My mom was a patient at Green Hills and I can positively say she was very well cared for. It's unfortunate that some other post are negative but maybe that's because the family doesn't visit enough. We took the position to be "part" of the care process, knew the staff and participated in my mom's care. I guarantee you that the other patients and family members we came to know were very happy with Green Hills. Thank you to all the staff who made my mom HAPPY!!!!

My father stayed here for several weeks. The staff was friendly but communication is poor to non- existent. Lab results were lost or delayed without explanation. Had to be hospitalized for bedsores from improper care. Not enough staff for the needs here. Buzzers and bells are ringing constantly at night. I think there must be a management problem.

It's a shame that I can only give this facility a one-star rating simply because their business department isn't at the same quality as their care-giving staff. My father passed while in their care leaving my mother temporarily without income to take care of funeral and medical expenses. While waiting for the financial fog to clear, Lifecare has made major errors in refunding the credit due my mother. Every time we call to inquire, there's always an excuse. My father's stay had to be paid in advance, as if paying for rent. However, he passed before taking advantage of the prepaid rent. My father has been gone now for over a month and still no refund for services NOT rendered.

I have had a family member at Madison Health and Rehab in Madison TN for over 9 months now. With all the issues I have had with incorrect meds given, not working with the patients, overdosing on ativan to keep patients quiet, not caring to a point where they have recently made many changes. I hate to report but the changes are still not good. They lie to you, do not follow doctor orders, keep removing orders from system and do not take care of residents personal items.

I have since moved my family member to another facility.

CMS is correct in their one star assignment. There is a reason for it.

This nursing home should be closed down. They are severely understaffed. They are currently experiencing a major outbreak of some type of stomach virus which we cannot find the name of from staff. At least one person has been hospitalized for the last seven days. The problem is directly related to the understaffing. The rooms are often filthy with urine and feces. The patients hair is almost always dirty and filled with cradle cap. They go days and sometimes a week plus without a bath. The laundry room is filthy and extremely disorganized. They have given drugs against my wishes to make the patient sleep constantly.

There is not enough staff to feed all the patients that cannot feed themselves. I could on but I think you get the picture. I truly need to feel that are certain staff that members that really care for my family member but are just overwhelmed with the impossible task of doing everything. The owners need to take some of their money out of their pockets and hire more staff.

This place is a dump! Smells bad, lousy staff, not clean.

Everyday, they position a circle of droolers by the 2nd floor elevator for recreation. That's it. That's the only recreation offered.

If you can be there everyday to monitor the situation it is acceptable not good. Otherwise, forget it.

They are only about the buck!

My mother went into the hospital in May 2012 for hip surgery. They discovered an infection which meant the surgery had to be put off for 6 weeks while she recovered from the infection. She was sent to Green Hills Health and Rehabilitation Center where she received EXCELLENT care from the day and night staffs who were very cheerful and willing to respond to questions day or night.

Medication was given on schedule. Meals were brought on time. On days when she was unable to eat, they provided Ensure and Mighty Shakes. She even had snacks in the middle of the night.

She was given limited physical therapy while recovering from her infection and more intensive physical and occupational therapy when she returned in July after her hip revision surgery.

Another thing they do is take your religious affiliation into account. As Catholics, we were used to going to Mass but of course, couldn't while she was there. A member of the Day Staff showed me where the Chapel was and made sure a Eucharistic minister was notified to come to my mother's room so that we could receive Holy Communion.

They also have activities during the week and Pet Therapy on Saturdays where a member of the Day Staff brings her dog to the residents rooms. It really is a morale booster for everyone esp if they have pets they have to be away from. On Sundays, families can bring their own pets to visit. This is wonderful therapy and I wish more places had this.

When we left the facility in September for the last time, we had tears in our eyes. Everyone there had become our second family.

I highly recommend Green Hills Health and Rehabilitation Canter for anyone who needs physical therapy or a nursing home. It really is a home away from home where everyone goes out of their way to make your or your loved one's stay a pleasant one.

The do not follow the doctors advise, the techs do not follow the staff advise. The they were rude when the talk to the family, and just lied to their face. The techs like to party when no one is around (Christmas Eve 1012 they brought in their beer). They don't check in on the patients during the day enough.

My mother just left this facility to be admitted back into the hospital with a new UTI, eye infection and a bedsore she did not have upon arrival. In addition, several life threatening medical issues were ignored even after I brought them to the staff's attention. No communication between the staff or oncoming shifts. OT and PT were very good though.

very caring….this is one of the cleanest Nursing Homes I've been in….the nurses treat my sister in law very well….all the staff is awesome…They feed the people in their good….food taste good…I have ate with my sister in law there…I really do not have any complaints….I would give it 5 stars all around..

I would like to take this opportunity to recommend Life Care Center of Hickory Woods in Antioch, Tn and their staff for the care and concern my husband is receiving during and extensive brain injury.

Not realizing how important it was to seek a skilled rehab center with exceptional therapists, We was fortunate to be referred to your center for the specifics of the therapy he required, for as you know he had undergone very involved brain surgery, with little to no mobility when he arrived at Life Care Center of Hickory Woods.

