I have had the pleasure of touring the Kindred facility in Lakewood, Wa. What I saw there forever changed my concept of a Skilled Nursing Homes. Having toured many in this general area this is what I found at Kindred Transitional Care and Rehab. after many hours of touring I found:
Competent skilled nursing. Call lights answered promptly. Friendly and gracious staff even in the most difficult circumstances. When residents were out of their rooms in the hallway; every time a staff member walked past them, the staff member greeted each with a smile and inquired of their needs. The place was immaculate , the food smelled good and looked "good". The residents appeared content. I could see they were safe. As one resident was attempting to get out of his wheelchair a CNA or nurse was there immediately to ensure their safety. When asked one gentleman stated he "really liked it at Kindred". The staff their clearly loved their residents (including janitorial, and management) and were willing to go at any lengths to ensure safety, and comfort. I will choose Kindred when my loved one is in need of skilled care. I cannot say enough good about Kindred and their staff. I have been to MANY skilled nursing facilities in the Tacoma area, no one can surpass Kindred Transitional Care and Rehab. PS the Rehab department is amazing. I just spent the last two and a half weeks at Alaska Gardens. Yes I have been to other rehab centers but Alaska Gardens is by far the best rehab center I have ever been to. From Justin in housekeeping, all the cnas, to the nursing staff all were exceptional. The Therapist were fantastic including Mia and Doua and all the rest. The area was beautiful surrounded by flowers, a center court and across the street from Wapato park. I would recommend this rehab center to anyone looking for a place for yourself or your loved ones.

really would not recommend this place to anyone. The admins have no care toward the residents and do not supply everything staff needs to do there job ex:not proving new hoyers causes old damaged equitment to lead to a hoyerfall resulting in resident death. CNAS there do an awful job and leave the rooms with trash and towels everywhere and very unprofessial converstaions in public.

The whole goal of this facility is to rehabilitate residents to return home. However, the staff is unmotivated and untrained to do so. There are inoperable flow meters and suction units on the wall to give the illusion of a functioning facility which is supplemented by portable units that may or may not work in an emergency which the staff is not trained for and may or may not respond in time to.
Policies are quoted at convenience without evidence of actually being a true policy. Medicare and private insurance monies are spent without regard to actual production of outcome. All residents are fed the same diet despite restriction status and placed on insulin making medication and outcome costs higher as well as exposure costs greater.
The education of their contracted respiratory staff is terrible and highly lacking. Their management is not held to standard of registration status and is only certified. Basic respiratory concepts elude half of their staff and constitute infection control issue and high cost in care. The lack of consistency in pulmonologist care adds to this issue and ignorance. Better suited would be a contract with local pulmonology which is who treats them with their frequent transports to Tacoma General and St. Josephs.
In short, there is a lack of available facilities for this area to send their family members for rehabilitation that have advanced airways or ventilator needs and Park Rose has taken advantage of this and is making money off a horrible situation. I addition, they are in continuous remodeling, or basically have signs up to make excuses for a crumbling inside.

As a healthcare professional, I have yet to see a patient in good condition come from this facility. Poor skin/pressure ulcer management. Overall poor patient care.

Fine for short-term stay (rehab/recovery from surgery, etc) BUT BE PREPARED TO PROVIDE YOUR OWN FOOD! The food service frequently serves ill-prepared and incredibly bland meals. The best item they have is a chef's salad, and the soups are quite good, but I could not live on those items. I was constantly hungry (I am not overweight) - thank goodness for pizza delivery.
If you are diabetic and experience falling blood sugar during the night, bring your own orange juice, etc. The kitchen is locked up after 8 pm or so, and while the nursing stations receive an allocation of snacks every evening, the only items available in the wee hours seem to be their regurgitated cat puke sandwiches.
The occupational and physical therapists are top-notch, and the nursing staff are mostly conscientous and friendly.
I recommend Alaska Gardens for rehab, and I would go there again, but only with my own food service.

There is a reason that medicare and the state rate this building so low. I was here for 4 days and would not recommend this place to my worst enemy. Very loud, food was cold, and it took two days after I arrived before my first therapy treatment. If you need therapy this is not the place.

