WOW! What a wonderful and unexpected experience we had! My father-in-law had surgery and couldn't go home until he was stronger and the hospital suggested we try Avita. I didn't know what to expect but I was not expecting the beautiful building, private rooms (with private bathroom and shower!). The therapy was as good as a private clinic. The food was not bland and boring - and he could order pretty much what he wanted. The employees were caring and professional. He got very attached to Raylene and Nichol! But everyone was wonderful. It is obvious management treats the employees well because they were not just kind to the patients, they cared about each other. Such hard workers! I wish I could name them all but my list would be too long! Please consider Avita if you or your loved one needs their services. You will not be disappointed!
My name is Robert Stallings i was sent there for iv antibiotics and physical therapy i was nervous at first but when i got there i was treated very very well made me fill like i was with family. They take care of all the patient's with care and respect i highly recommend them if u have someone needing care all the nurse's were so good and nice to me i just couldn't believe it and i just want to say to everyone there THANK YALL for the care and respect yall gave me i will always be thankfull !!!!
Don't support this business. It is ran by people who have zero knowledge of healthcare. They will bill medicaid and medicare for everything they can while providing residents with nothing. With so many good options out there, please don't support these con artists.
Very good people work at this building. They make you feel very welcome and treat you like family! :)
Recently after two complicated neck surgeries, I had the opportunity to spend six weeks each time at AHC, OP Kansas.
Initially I found the facility to be nicely decorated and it had a very warm feeling about it. Our individual rooms were very spacious and had all the amenities you could want in a rehab facility. The staff was most accommodating and very friendly.
The dining room was lovely and the food was quite good. I made some new friends and had a lot of laughs while enjoying the dining experience.
The exercise facility contained state of the art equipment. I benefited greatly from the knowledge and care of both my physical therapists and my occupational therapists.
Aside from the above, the most important thing to worry about in this type of facility is the level of healthcare provided. I was given excellent care by the doctors, nurses, aides and therapists. No facility will ever be perfect; however, I was impressed with the concern and speed in which mistakes were rectified. I found most of the employees really did care about what they did and wanted this AHC location to be the very best. This is why I chose to come home again to AHC after my second surgery.

This place is horrible. The staff is mean to the residents and their relatives. The Doctor does not treat patients with the dignity they deserve and he ignores the requests of relatives. The patients sit soaked in urine for hours and anytime the patient pushes the call light the nurses tell them they are too much trouble. This place is horrible and the pits.There is a great deal of patient neglect.
This place is a new but filthy facility with a minimum of staff. The two people on duty are not enough to answer a call, let alone provide good care to the patients. The nursing staff has no knowledge of the patients because there are no notes or instructions. Medical, nutrition and care needs are not posted. A call to kctcc is likely to be dropped or never answered. The staff hates the lack of direction from the management. The docs and ambulance providers suggest finding another place…….any other place.
I have a friend that was admitted to Legacy on 10th in mid August, 2015. I was initially dismayed by the low ratings I had seen for this nursing home. I am pleased to report that the ratings are undeserved. I understand the nursing home has undergone a change in management about two years ago. The new managers are fixing the problems they had in the past.
My friend has been at Legacy for 2 weeks now. He really likes the staff, both the nursing and care staff, as well as his occupational therapist and physical therapists. I have met all of them and I admit, I'm impressed with their dedication to their patients. I have also been told by many staff members that the staff retention is now much better. The physical and occupational therapists have told me that they have been at Legacy for 2 years and that they have over 100 years of combined experience. One of the nursing staff, our favorite lady, has been at Legacy for more than 6 years.
I am writing this because I feel it is unfair for the staff at Legacy to be affected by poor reviews from the prior management.
Zero stars. Unethical and dishonest in their dealing with family. Worst nursing home ever.
This place is absolutely not worth even one star. The ONLY thing good about this place is the caring physical therapists. They go above and beyond. The rest of the staff, from nursing care to housekeeping is awful.
My father was unfortunately in this facility for 3 months. Never once did I visit him, and find that the room was clean. Never once was the floor not so sticky, or the tray in the room that the patients eat off of, not covered with sticky filth.
Never once was I greeted with even an "hello" from any member of the nursing staff.
Absolutely one of the WORST Nursing Facilities I have ever experienced. The staff could give a damn less about the residents…If they would put as much effort into doing their job as they do texting and playing around online and avoid contact with the residents, it might be a 2 on a 10 scale. So unless you REALLY HATE your parents or loved ones, THIS IS NOT THE PLACE ANYONE SHOULD SEND THEIR FAMILY OR LOVED ONES…….Former resident

Not prepared for serious medical problems. Weekend staff is weak. Significant medication errors. Weekday professional staff are OK
Diversicare of Sedgwick maintains it's high rating by not reporting incidents to the State.
My 91 yr old father was placed here because it is the only nursing facility in Topeka that is covered by his VA benefits. This place in one word is TERRIBLE. It is not clean, has a rapid turnover of staff and in general feels like a prison with its cinder block walls and run down condition. My family is doing everything we can to get our father moved to a better facility, and it cannot happen fast enough. It feels like he is in hell right now…not a great thing for a WWII veteran. A visit here by any reasonable person would confirm this assessment very quickly.
