    This place deserves 0 stars. I believe they are responsible for the declining health and ultimate passing of my father. I wish we never took him here in the first place, but I want to save people from this place! My sister has since filed a complaint with the health department.

    He had hip surgery in December and was doing well in the hospital, they recommended that we transfer him to a rehab facility to get BETTER. These people put him in a diaper, never had appropriate food to eat, was left alone for hours at a time, and was NOT CARED for at all. Every time I went, he was sitting by himself with cold and hard food that he was unable to eat. At the time he was only supposed to eat soft foods and every time we went he had something he couldn't even eat. The "nurses" never pushed him to eat or do his rehab exercises. They neglected him and left him to feen for himself.

    This place is also terrifying at night, many elderly people left alone and often times when we went there were people calling for help. Once, there was a person cutting their toe nails in the cafeteria. Everything about this place is terrible and it is not an exaggeration. My dad gave up hope in this place. The worst experience ever was when my sister and I went, he hadn't had a bath in a week. We asked the "nurses" to give him a shower, he had been in bed for 7 days at this point. When they got him up, he had a seizure. My sister and I obviously had no idea what to do, but the "nurses" DID NOTHING. Literally, we asked for an ambulance or something. They did not even know what to do!

    It was the most terrifying thing I have experienced, after many failed follow ups on our end to get a conversation with the imaginary doctor, days had passed. We never once met a doctor in charge of his care. The only meeting that ever took place was about money even though my family pushed and pushed to meet his care team. I am fully convinced this awful place was responsible for his decline and ultimate passing. Care takers were rude when we asked questions and had concerns about his food and daily routine. They are unprepared for actual medical needs and hate their jobs, literally I heard them talking about it. I hope this place closes. There are so many more complaints and stories, but please stay away. It is like a terrible sad movie in there. Any positive review has to be fake.

    Had a parent, aunt and uncle at Burke, all at different times over the last 3 years. Found the center to be very caring and compassionate, administration was prompt to answer questions and helpful with discharge information. Therapist very good and the end results all very good.

    Best skilled nursing facility in northern VA.

    Excellent admissions, social worker, nurses, aides, PT, Speech Therapy and Occupational therapy. Clean, good food, response time to nurses aid call quick, Doctors are on top of medical issues but nurses are quick to access and call for ambulance if necessary. They keep family informed always free to talk and answer questions. Highly recommend.

    Fantasitc center! Caring staff thoroughout. I would say that my father received excellent care from the aides, the nurses, the therapists, the physicians and the social workers. It was nice to see the managers out and about seeking my opinions on the care. Everything was not 100% perfect but when I had issues, they followed through. I loved seeing the doctors there EVERY DAY! This is unheard of in skilled nursing facilities. I have been in many with my father and my mother and this was not the case.

    The physical therapy staff is wonderful. They work hard to motivate and help the patients do as well as they can with their therapy. The medical staff is troubling. They failed to notify us that there was fluid in the lungs of our loved one, despite the fact that we asked about the results of a chest xray. We found out the results from the physical therapist. The administrative people are less than competent. They informed us our loved one would be discharged in 10 days and then told us the next day that insurance was denied and she would be released in two. Insurance says the facility did not file the paper work. The discharge planner says she does not control insurance or the front office. Extremely frustrating and disturbing. We regret allowing our loved one to be sent to this facility and will not permit it to happen again.

    The PT staff receives 5 stars, the food no stars, and the staff negative stars, lack of communication between the shift nurses, staff does not know how to speak English or understand, they try to over medicate the patients, the staff CNA's and nurses very RUDE!!!! they do not bathe the patients but twice a week (REALLY), and when you complain they will ignore your love one. I would not even take my dogs there. Staff talking with patients about other patients… can you say UNETHICAL. This place was highly recommended to us, and all I can say is NO WAY! No compassion with the elderly…. if they do not eat their food, they will put the patients in a time out, facing the wall. If you have a family member that needs rehab, I highly recommend you DO NOT PUT THEM IN HERE!

    A horrible experience for my mother and her roommate, both in for rehab. The management are not responsive, not helpful, and clearly not managing well, and the quality of nursing staff/care was inconsistent.

    TVs blare all night & day keeping residents awake, mgt. say they don't have authority to do anything about it. Staff very rarely respond to call button at night.

