My father just spent a month in the short term rehab unit. He can be difficult to please but was very happy with the facility, staff, PT and the food.

This is the worst place to put a loved one. It is an absolute dump. It smells really bad, and the staff turnover rate is very high. Patient belongings get stolen by the staff and management does nothing. Extremely poor response times to call lights, incompetent patient care of bedsores and bacterial pneumonia. Senior management is rude and disrespectful of patients and families. Don’t put a loved one here!

My Mom was here for rehab twice (2016 and 2017), and I can’t say enough good things about this center. The management is great, the facility is spotlessly clean, and the nursing staff is outstanding. Everyone took wonderful care of Mom, and Mom loved the people. I’m grateful for the loving care they gave my Mom.

I admitted my Mother five months ago to Shangri La Rehab and Living facility only to be very disappointed by there service. The administrative staff seemed very pleasant during the first day of having her and everything seemed to be in place. Nurses were doing their job and residents seemed to be very happy. How wrong I was. First week I visited my mother she seemed very different. A once optimistic and talkative lady was now quiet and very paranoid about her surroundings. At the first visit she didn't tell me what was wrong and I just assumed it was a shock factor for her to be taken out of her home and placed somewhere new. As the months passed I could tell my mom's health was declining. During one visit we were in her room and the bed was a mess so I figured I would just go ahead and make it (my mom being a very tidy and almost neat-freak her entire life) so my disgust I found bed bugs crawling under the seems of the mattress. There must have been at least 100 right there. I instantly went to one of the CNA or Nurses in the hall and told them about the problem only to be shrugged off and be told "We're working on it." while she walked off with a very uncaring attitude. I went to the Administrator of the building afterword's to discuss what happened when I tried to tell a nurse but to find out the Administrated was just as bad. The man was in a very obvious annoyed mood as soon as I entered the room. For who I assume to be a nurse(maybe wife?) leaving the room in tears. I ignored it and brought up the nurse who was rude to me even though I didn't catch her name I had the description of her and what she was wearing. After telling him about the nurse and the bed bugs he gave me an eye roll and shook his head in disbelief. He told me that the bedbugs were going to be exterminated tomorrow and they already knew about it. My question was why didn't they contact me about the bed bugs so I could've removed my mother for the time being. It's cruelty to have them laying in bed bugs when my mother can barely move on her own due to Parkinson's Disease. So it took me about a month later to visit my mother since I was out of town on a business trip for about three weeks. The bed bugs were still there and not only that I use to be a CNA in my younger days and noticed something awful. My mother was complaining about pains on her sides so I looked for her. She had about a Stage Two Pressure Ulcer. I was livid and continued to look at my mother for anything else. Her nails were very overgrown on her feet and her dentures had to have been soaked in hot water since they were now warped to the point that they didn't fit in my mother's mouth anymore. My mother also had been telling me at night time that the nurses were so loud in the hallway screaming and fooling around that it was hard for her to sleep. As well as that nurses would take thirty minutes to answer a call light during the day and often would tell her that they would be right back and then return when it was time to get them up for lunch or dinner. They let her soil herself and would not change her until it was night time again. The last thing she told me before I discharged her out of that facility and moved her to the Groves over in Lee Summit is that the Dietary staff was very mean to the residents and refused to give them snacks when they asked. She told me the Asian lady was incredibly rude and would cuss at her when she start to leave. This place is awful, dirty, and very distasteful. I suggest if you love your ones in need that you do not place them in this horrible facility.

This facility is old, shabby and outdated. Unless you bring in your own furniture, the furnishings are ratty. We had a few nice workers who care, but for the most part the care is hugely inadequate and sloppy. Good food, but the care is really, really bad. I won't go back

The staff a Lakewood was wonderful to our mother during her stay there. They treated her like family and showed as much concern for her well being as we would. From the nurses to nurses aides, housekeeping, dietary ad therapy staff to administrative and social services they were top notch. I have worked in this industry as front line staff and management and realize how difficult it is to care for the elders with the reimbursement provided them. They have done an excellent job. When clearing out the room, staff found $5 that we left in a drawer. They turned it in to management who then called us to let us know. Wow. That says a lot for what kind of individuals they employ. I would recommend this facility to anyone. The Pflughoft Family

The Groves has been taken over by Rosewood Health and Rehab. When we first moved my Mother into the Groves with Alzheimers, she was well cared for, in a Christian environment.. She paid a portion of her care (about $56/day) and Medicare paid the rest.. . When Rosewood took over, care gradually began to diminish- still ok… but little by little, the staffing and resident interaction shrank to the minimums it is today When she passed away 5 years and $95000.00 (of her portion) later, she did so 6 days into a new month. For the last six days, she ate no food, and lay unresponsive in her bed, attended only by Hospice. Medicaid paid her expenses for the six days, and I called and offered to pay the $56/day that she had been paying for 5 years. But suddenly that was not good enough. They sent me a bill for $875.00, because that is "just how we bill". We had some extra funeral expenses, and cannot afford this. They told me that unless I paid their bogus balance they would not give me my photographs and oil paintings, which were done for her by my late, arthritic father and are of no value to anyone but me. This caring, Christian place has turned into something dark under Rosewoods' management. Do your loved one a favor and look elsewhere. Beware!

The person that answered my very first call - and therefore the first impression - seemed very 'put out' by my call.

I said I was looking for a facility for…and she interrupted me and I was put on hold. She came back and said the Dir. of Admissions wasn't there, to call back at 10 tomorrow.

As I needed to know today, because the doctor wanted to know where they were to go, I asked if they accepted a specific insurance. She interrupted again, that she didn't know anything. I figured a nurse usually knew…but…

…you guessed it, I Won't be calling there again!

