Blanca was incredibly helpful during my mother's admission into Seaport. Their rehab team is really top notch. They provide many little extras here because it is an independently run facility.

Although my father never actually stayed here, we were told that our first choice was full and we were encouraged to go to Seaport. I went for a visit at around 7pm, which is the best time to go for a tour because you can look around on your own and see how the care really is. I walked down the hall and saw rooms that you could barely walk into it was so tight, the smell was horrible, and I witnessed a nurse ignore and walk away from a woman who asked her to help the man in the room with her husband who was sitting in his own feces for over two hours. I'm surprised I didn't leave sooner than I did. Thankfully we were able to get my father into the place we wanted after all.

I'm trying to figure out how there could be a single positive review on this place and after scrolling through it hit me, the "positive" reviews have been made once each month around the same time and are obviously written by the staff/owner. Obviously someone set a calendar alert reminding them each month to write positive online reviews.
THIS PLACE IS A HELL HOLE. Don't let them suck in your loved one. My mother died there after the nurses made a medication error. The woman in charge of nursing had the nerve to pretend like she didn't understand English that well when me and my brother questioned her about the medication, yet we heard her speaking perfect English to someone the day prior.

Seaport is way above average when compared to facilities in the area. We visited others when looking for short-term care options for a sibling. The staff was really kind and the facility felt more comfy than most. It is located in a great area next to hospitals and places to eat. I highly recommend their services.

The floor were my patient always smelled clean and fresh, which is a good thing. It is a very organized, clockwork facility, tasks done on schedule and medicines given timely. There is a prompt response when the call button is pushed. Sadly, the food is overcooked and unappetizing looking. I took a photo of a dinner one day with my iPhone. Portions for patients with little or no appetite are way too large. We made home cooked food and brought it in on occasion to tempt the appetite. The best thing is that staff are all gentle, emphatic and kindly.

My stay at Berkley East after hip replacement surgery far exceeded my expectations. The nursing staff were all very helpful and the physical therapists were beyond encouraging, especially those mornings when the last thing I wanted to do was get up and work out. This place is not your typical nursing home. It was more like staying at a hotel and getting five-star physical therapy and nursing care.

If you care for a loved one, whether a family member or friend, AVOID the Rehab Center of Santa Monica!!!!!
My grandfather (age 87) fell and fractured his back. When in the hospital, they recommended several rehabilitation centers nearby in Santa Monica since he had could not walk nor sit any more. After speaking to a physician and some faculty at the hospital, we decided on the Rehab Center of Santa Monica because we heard they recently renovated, it doesn't smell horrible, and the close proximity.
When we got there, it was nice, pretty clean, and the nurse came in to learn more about the diet to prepare meals which he will eat. Once we all of us relatives left, than the problems began. I will provide a few examples:
1. One day my grandma went to visit in the morning. She found that he stayed asleep till late afternoon and wouldn't wake up. My mom asked a nurse if they had administered a sleeping med, the nurse denied with attitude because we clearly stated prior we didn't want him on sleeping meds. My aunt asked, "I don't believe you, let me see your med logbook." There was no documentation of administering a sleeping med. My aunt later asked another nurse who claimed, yes, they did administer a sleeping med. The other nurse had lied and they didn't document the med they administered!!
2. When we first took him there, we informed multiple nurses, "he rejects taking meds, you must crush the meds, mix in applejuice, and force the intake". He was on a blood thinner. 3rd or 4th day, had had major confusion. We later realized, they didn't administer his blood thinner and he got a stroke while residing in there.
3. My aunt found a big bump on his abdominal area and we asked the nurses about it, they didn't even know. It is the nurses job to make sure the patients stay healthy!!
4. One day I was visiting, he wanted to roll over on his side and we needed assistance. I went outside and asked a nurse to help, waited but received no assistance. Then, I asked for them to come in and crush a Tylenol so I could try to get him to sit down….no response. I told a nurse to come put on a diaper because I needed to empty his bladder, he took 5 minutes just for that. After I went down stairs and informed the manager of these issues and threatened that we will transfer him, all the nurses came running in to finally take care of everything.
5. The final day, we aunt went in and noticed he could barely breathe. He immediately called in a nurse and they found his temperature to be sky high. If my aunt didn't go there, he would have past away within hours.
5. He finally left the Rehab Center of Santa Monica because he acquired Pneumonia while in there. Now hes back in the hospital with IV antibiotics with more fatigue than ever because of that horrible place.
When seeking a Rehab Center, please visit the location. Not just the lobby, but the actual rooms. Learn what is the ratio of nurses to patients. Are any nurses actually sitting with patients conversing or are they running around like they have way too many patients? How is the attitude of the nurses? Even spending 1/2 hour there might enlighten you.
It's all about business. All they care about is the money. They just try to make family members feel comfortable and hide the fact that there is a lack of sufficient nurses. They spent a ton of money remodeling the lobby just to attract people, but the patient care will lead to the illness and possibly death of your loved one.

