My grandmother was set to be in this rehab until her4 broken ribs set .She has her mind and sound she is not a cripple she is not helpless .she had 4 broken ribs ,the first week she was inside this facility the Dr there refused to speak with her Dr and refused to follow the hospital meds and the medication she has been on for years .So in her not receiving her water pills ,she swelled up for 5 days in her legs not able to walk or do the rehab because they refused to give her the water pills ,in witch then caused very painful sores to break out on her legs so the water can drain from her body .

Family and I were there every day ,making sure they were on top of everything she was left sitting in her bed with the arms of the bed up not able to use the bathroom also some days sitting in her dirty diaper for hours until staff or family came to then change her .

Her call bell to call for help was broken the whole time she is there 3 weeks at this point they handed her a bell ..Like a dinner bell, nobody came I personally changed her many times I personally changed her my self and begged staff to assist ..

Some times waiting 2 hours everyone there pushes the job on someone else

She was giving frozen food one afternoon while I was there not to shocking however, waiting for the teams to come in or speak to the dr never happens they never call back never give you and answer to our faces (family) Now she has bed sores and it is stage 2 because the only time she is getting up is when family is there and we all work.Her bristol will not take her with stage two bed sores until she heals ,these people are a disgrace to the health care field I watched 4 people come in and family took them out within the three weeks my grandmother was in this place..I work in the health care field this staff is sicking .And very sad and very frustrating .WE are removing her and putting her else were to heal ..

GRACE!!!! PLAZA!!!!!

There is absolutely nothing graceful or lovely about this facility (maybe 20yrs a go it was the place to be). However, now it's the place to run from as fast has you can!!!!

This is a seniors worst nightmare. First, you are confined to a wheelchair or bed, if you need to go to the bathroom you are told to ring the bell. however, nobody comes . Maybe 30 mins later when its obviously to late. Most of the aides are nasty and extremely lazy. i employ you do not place your love ones in this place.They will suffer!!!!!

After an eight day stay at a local hospital our mother was placed in this facility for rehab. First of all she was not seen nor was she examined by a doctor the whole time she was there. They refuse to administer the proper meds for her. Most of the meds are over the counter or the cheapest generic meds you can buy . Our mother had a blood glucose level well over 400 at one point. And all they were doing was giving her shots of some cheap insulin that did not work. The final straw was when one of the aides actually brought cold dinner and basically threw it down in front of my mother.( I had to restrain my brother from physically hurting that aide) WE took her out !!! if this is how my mother was treated while we were there, can you imagine what they do to these poor souls whose only crime is that they matured gracefully!!!

This place should be closed!!!!!!!!!!!!!!

I used to be a member of the local fire department and basically witnessed the staff of "Death Plaza" nursing home kill people on a day to day basis as they had no idea what the hell they were doing during medical emergencies. They suck. Keep your loved ones out of there.

This place should be shut down!!!! They have no regard for the people the take care of! My mother in law was put there initially seemed just ok but took less then 1 week to see the true workings of the place. The staff was never there to help. The patient fell too many times to count unbenounced to whomever was to be watching her. She fell and we were called advising that she did the last time and they said she was ok no need to go to hospital. Surprise less then 1 week later she was brought to the hospital b/c as they were supposedly cleaning her they found a bruise on her hip and surprise surprise she had fractured her hip….We could not get her out of there soon enough but thank God he did it for us first…Please NEVER put a loved one here place should be condemned

My mother had been in 3 different nursing homes until we found Highfield. I can not praise this HOME . The place could use a little updating but you cann't beat the staff.

It reminds me of CHEERS where everyone knows your name. My MOM just loves it there.

