    DO NOT SEND YOUR LOVED ONE HERE!!!! This place is sooo bad. They do not have heat. It is always out. My love one was here just recently when it was the coldest days in January and there was no heat in most of the facility. They were moving the residents around daily because of it. THE FOOD IS AWFUL!!!. When we took the tour the director told that the food was good. Lies all Lies. It is barely edible. The staff wouldn't even eat it themselves. They did not following any of the standard protocol. My loved one was released without prescriptions for critical medication that were needed. The social worker Eric Bobila lied about contacting my love one's doctor and about taking care of the medication. I do have to say that some of the nurses were very nice and tried to be very helpful. Thank you to them. We tried call them to get them to help with the issue with the med's but they kept giving the runaround and then the just wouldn't take the call. They would just hang up on me. I say it again..if you are considering this place DON'T DO IT!!

    I'M SO SAD, I WANT TO CRY:

    AS A PROFESSIONAL WHO ENJOYS BEING A PART OF A PROGRESSIVE TEAM WHO WILL WORK DILIGENTLY TOWARD THE GOAL OF QUALITY CARE FOR PATIENTS WAS A DESIRE EXPECTED TO BE LET AT THE CURBSIDE.

    FINDINGS: NO ONE REALLY WANTS TO DO A DARN THING EXCEPT FOR TO KEEP UP CONFUSION, BACK BITE, WANT TO BE IN CHARGE AND SHIRK THEIR DUTIES AND RESPONSIBILITIES ON TO SOME OTHER STAFF MEMBER IF THEY CAN.

    I HAVE NEVER SEEN ANYTHING LIKE THIS IN MY LIFE IN HEALTHCARE! WELL, NOT UNTIL I CAME TO THIS PLACE TO WORK.

    HOWEVER, HOPEFULLY AT SOME TIME IN THIS LIFE TIME ALL WILL COME OUT, AND THE WORD WILL GET OUT IN THE COMMUNITY, THAT IF ONE NEED GOOD SAFE NURSING CARE, DON'T CHOOSE THIS PLACE, AND IF YOU ARE A GOOD CAREGIVER, BY ALL MEANS DON'T APPLY TO WORK PLACE. WHY? IN BOTH CASE SCENARIO'S, ONE WILL BE FULL OF NOTHING BUT REGRET--JUST LIKE ME.

    I know when there is a excellent director, there is a excellent staff of Managers. If they don't communicate well, nor will their workers whom they supervise The updates, patient care and ability to encourage patients to do their best is missing. Can a elderly dementia patient make the best decision for themselves? Of course not. As a family member and care taker, you must have the ability to communicate well with us because we are their voice. My experience was not of the best for my loved. I do not want ultimatums, just results.

    Not Recommended. My 90 year old father received lack luster care and was cleared for discharge with a contagious bowel infection and pressure soars that he developed at this facility. Also, he was retaliated against after we complained of him being allowed to remain in feces, urine allowed to pool on his floor for days and not assisting him with his meals. This is a reminder of a few principles that underlie nursing home/rehab facility neglect lawsuits. First, many nursing home chains such as this one exhibit substandard care over and over again. With profits so large, many of these facilities simply assume that lawsuit payouts are part of doing business. Residents are hurt over and over with little changing. Second, even then, facilities often do everything in their power to prevent paying for the consequences of their negligence, even if it involves appeals that drag out the process for years. Unfortunately, fairness for residents is often low on the totem pole for owners and operators of the largest nursing home systems. My father will probably have to be readmitted to another hospital. This place needs to be investigated for their substandard care.

    My father was in Brentwood for only six days before we removed him and took care of him ourselves for Alzheimer's, within three days he was so much better with us taking care of him. They did not inform us that he was on risperidone (Risperdal), a medicine for schizophrenics, in which he is not nor ever been. We feel he was on risperidone (Risperdal) to knock him out all day and night so they wouldn't have to deal with him. The staff would not answer our questions and we were told to ask the administration, when the administration were asked the questions they gave us the run around. The so called Doctor was very rude and uninformed. Avoid this place like the plague.

    My sister discovered The Bridgeview Health Care Center through an agency. After a quick facility assessment and after having checked other facilities she choose this one.

    My mother has been a resident of the Bridgeview Health Care Center for 2 years and a few months. At the time she was admitted in 2011, she was suffering from mistreatment from a nearby Center located on west 87th Street. She painful had bed sores on her right foot and had gained 50lbs. Mom was totally non-ambulatory although she had been able to walk somewhat when she was first admitted to the other Center. She seemed to be coming down with dementia (according to the doctor at the other Center) and was disoriented. She is now no longer has bed sores, she's able to walk with and without her walker. She is no longer disoriented. Her dementia wasn't dementia at all. She had had at least 5 minor stokes, but she's doing much better. She smiles a lot, she holds intelligent conversations, we are able to take her out to restaurants and family homes because now she can climb stairs. All of this is because of the excellent care she received from the Bridgeview Health Care Center. I didn't give them 5 stars because they're not perfect.

    There are a few negatives: 1. from time to time we have run into issues pertaining to the physical exercise she is permitted to have in the evenings.

    2. She doesn't like the food.

    3. As with other health care facilities, the stall is nice but not all staff is trust worthy so attention must be given to their belongings..

    The staff is incredibly committed to the residents. Everyone is friendly and helpful. The building is exceptionally clean. Therapy is wonderful and the respiratory team is excellent. Everyone seems to work together well. I would recommend this facility. All the management staff is eager to help and always available. The care was great and discharge home was well planned.