This amazing facility did a fantastic job helping my father recover from a broken hip. The therapists and nurses were all very helpful and loving. With out them I don't believe my dads recovery would have gone as well as it did. If you need a place for therapy this is the one!

Every place has its issues and my family and I can be hard to please. I can honestly say that the administrator and nursing team do sincerely care and take Care of the problems they can.
I highly recommend this building. Not everything will be perfect, but these guys care and respond.

My 80 year old mother was a patient was a patient here for 8 months before being moved to a better facility able to work with patients with Dementia. Do not choose this facility if you want compassionate care for your elderly. This facility is for profit and it shows.

Don't send your loved one's here. The staff incompetent. Managment is rude and insulting to the residents/Family members.
Staff does not pay attention to details. There lack of attention caused my mother to have avoidable injuries.
Finacialy thye will hurrass the resident /family members. I would give this facility a lower score if I could.

the staff took good care of my dad after his heart surgery. i really appreciate the patience of staff because my dad can be really hard to work with! the food cold be a little warmer but overall it was pretty good considering there are a lot of people to please and they provide variety. the therapy team got my dad back home, thank you!!!

Care of my grandma was awesome after her knee surgery. Rehab team was top notch. If you are going to a nursing home for a short rehab stay this is the place to go.

That place has the worst care for patients..It smells very bad..Patient r not takin care pf properly..Food is left in there rooms for days..Patients r left in hallways for hrs..Also they can be forgottin that there outside for hrs..some have fallin asleep outside in there chairs and not brought in till early in the morning..(Ex..patient went out to get air and fell asleep not to b brought in till after 4a..)..Im sure there r nurses that do care..But in all I would never put my family member or send anyone there..

If I could give it less than 1 star I would. I strongly suspect that someone there is paying off adult protective services caseworkers to serve as collection agents. We were given incorrect information about coverage, and they lied to me about the laws regarding payment. I made the mistake of saying to the administration that I was getting a lawyer. The next day APS contacted me. Lawyer said what I was doing was correct. Financial caseworker for DSHS said what I was doing was correct and completely legal. But APS didn't investigate they simply harassed me to pay them. Administration there would harass me when I tried to visit, and harassed the resident, denying them rehabilitative care and telling the resident lies trying to confuse them. We had to sell off the resident's physical assets to pay them in order to stop the APS threats.

i am a resident of Avamere Skilled Nursing of Tacoma. The Nurses and Aides are very good and caring. The one bad issue is the food. most of the meals are not good enough for my dogs. I ate a grilled cheese sandwich for dinner that was cold and I was not able to just bite into.
There is a new Administrator, who is making a lot of very good improvement

I found them to be lacking in qualified personnel, although some people there are very caring, the staff are simply after generating cash flow and lack empathy . Ive seen some pretty bad care given especially when I had to insist that my father had a breathing problem and they shrugged it off until I insisted that a doctor take a closer look and it turned out that he had to go to the intensive care unit with pneumonia and died two weeks later

Alaska Garden's employees are very rude to residents and are not attentive to residents. Food is always cold and never delivered on-time and residents and residents are miss treated by not being able to go to bathroom when they ask.

I have been requesing an itemized bill from Life Care of Puyallup since January, the numbers just don't match up with what I was told it would cost for the level of care my mom received. Their response was that if she is a cash paying resident it would be $9,000 for her level of care per month, but when I received my Mom's supplemental insurance break down it was more like $25K for 3 weeks, their explanation is that they don't have a set charge for one individual item it fluctuates daily. They have threatened me with court if I do not pay the supplemental amount within a week, they accused my mom of forging a signature, they have hung up on me when I challenge their statements. I know that money is the root of all evil, but to be a professional establishment and yet lack the professionalism that is required when dealing with a customer is unforgivable. I want an itemized bill, I do not understand why they are still billing Medicare 5 months after my Mother's discharge and getting paid. Is there any oversight to facilities like this where a person can call and get the answers that are rightfully theirs to have?

Very unprofessional atmosphere, too much turn around with nursing staff and care staff. Continuity of care is very low in this facility, and very poorly managed by the management staff.
