As a loved one for someone who personally pays for these services, they are completely horrible. They do not feed my loved enough and the food quality is horrible. It is stated that patients can have seconds and have unlimited coffee, but this is completely not true. They expect that anyone there signs a blank check that they fill out, there is no billing statements or explanation of benefits. It was explained that there would be exercise, but there is none. When trying to speak to the administrator, instead of having a conversation, she will hang up on you. Staff tries to claim that my loved one is going crazy or is out of his mind rather than admit that the facility's actions are improper.

Great place! They are so patient and loving with my granddad who can be very difficult! The cat that lives in the facility is a great home like touch. The nurses are caring and informed. The cans work so hard caring for everyone. The building is clean and smells good.

The facility is corporate owned. The building site (over 50 years old) sits in a flood plain and suffers from flooding issues whenever it rains. The drainage is very poor. There is black mold growing under baseboards and sheet rock walls. Staff are inconsistent and very under trained. Petty arguments erupt all the time between nurses and CNA's. There is a procession of directors and administrators that come and go. The one good administrator they had provided very good direction and leadership. But, his son died suddenly, and they let him go a few days after his son's funeral. Not a good look for the business. I would not allow my dog to stay at this place.

My Mother is a patient at Gracepoint. The Director is rude. My Mother's things keeps coming up MISSING. The food that they feed to the patients is slop.

There are very few professional staff out in the nursing home facility. My family and I were met with resistance when explaining about an issue about our loved one. I have observed residents not properly cleaned, the food appeared to be not appetizing at all. On a few occasions, I observed where staff would yell out residents and they cannot help it. There are no actual activities provided to help keep the residents active. There are a few professional staff but they are being ran away because of the unprofessional employees.

My mother-in-law was transferred from JRMC to Whitehall Rehab on a Friday afternoon.
After the initial "sign in" she was ignored for the rest of the weekend. I asked one of the nurses about her dinner meal and was told" There wasn't a name on the door so we didn't know anyone was in there. (Even though the door was open and there was a least two others in the room). She did start to receive better care(meals were brought and she was checked on) once her name was posted on the door (Monday). The family didn't have to bring food from home or remind the staff she was in the room. She is incontinent and her care was not very good. On more than one occasion she was left in wet diapers, the family started writing a time and date on the diapers to ensure they were changed on a regular basis. There was one outstanding lady who took great care of my mother-in law, but the rest of the staff had a very poor attitude. I rate the Whitehall facility as one of the poorest in Jefferson County. (I have loved ones in other facilities) in the county and their care has been so much better. I regret that my wife and I thought that the Whitehall facility was good.

I walked into the facility for the first time and I was almost knocked over by the smell. It smells like the toilets overflowed and it was allowed to sit for a while. The nurse who came in my moms room was trying to look at her wound. I asked where her gloves were and she said she passed them on the way in. My mom is in a diaper and has a pressure sore on her bottom. She is in there for rehab to get her strength back, not to lay in a diaper and soil herself. I'm afraid she will get another infection. She was told no pain meds because the Zyvox she was on is an antibiotic. Turns out it's to keep stomach acid down. Not an antibiotic at all. I pray my mom gets out of there quick.

When you live far away from your loved one it's a difficult situation. The staff has been unprofessional in phone calls. They are not forth coming in the health and physical care plans for my mother. They do not have a long term bed for her to move to once rehab is over yet they will not send referral packets out. She can not go home and needs a long term care facility. When all you have to go by is the way they speak to you I have no trust or confidence in this place. I would never recommend this place to anyone.

While my father was in this facility he caught scabies and the hardest time getting rid of it. And he was not the only patient to catch this. It seemed like almost everyone in his wing was getting it including staff. The nurses seem to be inexperienced and unknowledgable and there always seems to be a shortage on staffing. Not the best facility around and definitely not a five star rating.

DO NOT PUT YOUR LOVED ONE IN THIS FACILITY. THE STAFF IS COMPLETELY UNPROFESSIONAL, UNCARING, UNQUALIFIED, AND RUDE. THEY COULD CARE OR LESS ABOUT THE DEFENSELESS RESIDENTS THAT DEPEND ON THEM TO DO THE JOB THEY WERE HIRED TO DO. IT IS HEARTBREAKING TO SEE HOW TERRIBLY NEGLECTED THESE LOVING RESIDENTS ARE.

I agree with the post from 2012. Patients with a high level of care should not be placed here. Due to new Obama laws if you are not dying of a disease, you do not qualify for hospice, or at least this is what we are told. This is sad to me. Pain medications are given an hour to two hours late, changing is not done every two hours, call lights are not answered in an adequate time. To specify more on this, we have had a call light on for 45 minutes, and not have anyone come so we ended up changing our family member ourself and turned the light out. Watching your loved one scream out in pain is not the way you want to see them in their last days. They should be on comfort measures, with pain medicine given on time. If I could underline the "On Time" I would..I understand we aren't the only family who has someone in the home, but this is still over the top in my opinion. We have been requesting for a change in diet for almost two weeks now. Our family member can't chew anything, nor drink out of a straw yet being served Fried Chicken, Pork Chops, etc. Seems like the blaming game went on for a few weeks. Getting desperate we put in a call to the DON who informed us that the diet change order was put in last night which means that the staff was dishonest with us for almost two weeks. Every meal we would ask why the diet hadn't changed, and we were told that they had submitted it to the kitchen, but the kitchen must not have updated their records yet. You need to have a full time family member around in order to assure your loved one is getting the proper care, fed, changed when needed, and meds on time. Sadly I know we don't have that much time with our loved one, it is going to be hard. We just want for them to pass with dignity, no pain, and knowing they were loved. The way any normal person would like to be treated.

In April 2015 my mother went to this home for rehab but we all knew the outcome. She had cancer and it was spread threw out her body. The nurses and staff were the best I have seen and been around in a long time. Our mother just wanted to go home she was tired. She was only there for a very short time. Here second day she went into the sleep, she was preparing for her journey. Within 48 hours she went home to her father our creator.. I have never experienced such caring and kind staff. They let all 9 of us kids along with grand and great grandkids friends and family stay there 24 hrs a day. The staff was great with our mom and I give a great big thank you to the nursing staff and all the others.

My mom was signed up to go here for Rehab after Bypass surgery. When we checked into the Facility the staff was arguing amongst each other. The administrator apologized to me and said that that never happens. The next evening my mom was rushed to the hospital. The night nurse did not monitor her oxygen or check on her through the night. Our Dr at the Heart Hospital said that if she went much longer without oxygen she could have had brain damage. Speaking to other residents there I learned that the staff argues constantly in the halls when visiting family is not around. Protect your loved ones and keep them away!

The administrator doesn't care about the dignity of there residents. The residents who become bedfast are expected to use the bathroom in there briefs, because bedside commode's are considered a health hazard. Families aren't notified of medicine changes . Apparently if a CNA doesn't want to work to much, or at all, they just forget and leave their portable call apparatuses at the nurses station or anywhere else but in their pockets, where they should be. Their are a few good CNA'S out there , but few, and lots of unhappy residents and family members." ONE STAR "