    My mother was chastised and made fun of for calling for help going to the bathroom 4 times one night (due to medication) then chastised & humiliated the following night for having wet the obligatory diaper because she was afraid to call for help.

    No general information is provided when residents check in about the daily process, schedules, responsible management to contact, etc. Residents don't have private lines to bedside phones, so after office hours, residents can't receive calls.

    The phone extension given to call to request alternate meals doesn't work and no one was ever able to provide the correct one. The promised menu for selecting meal choices never appeared, even after several inquiries.

    There are no grab bars in the bathroom, accessing the flush handle is difficult, and the side railings are full of pee bottles & bed pans. Rooms (the "refurbished" ones!) have a single wall unit AC/heater that is next to the bed of one resident, who controls it. One resident is cold, the other hot. Wheelchairs are in short supply and not available for use. The nice patio at one entrance is occupied by smokers, so not available for use by non-smokers who want fresh air.

    It seemed apparent that their intention is to keep rehab patients for as long as possible…$$$. I found later the facility and its doctor are known among local doctor's offices and home nursing companies as poor performers who are difficult to work with or just not responsive.

    Many of the nursing staff were very kind & attentive, and facility cleanliness generally was very good, but overall Iliff provides an unpleasant rehab experience. How they win awards is beyond me -- standards must be very low indeed.

    I wanted to give it NO STARS, but, it won't let me!

    I was there rehabilitation after a total knee replacement. I was 40 yrs old at the time. I was left in my room with my walker out of reach, for a couple hours once I got there. No one would help me to the bathroom and even though I had a walker, they wouldn't let me use it on my own. The doctor saw me once, for about 2 minutes when I first got there. Never saw him again. One night, when giving me my pain meds, they tried to double up on my blood pressure meds. I refused to take more than what I was supposed to. The "nurse" got mad and stormed out (after arguing with me). An hour later, she came in, woke me up, and took my bp. It was 101/51. Just think if I had taken that extra pill?!?! I also had a man walk in the bathroom on me while I was having a "private moment". I was also harassed about my ice machine because "they didn't have an order for it". Basically, the medical staff is WAY MUCH TO BE DESIRED!!!! THE PHYSICAL THERAPY STAFF…WELL I LOVED THEM!!!!!! CUDO's FOR HELPING ME WALK OUT OF THAT PLACE!!!!!! (rather than the medical staff which almost sent me out in a stretcher!)

    The one star is for the only good thing about Burke Rehab: its physical and occupational therapists. Otherwise, it's a hellhole that I wouldn't send my dog to. My mom was sent there in 2013 after recovering from surgery. Let's see: the charge nurse tried to get her to take what my mom knew to be an overdose of one of her known medications; the same nurse ran into the bathroom when my brother, who happens to be a police officer and in uniform at the time, showed up to visit my mom for the first time; my mom failed to see an attending physician within the required 48 hours despite the fact that I was frantically calling to obtain medical care for her because she had swollen to the size of a Macy's Day float and was severely nauseous (it was later determined that she had a blood clot); upon speaking to the Center's Director 72 hours later, it was clear she had no idea who my mom was and proceeded to lie to me that a physician had seen her within 48 hours when I knew for a fact that he had not; and on one visit, I found four wheelchair bound elderly people placed at various spots around the nursing floor, one of whom had vomit all over her chin and shirt, and the nursing staff just walked by her as if she were a piece of furniture. A very sorry place, in my opinion.

    I am so thankful for all of the staff at Burke Health & Rehab Center. My mother recovered there after a fall and fractured hip (needed surgery too). She was completely independent before the fall- went to church daily, volunteered at the pediatric unit at Fairfax Hospital and came to weekly soccer games to support her grandchildren. Thanks to all of the rehab staff and equipment available in the rehab room, she is able to walk again and back to being independent. Everyone was so caring and helpful at Burke Health & Rehab Center- from the receptionist, every physical therapist, all the nurses, social worker and the cleaning ladies. My mother would not have been able to have such a great recovery without the help of everyone at Burke!

    The Commonwealth physical therapy department is top-notch, very professional, and great at helping patients and residents with their therapy needs. The RNs and LPNs are great, and the majority of the CNAs are very good at what they do. There are a couple of CNAs who could do a lot better, but for the most part, the care received here is great. The physical building is not very inviting, but the housekeeping staff is on the ball and does an excellent job of keeping the center very clean. The administrator likes to be kept informed of any issues and is good at staying on top of things; he has been a great help.

