Carrington Place in St. Petersburg is for sure a place to stay away from. The food is horrible, they are lazy and will not take them to the dining room to eat or if eating in the room it arrives hours late and is cold. The entire wardrobe of clothes did not return to the room and lost forever, they do not care. The care is pathetic, no changing of the diapers or bedding, no time with the patient, no doctor to see the patient, no showers, and not one staff member cares. Every aid, tech, CNA, nurse, or any staff member says they do not know and passes the buck to another and another and another. This place is horrible - do not sent your loved one here. Cars were broken into so leave no valuables in the car, no reasonable parking whatsoever. The hand rails to either go up the steep steps or the handicap ramp are sticky and gross. The ashtrays in the front or the smoking area are not cleaned. Disgusting place!

Someone I knew was staying here because she had bacteria in the heart and was on antibiotics every 4 hours one of the male nurses was consistently sending messages to the patient so I went through the phone and read messages that they were having sex and doing other things. I believe this is very wrong and hope the director fixes the problem.

This is one of the filthiest places I have ever been in my life. There is caked on dirt, food, feces etc.. in the hallways and rooms, the PT room has blood or another dried substance on the wall near the sanitizing station. My husband (hip replacement therapy) was supposed to be admitted Sunday 2/15/15 first thing in the am. The transport driver came to BAYFRONT hospital to pick him up at 8:30am and was told to turn around and go back as the room at Lexington Rehab Rapid Recovery had to be sanitized. When he finally was admitted at 4:30pm to the "sanitized" room, the wallpaper was peeling from the walls, bugs were climbing on the walls and the baseboards and corners of the room were black from months and months of filth. I HAVE PICTURES OF ALL OF THIS. The food portions are miniscule and the food is all processed loaded with salt, no fresh veggies or fruit is served. Bfast today was corned beef hash with toast and oj. Who eats corned beef hash these days. This facility must be experiencing financial difficulties. All but a few of the nurses were helpful, everyone walks around looking like they are busy but most of them are sleeping in the TV room which is also filthy and cups from starbucks are sitting there from days. We asked to see the Head Nurse to get an early discharged since he had been there since Sunday and had only 2 therapy sessions. The staff kept telling us they called her and 24 hours went by and we never heard from Barbara and finally I had to raise my voice to get some answers and she came to see us and told us the staff never contacted her. We got the runaround the whole time he was there. We had to request he have his bandages changed. I was under the impression that when a patient is leaving a hospital to be discharged to a rehab facility that a list of facilities need to be presented to the patient. Christine who Bayfront hires to assist patients in this procedure never presented a list, she mentioned Lexington, Apollo and one other, not up to par facility. If my husband did not have me an advocate (squeaky wheel) to get some action to this shoddy facility I am sure he would have still been in there. Don't allow your loved ones to go to this place.

This facility is terrible when it comes to patients needing trache care. My mother was breathing off the ventilator for nearly a month. Sent her here for continued rehab and she passed away 5 days later! They don't like to suction trache patients. And because of that my mother aspirated. The late night nurses are lazy and don't check on the patients nor answer the phones when you call late night to check on your loved ones. I hate I sent my mother to this place. No compassion when she passed either. Just a simple call stating she had passed, yet when I arrived shortly after she was already cold!

It does not meet very many objectives,only pts are cash cows they are there only to keep money coming from medicare. I know 1st hand I was there,by the grace of god and my wife that I'm home now. It is so bad that no way I'd send my worse person I knew to go thru it. God knows who ever that may be. Local tv has expressed some of the goings on there for inspections. I can only wait

Used to work at Carrington place. The staff are intolerable, the administrators are uncaring, worthless, money greedy individuals and the kitchen is full of COCKROACHES!

I was a patient in the newly opened Physical Rehab section and was miserable the entire time. There is no pharmacy onsite and there were a number of instances that I had to wait many hours until the 5am delivery for basic medicines and then it took another 2 hours once medication arrived for the nurse to bring it to me. The Rehab section is severly understaffed at night (God forbid anything happen between 11pm and 6am or the weekends, you will be S-O-L) Horrible cold food, cold coffee hardly ever available. Extremely uncomfortable beds. Visits from doctors were very infrequent, issues & complaints often fell on deaf ears (or just fell through the cracks.) No one picked up the phone at the center after 5pm so family couldn't get any info. I would never ever reccommend this as a place to put a loved one (maybe just my enemies)

The staff is not very friendly and unqualified. My son is there and he has been trying to find another place to live. His mail is opened all the time. The staff discusses the residents and the reason why they are there. He has to plead for help and they just ignore him. It's not just him but other residents as well. He has complained but nothing changes. I would not recommend this nursing facility to anybody.

The staff are really helpful and caring and the quality of nurses is good. The only downside is the food is marginal. If more care is needed we will return.

We chose this facility recently for our dad because of their location and their inspection rating. The did not have any tags on their inspection from the state inspectors. It is a very small facility, but the staff were wonderful to dad and the administrator seems to be there night and day. If we ever need a nursing home again Concordia is on the top of our list.

This Nursing Home needs to take better care of peoples belongings. They take my grandmas clothes and give to other people.. or they get washed and never returned. I made a blanket that was to NEVER be removed from her room and its missing… They have nothing to say about it..

This place deserves a #2 Rating due to its below-average, inattentive and impersonal care, especially from the nursing staff. While the # of staff present seems adequate, they're simply not routinely providing for the most basics needs of their patients, including my dear friend. For example, family members have had to: empty patient's bedpan/urinal; seek out clean linens & ice; etc. Requests for pain meds (including RX) are not promptly followed up on. Patient advocacy is nonexistent; and social work/caseworker services are completely inadequate, especially re: discharge planning. (A caseworker is only available during specified times two days/week; and when asked on one of those days for assistance with discharge planning/in-home services, that individual simply provided a handout listing approximately 30 possible resources for family members to contact.) Supposedly a physician is available at all times, but the family never saw one over the course of several days & visiting at different times. Thus, some specific medical questions/concerns related to the individual patient & rehab progress remained unanswered. If you have any other viable rehab option for a loved one or yourself, please take them--or go--there.

The only reason I give this a two star is because of the hard working nursing assistants. The building is old and they are making additional rooms for patients. The interior of this building is very old. I never see a water pitcher in any room, occasionally I see a cup of water sitting in the room. The mattress's are old and need to be replaced. The bedspreads are very old, not a very pretty sight for sure. No comforts of a home setting.

awful service, the director of nursing is rude, the administration is terrible, i pulled my grandfather out of this place. he was missing 300 dollars worth of clothes that were never found and he was never reimbursed for.

My mother came to LaurellWood for short term rehab, they did a wonderful job. The staff were friendly and helpful. Nice home-life atmosphere, small and clean. Its a older building but the nursing care is excellent.