The residents in the assisted living wing, the health care wing and the Alzheimer's unit are wonderful. The majority of the staff are very good. However many of them are very unhappy working there. Nicole and Kat in activities are outstanding. Ertha and Musa are the best. The upper management however are god awful. They are some of the meanest, nastiest, immoral, unethical, dishonest people you would ever want to meet. The turn over is non stop. The executive director position turns over every 6 months. I was terminated after volunteering for over 8 years over false accusations and lies. If you are looking for employment, a place to volunteer or a place for your loved one, go somewhere else. Consider yourself warned.
My mother in law was a patient on the rehab hall at Delmar Gardens of OP and she recieved the most horrible care there. It took then over an hour to answer her call light and the cna's were hateful and rude! When I complained to the so call administrator I was told something would be done but nothing ever happened and my mother in law actually got treated worse so we found her a new place to finish her rehab. I would not recommend this place to anyone! I spoke to other families there that were extremely unhappy with the care their lived ones got.
Staff does not communicate to each other very well. No one takes responsibility for any actions. The nurse expect the aids to know treatment plans without making sure they are written or spoken. Many aides while very caring of residents due to not knowing what ahould be checked know or not with each person are becoming jaded and closed off. My mom has been here 3 years and now is terminal. It's taken 3 weeks and a laminated note on the wall for them to stop trying to give her whole foods like chicken legs or even rice. When she came back from the hospital after the diagnosis of terminal the aids who needed to check on her at least hourly were not informed she was unable to even get out of bed. When right before she could. So for a few days she was in her own waste more than once. Again not due to the u carrying of staff but simple non communication. All so since her health changed so fast it was also a huge battle with the nurse to get her on a regular pain med schedule, because they didn't understand ahe couldn't communicate and when does is confused. I feel its more of a policy and procedures issue over a staff issue. Many of the aids, cnas and nurses are very carrying of the residents.Juat much care fall through the holes wwithout the right policies in place.
My mother was on the Express Recovery at Sandpiper Healthcare. I was kind of afraid of taking her to any rehab when she got out of the hospital, but I looked at the reviews and Sandpiper had 5 stars and a silver award. I chose to take her to Sandpiper and it was the best decision that I ever made. The nurses were top notch, the therapy was the best and everyone was just super. The staff met me and my mother when she was admitted and made us feel like we were the stars of the show. She was there for three weeks and when I took her home she back to her old self. She was strong and healthy. Sandpiper doesn't just take care of the patient…they take care of the family too. We would definitely go back to Sandpiper if we ever had the need.
Sweet Life has been awful ever since my grandmother was moved there. They are understaffed, and constantly at full occupancy. She has suffered severe neglect and abuse, to the point of permanent physical injury. All residents suffer neglect, slow help, and little attention because they do not staff properly. The care is far below what is standard, especially in Kansas. I have worked in other facilities similar to this, and I expected good things. We have been utterly disappointed. She will be moving out, as soon as we can figure out where. Protect your family. Do NOT send them here.
Sumner County Care is less than desirable. My father was there for a few months before he passed and it was difficult seeing the residents there. Often times one elderly person would be crawling around on the floors of the hallway. Others wandering aimlessly, My dad stayed in his room mostly as he was bed ridden. He passed in the middle of the month and when I requested a refund they gave me the run around for five months and only sent less than half what they owed him. Very poor office staff couldn't even tell me what the charges they deducted were for. I did file a fraud report with medicare.
this nursing home needs to get better. the nursing staff and cna's are just so rude and mean to the people that live there. i have seen alot visiting there and it is no place to live. im trying to find a better place for my mother. people that put their parents there should take them out. i have seen people get hit, push and yelled at by the nursing staff.. i have seen nurses and cna steal money, ciggs, and food from the people living there. i think the state should send someone in as a employee and give them feed back on whats going on like a undercover state person acting as a nurse or cna and get the inside story because if they know the state is going to be there then they are going to act like they never do anything wrong. it smells like the people are walking around in pee all day.. im just a outside person looking in and think something should be done about all of this. i have been in the dining room and the kitchen staff makes good food but they need better food choices.
The staff were very loving for my husband and very supportive until he passed. I will miss the staff and visits there and am pleased to know my husband was given good care during his stay. Knowing the staff and having them cry with me and hug us, showed their heart and caring. I can't think of a better place for him to have spent his final time.
If you are thinking of placing a loved one in this facility, please take time to rethink your options. I removed my father after only three weeks residency. Within hours he was admitted to an ICU at the VA hospital and later moved to a rehabilitation hospital for an additional three weeks. We do not believe he received the care he rightly deserved and needed while at Legacy. He fell twice, lost fifteen pounds, needed treatment for infections and had not been receiving the aid necessary for Parkinson's patients. Our hope is that Legacy will identify their problems, whether it be policy or staffing issues, and strive to make improvements.
