….Her hearth and her physical health was just fine. The only issue she had due to knee surgery she lost her mobility. Now you go figure this out…..WHAT WENT WRONG the night that my mother passed still is a puzzle to us???? God only knows???? If I were anybody who is reading this review I would seriously think about this place before admitting any loved one. Unless you really want to lose you loved one.

    Please put your love one in another facility other then Lexington Home Health - Nursing Home of Lake Zurich.

    First we met with the Lexington liason person at the Hospital and we thought we were getting the truth about the facility. Unfortunately, instead my mom ended up initially in a room with 3 other roommates, leading to a diaper that had not been changed over 9 hours and a bandage that was not changed. I could not figure who was in charge.

    I had a meeting with management and the intake nurse lied that she did change the bandage but I saw her take it off and ask another person to redress it which never happened. There is no excuse why my mom should have been in a soaked diaper

    for over 9 hours. I had to be there almost all day and night to make sure she was getting the care she needed. I never saw the doctor in charge until the 71th hour after admission, Illinois law is 72 hours I believe. My mom did get moved into a semi - private room with some CNA's that were wonderful but admitted the management keeps the supplies low because of cost. I guess that is why my mom did not get barrier cream on her sores. I moved my mom out of the facility as fast as I could. Who is in charge I could never figure it out. On a side note, the PT department was very knowledgable. I hope this was helpful for you since it is the first negative review I ever wrote but thought it was my duty to protect those who are helpless and those who we love.

    I was visiting my friend and she needed to go to the bathroom , was disconnected from alarm device, help button was on for 6-7 munutes . No one come to her room !

    What is rehabilitation for a stroke patient ! I never want my love one to use this

    care at all. If we didn't help her she would be dirty in bed . I felt sorry for old patient

    who laying helpless in bed .

    I had elective hip surgery on July 31, 2012, and I had the good fortune to stay at Highland Park Nursing & Rehab for about 2 weeks for my recovery (August 3 to 18). During my stay, I thought my private room was my “spa retreat” as the room was beautiful and the picture window view was so lovely. The staff was OUTSTANDING—in competency; in their nurturing approach; and in their sincere interest in the patient’s well being. The Therapy Trainers for me: Chris and Jack—were just incredible. Every time I was in therapy with each of them I made progress. They found ways to accelerate my learning and capability.

    I highly recommend HP Nursing & Rehab Center.

    We have nothing but good to say about the Wauconda center. We transferred my mom to Wauconda after another home in the area left her so dehydrated that she ended up in the ICU for 5 days. The PT, and OT programs are wonderful, my mom has seen a steady improvement over the last three weeks, and the nursing staff is very proactive and is always very helpful. They work with the on staff doctor and will call if there are any issues that come up. (We had this happen twice, and I am grateful for it!)

    The food is good quality and there is a nice variety, they even have a salad bar each day in addition to the regular meal choices for lunch and dinner. Breakfast includes the patient’s choice of items including eggs, meat, waffles, pancakes, fruit, etc… (Keep in mind that they also have to follow the person’s diet restrictions)

    The patients have many activities available to them and the place is very clean, bright and friendly. As you walk through the center , we see patients gather together for cards, TV shows, etc……

    My husband and I visit my mom at least once or twice a day at different hours each time, and we have been very happy with all of the staff and the high level of care she has been getting. (We just wish we had my mom go to Wauconda immediately after she was released from the hospital, instead of having had her go to the other facility first)

    Antiquated heating and cooling systems. Staff speaking everything but English in halls and rooms, RN didn't fully understand spoken English. Administrative RN at nurses' desk response to cold air recycled air blowing on patient was, "This is an old building." and kept writing. Even the front lobby bathroom has broken faucets with no hot water and unsanitary counter tops from aged cracks in seams. My relative's dinner was very unappetizing, all canned and boxed foods except the boiled and then baked chicken. Some very cheerful assistants came in the room and they seemed to care about the patients but they had no authority to correct issues regarding constant problems with the air conditioning and heat. Everything was the same as the last time my relative stayed there…thermostats broken in every room.

    Patient not able to move has food left on a tray 2 feet away from her. No one came in to move the food to her or help her eat. She rang for a nurse for pain medication. She in extreme pain. no one answered her call. Visitor rang for nurse but no assistance. Had to go out and complain that it was over 40 minutes and no reply to patient. Only after getting angry and complaining did a nurse come in. I don't know where these rating come from but this place should get a 0 rating. Uncaring staff. Patients sit strapped to wheel chairs in the hall all day crying , screaming and are just ignored. This place has so many defiiencies. Too many to mention in one comment. Our loved one is being moved out of this place. Make sure you visit and observe what is going on here before admitting your loved one to this place.

    My mother was a resident of Wauconda Health Care for 4 years. Up until the last year to year and half of her residency there we were very pleased with her care. A noticiable decline in services and patient concern were evident to us when a new administrator was put into place. After an extremely unfortunate incident that involved theft of personal property, and very little cooperation, accoutability, or follow through from the administration, we made the decision to move my mother to another facility. We were never unhappy with the CNA's or nurses who were extremely kind, caring, and hard working, but we could not look past the lack of compassion or concern from the administration.
