Manor at Perrysburg Perrysburg, Ohio 43551

My father was a patient in the skilled unit for several months. Being a very particular man, he was most pleased with his care, the food, and his surroundings there. Staff was most friendly, patient, and caring. At discharge, the staff approached him with hugs and wonderful wishes for recovering further. Our memories of his stay there all positive ones. He is now with home hospice and we are seeking respit. His wish is to return to Manor of Perrysburg were he feels comfortable and cared for. Kudos to The Manor at Perrysburg!

My Mom was at Avalon in Monclova, just a few miles from Perrysburg's location. She lived there for a few years and was the first resident in their new home. She had excellent care while there by a very caring staff. I would recommend them to anyone again. Even the morning my Mom passed they allowed the entire family of 16 of us in with her to hold her as she took her last breathe. Then offered whatever we needed to each of us, rather it was a hug, share a tear or just a bite to eat and some coffee. I will always remember their kindness. The family of June Baxter

They need to have hand sanitizer accessible to the pts as well as the nursing staff.

My urinals are always full and this is observed by the staff.They will bring my medicine or food tray in and set it down right next to bottles of urine and it is not until you bring it to their attention do they react or tell you I will get the aid becasue I am the nurse.

I have also been told "Oh I thought you and your family did everything"

Call light not answered in a timely manner. My room doesn't get cleaned unless asked and that may take days.

Patients that are able to communicate all express their concerns to one another about their care.

I feel sorry for the individuals that do not have a voice or someone from the outside to assist in their care.And it seems when you express your concerns you are not heard and there is no follow up. Most importanlty they consistantly run out of my medication for pain and when the medication is in stock it is never on time, you have to wait on most occasions 1hr past the time you were due.

I do feel the therapy & activity department/staff there are wonderful though.

Mom has been in this facility for 2 years, and I think they are doing a fabulous job of taking care of her! The building is beautiful…rooms are decorated "just like home", the view of the wooded area is lovely, the food always tastes good, the nursing staff is A1. The type of place you would go to yourself if able to make that decision on your own.