I was pleasantly surprised how comfortable and welcome we felt as soon as we arrived. Lois, Paula, and Ray worked tirelessly with him and made each therapy session very pleasant and uplifting. The CNT’s and nurses became part of our extended family.

Meeting you allayed my fears, and he soon began to realize that if he worked with you, results would be seen. It certainly was not easy and there was of course discomfort and pain during parts of the therapy, but with your guidance he will be able to overcome these problems and finally reach the functioning level that he was at.

Observing the other therapists, made us feel secure that he was in the right place.

Life Care Center of America in Hickory Woods will be recommended by us because of your work ethic, professionalism and care, which is an asset that you and your staff should be proud of.

They allow married and non married couples to work there and one couple that works nights are having sex on the job. A black guy last name brooks . So beware of the care your family members receive mainly at night when supervisors are gone.

My Grandmother received therapy at McKendree Village and she had a very positive experience. She always talked about the staff kindness. It is clean with no odors. I think it is a great place.

Go somewhere else if possible! My loved one ended up here, don't know why. Day staff has been good, food is what you would expect (not good, but if you're hungry..). Night staff does not give pain meds as scheduled (brother in moto accident, needs pain meds as scheduled, no more no less, has tried to be tough). He complained, and they delayed that night's meds for 2.5 hrs (he had significant elbow surgery day before) and nurse made comment, "Well, I guess you just need to complain, don't you?..words to that effect - formal complaints being filed with corporate office. Director treated me like I was stupid. No response when I called during the night. Family lives outside of area (Alaska vs. Tennessee) and concerned family member will not have proper care - this is very concerning - fortunately family member young enough to be vocal (not enough though, he tried to be patient and polite) and am concerned about elderly and infirm in this facility.

A relative of mine stayed at Life Care of Old Hickory for 5 weeks. This brand new facility is beautiful and the dining service, maintenance crew, and therapists were wonderful. However, the evening Tech staff was seriously understaffed. I walked up and down the halls many nights looking for a Tech or nurse to ask questions. It was eerily empty. Rarely was anyone at a nurse's station. Lights flashed and buttons rang constantly with patients waiting as long as 45 minutes to be seen. I timed it, so I know. The other issue I had was poor to no communication. I left many messages with nurses (again, at night) that were never returned. Nobody to let visitors in between 8pm and 8am, so if you want to visit at those times you must call to be let in, but many times nobody answered the phone. Once management gets their act together and hires enough staff, this will be a quality place, but I cannot recommend it yet, for the lack of support, security, and supervision at night.

I have a family member there she seems generally happy. If we could afford it i would put her somwhere else. Its not a very clean place nor does it smell good. They keep the soiled diapers in the hall instead of in a closet away from tenants and guest. This cant be healthy. The nurses arent very.friendly with the exception of a few. Overall not a great enviroment.

We absolutely love this facility and all of the staff and workers!!! Everyone is so loving and caring as well as professional. The rehab therapist are the best!! My 88 year old mother has thoroughly enjoyed both of the extended stays that she has had at LCC of Hickory Woods, and has made many friends (among residents and staff and workers) both times. We went back to visit almost every week between her two stays, and will continue to do so after she comes home this time.

This is a pretty facility. That is the only nice thing I have to say about it. The care given there is TERRIBLE and all you get are excuses, for example, as to why your loved one (who has bedsores) was left sitting in an uncomfortable wheelchair for over 10 hours! There is no excuse for that! Oh, and they're overly proud of their chef. That is the main feature that they'll try to sell you on. Yes, the food is pretty good as institutions go - it's just that this chef has no idea how to cook for old people. There's no simple food on the menu - it's all fancy stuff. Old people don't like that.

If you are thinking of putting your loved one in this facility, please think again. It is the worst facility I have ever encountered. They do not tell their staff members about incoming patients so they have no idea what is going on with them, they do not provide basic care (brushing teeth) for their patients, the patients sit in their own waste for 1-2 hours, half of the techs are very rude. They are unaware of the signs of infection. The nurses are very nice, but are stretched too thin. PLEASE do not use this facility if you care about your loved one.

I have been to so many long term care facilities looking for the right place that feels like home. Bethany is by far the warmest feeling location I have found. The building is fairly new and much of it is up to date with flat screen TVs for each resident. There are various day rooms, sun room, and patios for residents to mingle. The is even a gorgeous beauty shop with their own beautician 3-4 days each week. I have seen and heard so many great stories of healing from those who have gone through therapy here and the staff really go out of their way to make the residents feel at home. One thing I found incredible was the change to meet and talk with the CEO while visiting. I have never been given that opportunity elsewhere. I would recommend Bethany 100% for any and every family member or friend!

Family member nearly died after a bed sore became septic. This is a preventable issue. Nurses are trained to watch for this. The bed sore should have never gotten so bad that the patient had to be sent to the hospital, spend days in ICU, then to have to undergo multiple surgeries to repair the wound.

I have a loved one in the nursing center……….I would rate this facility on a scale of 1-10, about a 6 overall. My biggest complaint is the number of no-show certified nursing attendants that do not show up for work on the 3-11 pm shift. Which means they are terribly understaffed…….what happens if there is a fire????????? Also, if a resident rings his nurse call button, it takes way too long for them to respond………..during the daytime they are also understaffed.