    This is an outsider's review. I live close to the center and cannot tell you how embarrassed I would be to be affiliated with this business. The employees stand outside on their smoke breaks and act like they are at their own back yard BBQ. The language, tone of voice, and their volume is completely unacceptable! I am offended and I'm not even a patient or employee there! It doesn't matter if it's 6AM or 6PM, they have no sense of what is appropriate for the workplace.

    It's very disappointing and a little frightening to think some of these people are raising children that will one day try to get a job.

    The place is just awful, can't even describe the care, treatment, lack of human compassion for the resident is heart breaking. My son spend over 5 weeks in the facility and on day 2 he had a personal belonging stolen. There are a few employees that try to do their job, but is so much that they seem burn out. I am surprised that a facility as this is still open. My rating for this facility would be half of one star, for the few staff members that cares.

    My mother was transferred from Trinity Hospital to Brentwood due to burns on her leg. I was very nervous at first, but during the 27 days she was there the staff treated her with respect and were attentive to her needs. She was dealt with gently as if she was their loved one. Social Services was excellent, I would recommend this facility, and my mother still talks about it, she even wants to visit the staff

    My grandmother was transferred here from Christ hospital while recovering from pneumonia and further breathing issues. First I would like to say that our family did not have any complaints about the morning staff, but as the day progresses the quality of care and attitudes of the staff declines drastically! This was the typical situation that once no visitors are around and miserable employees are working at night, proper care and treatment was not given. The first night there my grandma did not receive her medicine nor was she helped with being transferred to the bathroom. Important note* my grandmother is extremely mentally competent and very independent but due to breathing problems her mobility was limited due to being a fall risk and dizziness! She was forced to take herself to the bathroom& at the same time her roommate was YELLED AT for defeating in her wheel chair, after her and my grandmother repeatedly pressed the call button for further bathroom assistance. For PUNISHMENT she was left in feces along with it dripping on the floor (sorry for the weak stomached). The following day out family demanded better care and a private room, hoping for a better experience. No such luck! My grandmother yet again did not receive all of her medication and was told "you'll get it before midnight" (lie ad lazziness)! Also she was never given a new gown or clothing since leaving the hospital, again she was told that she'll get clothes "later!" Her call button remained to be ignored and she was treated rudely but the incompetent night staff! When my mother tried to follow up with her second night of complaints with the nursing staff she was ignored for 40 minutes on the phone- no answer! She was at the facility for about 5 days and was never granted any physical activity let alone the physical therapy she went there for! This place should be shut down FOR REAL!! Looks can be deceiving during the day and I'm sure staff members wrote half of the recent reviews! As a CNA who has worked at a nursing home this place makes my blood boil! Totally agree with other people that this place doesn't deserve ONE or a Half of a star!

    I can't even give this place a star, but had to in order to submit my review. It is horrible! My brother unfortunately does not have insurance and has landed up in the *ell hole. My heart breaks for him and all the others that are "stuck" there. I don't understand how the state can allow such a place like this to remain open. It should not matter what a person's financial situation is… they are human beings and deserve to be treated with dignity and respect. This does not happen at Oak Lawn Respiratory & Rehab. DO NOT put someone you love in this place. God help them if you do.

    This place is trully a nightmare only to end up losing your LOVED ones. They only do their job when you are on top of them & thats not suppose to be our job, especially when your there worrying about your LOVED one & top of it all also worrying about the staff that they are not doing what there suppose to be trained for. I wouldnt put my worst enemy in this HELL HOLE !!! AND THATS NO JOKE , SINCERELY A PERSON WHO LOST THERE LOVED ONE, DUE TO THE NEGLECT OF BRENTWOOD STAFF. SHAME ON YOU!! NOW THIS PAGE WILL NOT LET ME SUBMIT MY REVIEW UNTIL I RATE YOU. BUT THE PROBLEM IS THAT THERE IS NOT A RATE LOW ENOUGH TOO RATE BRENTWOOD @ 5400

    My father served in the army for this country. After being kicked out of Hines VA Hospital last week, my Mother and I had very short notice to find a rehab/nursing home to transfer him into. Concord was our choice with this short notice. I won't get into how Hines treated my Father in this review as Concord is the subject.

    * Concord Nursing & Rehab Center is outdated. The staff, while friendly at times, works with what they have. And sometimes you have to stay right on top of them to get attention to your loved ones needs.

    * Management tried to give anwsers regarding treatment for my Father, but I always sensed an underlying shift of blame. The term "State Reg's" was mentioned by them severial times.

    * There is a renovation of the dinning room at this time. I would love to hear if this renovation ends here, or is part of something bigger through the whole facility. My guess would be no! And if I'm wrong, I would ask anyone at Concord to despute this with me in person.

    Bottem line … research this place first if you can. There are better places not far from here. We found one a couple miles South and we are transfering tomorrow!

    The facility was cleaner than other facilities I visited. The staff for the most part extremely caring. However, every facility has employees that either won't or don't do what is requried of them and it makes it bad for the facility. I've encountered a few such employees who provide poor patient care, don't respond to the caller button, don't leave water for patients who are unable to get around on their own just don't care. I visited a patient there and asked for supplies to clean the room, the table and the night stand. The cleaning rags were black and even green. I used several disposable cleaning cloths before I confirmed that the area had not been cleaned in an incredible long time. Overall it was on the nicest nursing and rehab facilities I visited between Chicago and Indiana. Some issues can be addressed with administration and perhaps corrected, others may require a staffing overhaul. Staffing should keep their opinions about patients and their jobs to themselves when visitors are present. It gives a very negative overtone. And if peopel working there are that un-happy what kind of service are they providing for the patients who are sometimes helpless to their hands or unhappienss-They make an otherwise good facility bad.
