This facility is a nightmare. It is not even deserving of one star.
My mother was a patient here.
During her stay she was left to lie in her own waste for hours.
We documented multiple medication errors.
The wrong food trays were delivered to her.
The physical therapy staff had her working on "incorrect" equipment. (we learned this after her discharge when a home therapist saw the equipment she'd been using at Boulevard)
The aides spoke little or poor English. Her "call" button could be lit for hours before anyone responded.
Communication between patient and staff was minimal or non-existent.
The food was virtually inedible on most days. My mother was existing on grilled cheese sandwiches and Ensure.
The management can appear friendly- even charming. But be warned: no one here is capable of responding in any meaningful way.
Because my mother was fully alert, fully aware of her needs, fully cognizant of the lack of organization and recurring mistakes at Boulevard she was seen as a "nuisance" patient.
She is now is another facility where she is treated with respect and dignity. She is getting attentive care, good food, excellent physical therapy and her prognosis has improved 100%.
Boulevard Rehab is truly bottom-of-the-barrel. You have alternatives. Give your loved one a gift. Don't bring them here.