Recent past year drastic change in employees. No one is happy, scowling faces. Too few staff. Daily hygiene of patients and facility are poor. Complaints get you nowhere. Question whether new ownership, severe neglectful treatment.

Quality and food choices are pathetic, giant scoops up starch packed on plates. Nothing fresh is offered. They just don't care.

If you're taken on a tour they keep you in the rehab new side of building where there is nicer furniture, but if you go into regular semi privates rooms you see ancient beds, poor thin mattresses and beds and sticky floors and furniture. Some days the smell of urine and feces is so strong you gag.

This is a shameful operation.

My father-in-law was a patient there before he passed away. The care, compassion, and the cleanliness was outstanding. The nurses were professional, caring, and really knew there stuff. I realize that no one is perfect and that people make mistakes, but they owned up to anything we preceived as being wrong and they proved to find a solution. We are glad to have been there and would recomend it to anyone.

6/23/14 Express Rehabilitation of Independence is a joke. The doctor didn't even take the time to visit my mother but once the entire time she was there. He judges his version of care and pain medication on the statements of LPN's most of them just out of school. I tried to talk to him but he said he didn't have time to talk to me. My mother was very discouraged with her care or lack of it. Every time I left, which was seldom, to run an errand or go get a bite to eat I came back to my mother crying in pain. The employees lied and blamed one another to cover their errors including the LPN in charge. Therapy was disgraceful. I expressed my mom right out of there. Don't leave your loved one there unless you are going to stay with them 24/7. The LPN's need a lot more training, continued education, and monitoring. I feel so sorry for the people there that don't have anyone looking out for them.

Med Wing suggests plans for paitents but then do not follow through….One must monitor this area of Armour Oaks continually and never assume what you are told is what is being one.

If I could give this place zero stars, I would! I worked as a Social worker in this place with one specific individual for quite some time and found this facility to be an absolute NIGHTMARE!!!!!!!! It is filthy, the food is deplorable, putrid, disgusting. The caregivers are insensitive and in some cases mean & rude. There is a certain facade that the general public will see, but behind those electronic doors, it's a whole different ball game. Everything you see on their home page is a lie. The rooms are absolutely FILTHY! As a professional, I could not do my job because I was so busy doing the job of the facilitators. I had to change my patient's sheets, clean his room, help him with his personal needs. Those people who aren't fortunate enough to have an outside case worker (this is most of them) are in for a very rude awakening. I used to call it the Cuckoo's Nest, but it so much worse than One Flew Over the Cuckoo's nest. If you even consider putting a loved one into this HELL, you are making the most grave mistake of your life. My eyes were opened to this place on a weekly basis. The only reason that I do not try to legally do something about it is because I am afraid that other places, God forbid, might be worse!!! I've seen this place go through inspections that PASSED (I don't know how), I've seen patient complaints to authorities turn on the patients, and I've seen retaliation. One is almost afraid to seek recourse or make complaints because the entire system is ROTTON TO THE CORE! They take people's social security checks, feed them slop, don't adequately care for them, AND they get by with it!!! I would LOVE to see a full blown investigation of this facility - but I'm deathly afraid that other facilities are like this. OH! And don't believe that BUNK about them having a Doctor, a barber, qualified nursing staff, etc. It's all lies! My patient went WEEKS with 2 separate injuries and I begged and begged for a doctor's care. They don't have one. They get ahold of whatever doctor will take medicare/medicaid (and there aren't many) and it can literally take WEEKS for the patient to get medical attention. This place needs to be shut down! For the LOVE OF GOD DO NOT PUT YOUR RELATIVE IN THIS PLACE!!!!!!

The nursing home has problems notifying my either my father-in-law (patient) or mother-in-law about appointments with outside providers. We have talked with the nursing home about keeping the son and mother involved and that continues to be ignored by the social workers that schedule the appointments. We've been told by the Director of Nursing that this would not happen again and yet today we are dealing with the same issue again. My mother-in-law visits my father-in-law multiple times a day everyday. And still no one can seem to find the time to tell her or my father-in-law that there is an appointment. The excuse for not notifying the son or daughter-in-law is that it's too many people to notify. They are not even telling my father-in-law and he is in the facility.

This facility is absolutely fabulous. The nurses are great and the facility is absolutely beautiful. Dont be fooled by the medicare ratings which are old and based on the previous owner. My mother is at this facility and whatever she needs they provide and its very clean. The clean all day long there is absolutely no smell. The beds are electric, the TV's are flat screen and wide, each room has their own heater and air and BATHROOM with a shower. Its unreal. I would highly recommend it.

We have been fortunate to have several of the health care providers be of excellent quality.Fortunately they are the norm, but there have been a few folk that have made this difficult. Transitioning to a facility is difficult, and that process was not made any easier by certain "professionals" not taking their duties seriously . We are also very disappointed in the quality of the food and the lack of personalization of a diet plan. The nutritionist hides in her office, and refuses to speak with us despite repeated requests. Any and all issues could be resolved by visiting with the Nursing director, or the director himself, but its not their job to discuss dietary needs with the family. We would have scored this facility higher had all personnel shown the same dedication to excellence that a select few have shown. Those few shine like jewels in a very rusty crown. We are thankful for them, but wish the rest of the staff would get a clue.

This is one of the worst nursing homes I have ever been in. The way the nurses and "bosses" talk is very distrubing. It is a filthy place and should be closed down! I feel sorry for all the elderly people that are subjected to this life style forced upon them. I am thankful that i chose to take my family member somewhere else!