If I tell you this place is a snakepit CLOSE IT DOWN!!! All the staff is truly the worst I've ever seen. The lack of medical care and compassion sickens me. My dad was left there to die thank God I got him out of there alive. When I tell you I was appalled in the treatment of all the residents there. My father who was gravely ill and they came out and said "he's not going to get any better so accept it you need to stop trying to keep him alive" Well Grace Plaza staff,social worker,Medical Director he's still alive and making great progress in a much better facility,where the staff is loving,caring and yes medically fit for my fathers needs.I write this for all of you that maybe considering this place and for those that may have a loved one there,GET THEM OUT AND KEEP THEM OUT. The State of New York Osbudsman classifieds the snakepit as a class A facility ugh. I don't give it 1 star it is truly awful.

This horrible nursing home contributed to my beloved Aunt's death by not recognizing the signs of a stroke--she also had a stage 4 bedsore caused by their laziness. Their staff neglects the patients--the resident has no dignity and they make sure their patients become dependent on them for everything. It is appalling! DO NOT let your loved one stay at this nursing home--they should be SHUT DOWN!!!

I was very happy with the care my mother got at this facility. Most of the caregivers were kind and caring and the recreation staff was great. The decor and bathrooms could use updating but it was always clean. My mother was well groomed and cared for. The first nursing home my mother was in was beautiful but but the staff was cold and you felt like they didnt care at all. My mother had Alzheimers and I felt the staff understood it and handled it with patience.

There is no care or concern for the well being of our loved ones.

It is an outrage that the state allows this facility to remain in business.

The rooms are disgusting and dirty, The staff is rude and lazy, They are physically and medicinally restraining the residents!

WHEN IS THE GOVERNMENT GOING TO OPEN THIER EYES AND DO SOMETHING FOR OUR MOST VOULNERABLE CITIZENS!!

My parent died at thier hands because of PURE NEGLIGENCE!!!

I don't want to see anyone else ever have to go through this!!!

The state should close this facility down, solely for unexplainable injuries to its frail residents and useless social work department!!! Horrible care at the cost of our loved ones! There is no person centered planning based on individual need and one of the overseeing physicians is an elderly man himself whose judgement is flawed. My relative has sustained numerous injuries that could not be explained according to the staff. Beware!

The care is top-notch. The facility itself could use an update. However, the staff has been great - even the head of nursing stopped in to say hello when I have visited. As far as nursing homes are concerned, I'd say this one has exceeded my expectations, care-wise. As a state employee, I know how hard it is to please the state. This nursing home has a 4/5 Federal rating.

This is another case of not judging a book by its cover. When I first arrive with a relative for rehab to Grace Plaza I must admit the place looked great, the people from admitting seemed very nice, However the first week my aunt was registered to this rehab inspite of telling everyone her regular prescription needs that included PAIN MEDICATION for almost her entire stay the staff neglected to acquire her medication. This went on until after 1 week after admission I finally brought her pain patches from home But then much to my suprise, still for 2 days they failed to administer her pain patches until I screamed and made a stink that, infact they had these patches in their possession! Unfortunately the head physician neglected to tell anyone he put her patches behind a door in a locked cabinet. My Aunt was also subjected to wearing an alarm that were attached to her clothing and her bed (supposedly to scare the patient into NOT LEAVING THEIR CHAIR OR BED! ***(OR THE PATIENTS OWN PROTECTION) this alarm which was attached to the robe just below the ear could wake the dead and startled anyone who was around and not accustomed to hearing it.. I was told this alarm is required of all seniors and residents. I failed to understand why anyone who calls for assistance should have to wait more than 30 minutes to use the bathroom or for that matter to even have a staff member investigate why the patient needs to be seen. Indeeed my relative was very irrated and acting out. One night inorder to help her settle in I offered to stay over night I asked if I could sleep on the recliner that was in one of the excersise rooms (its on wheels) and they refused! Instead they gave me a hard and uncomfortable chair to sit on.

This nightmare ended when my aunt said the people thier were verbally abusive to her she told me during the night an employee said to her " your 91 years old why do you complain so much, your going to die tonight anyway". That afternoon I pulled her the hell out of their.

I reported this to the nursing supervisor who assure me she would investigate this claim.

PS I never heard from anyone again!