    The overall cleanliness of the facility was good. The cleaning ladies were constantly cleaning during the day it seemed. CNAs barely spoke English. There seemed to be a contest to see who could turn the call light off the fastest and never respond back to the patient. My father would push the call button, the nurse or CNA would come in, turn it off, and say, "I'll be right back", but never came back. They would never even bother to at least ask during the few seconds in the room what help he needed. At night, when the sun goes down, the nurses go to sleep. We would pray and hope for the best, hope for a good nurse or aide. The place once was really good on customer service when my father was admitted her before, which is why we came back, but not so much anymore. If it wasn't for OT and PT, I would have taken my father out of that place. The rehab people are top notch, made my father feel like a human being.

    While the PT and OT is doing a great job at this facility the nursing staff leaves MUCH to be desired. It was our impression that nurses did not communicate with each other (the weekend nurses never got instructions or a report from the weekday nurses on patient status or progress) and they will come into the room, say they will be back and never return. The doctor also is absent and when there walked into the room and asked my family member, "are you my patient?". Put your loved ones elsewhere if they need any kind of medical care or attention!!!!

    I cannot say enough about this facility. My mother spent 3 mos. there recovering from a very bad fall and the staff there were extremely attentive to her needs and their bedside manner is top notch. Our family would visit at random times and the facility was sparkling clean every single time. Being a nurse myself, I can tell when patients are not being attended to, and I can tell you that the staff in this facility are top notch, caring, and efficient. The administrator was very visible and she was always asking us if everything was ok with my mom's care. I don't know where a few of those negative comments came from, I just didn't see it!!!! I'd bring her here again in a heart beat!!

    Burke health and rehab center is the cleanest and best run facility in the area. I had a wonderful experience while there. I was in need of short term rehab and thought the nursing and therapy was first rate. Customer service was excellent and I developed strong friendships with the motivating staff and many other patients there for rehab. You really must see this center! The therapy gym will wow you!!!!!!

    This was sold to us by Loudoun Hospital as a "hospital setting" with access to doctors 24/7. It was not. My husband was ignored for hours after he arrived and could not get all his pain meds until a doctor saw him, which could be whenever. They come in only a few times a week to see patients. I was lied to by the Administrator, the Director of Nursing and the Nurses to cover up their incompetence and failure to properly care for my husband. He did not get his meds on time and there was no real hospital care at all. I had to jump through many hoops and make lots of calls to get him transferred to a place that could take care of him. The Dr we saw was kind but no one that worked there seemed to care at all. Don't leave your loved ones here.

    I was a patient at Leewood from about July 2010 to September 2010 after I had been released from the hospital due to a near-fatal accident in April 2010 when I was struck by a car while riding my bicycle. Although some of the nurses were good, the CNAs were terrible. Some of them weren't even native English speakers! Granted they were not equipped to handle the care of a young man like myself, but I would NEVER allow them to attempt caring for anyone I loved!

    My grandmother was here for a short time for rehab. I enjoyed working with the staff in getting her back on her feet. She is currently at home and doing really well. I am a nurse by profession and I am aware of the challanges people face at nursing homes however, Commonwealth Health and Reahab was above and beyond a great place to put my grandma.

    The staff was inept; they were not trained to appropriately handle my father's needs (although stated they could handle acute care). There were repeated occassions when he needed assistance--had his request light on--and after 45 minutes of waiting, called my mother, who then ran over there to get their attention. This was unacceptable. Under no circumstances would our family recommend this facility to anyone.

    This facility lacks proper communication. When an issue with the apartment occurred, I receive a less than amicable response from the general services desk. The manager in charge of housekeeping is less than knowledgeable and rather belittling/rude with their conversation with residents. The way I saw the outside workers/third party companies, treated was distasteful. I found that while the campus/neighborhood was beautiful, having to deal with certain departments was rather distasteful. Not recommended in my experience.

    Commonwealth Care is a poorly managed rehab facility with significant staff turnover and low quality care. My father-in-law was in the facility for over 2 months, during which time, we needed to have a family member present throughout the day. Staff is slow to respond to requests for assistance and spoke poorly of other patients in public areas. I would never send a family member to this facility.