This place is AWSOME!!!! From everyone that works there, it's clean, welcoming, and not crowded. They are all committed to the well being, and dignity of the patients. The patients are so well taken care of. So nice to know that these places do exist!

This place has such a good location. Very close to two major hospitals. The staff was really kind to my father and welcoming to family. They even let us bring in our little dog to cheer up my father after his hip replacement surgery. Parking can be tricky sometimes, and you have to pay close attention to the street signs. I really recommend taking a tour at the very least.

I think Seaport 17th must have improved a bit recently. I read some of the reviews and was surprised by the 1 star comment. On the tour they showed me that they were a Medicare rated 4 star facility. The rehab staff is super! The location is excellent, and my mother loves being near the hospitals in the area (UCLA/St. Johns). Parking might be an issue for some, but this place is worth a visit. I gave it a "four" because of the parking.

Seaport has changed for the better. My father was at Seaport 17th in Santa Monica for rehabilitation after his knee replacement years ago. It wasn't a four star facility then. My mother was a recent patient there having rehab work done after shoulder surgery. The facility has improved greatly since my father was there. The rehab and nursing staff treated my mother with respect and she actually enjoyed their company.

Having spent 2 months in rehalibation, SMHCC became like "home" to me. It was clean and kept that way, employees always smiled and were understanding of elder mishaps and vocalizations. Meals served appropriately, every presentation balanced and nutritional. Restrooms clean and serviced. An older building, but cheerfully furnished, having clean beds and a tremendous physical therapy department. Staff went out of their way to ensure each resident received therapy within their ability. Old, dying, or recovering, each person was shown value.

My father is currently at Berkley East and I have been so pleased that I wanted to share my story with others. My father has been treated with nothing but compassion and support by all the nurses and the physical therapists are miracle workers! He has a very large room and although he does share it with one other patient, they made sure they got along well. My father's first roommate was lovely but he wanted to talk with my father all day and wouldn't let him rest. Rather than having the risk of my father hurting his feelings by asking him to not talk, the staff was more than helpful finding another roommate for my father. It all worked out in the end because the other patient's new roommate needed a friend too and they seem to enjoy talking with each other all day.
There are many other examples I could give to how accommodating the staff have been. If you ever have a question or concern, all you have to do is ask and you shall receive.

The staff is very friendly. My mother & I were very pleased with the therapy she received after her hip replacement. They were very thorough and courteous. The grounds were not the most tasteful in style, but I think it was the staff that made the place seem so nice. The facility is also very close to UCLA Medical and there are places to eat within walking distance if you're visiting a loved one.

If you have no other choice then do not go here. I could not believe what they served for dinner one night. Would you believe they served CELERY as a Vegetable. Now how do sick patients get well eating that. The nurses do not respond to you when you call them. This is not a place I would put a DOG.

awful rooms - very depressing for a long term stay. great cna's - very helpful if you are infirm - response time can be long but they are caring and keep residents clean. nursing staff is terrible - won't answer questions or address issues. i was so sick i almost died and the nurse would not allow my friend to take me to the hospital - she called a private doctor to come to the facility to verify the urgency - was admitted to the hospital in critical condition. i did like the physical therapy staff and saw them doing some great things with other residents but did not get a chance to work with them.

rooms are nice - pt staff is decent - for short, term rehab on healthy patients a good option - group rehab only. DO NOT GO HERE IF YOU ARE SICK!! nurses/cna's non-existent!!! 400 people hanging out in the lunch room but they only come in your room if they absolutely have to!! during the course of my stay i progressively more ill and never received help - even when extremely ill and needed hospitalization I received zero support from nursing staff and had to call 911.

Piece of crap they didn't take care of my dad from may 2010 -June 14,2010. One time that stupid bed was burning up with him in it and stupid nursing staff was busy watching the nba finals! You called that professional? I saw other patients fall off the bed and no one picked them up. And racist so call nurses looks like wanna be from 8 month credit schools thought they knew everything! Wrong!! Gee thanks gold star rehab for killing my father and not caring about anyone but you self! Dirty and receptionist flirt with the male employees there too

I was supposed to stay 20 days after knee replacement. PacConvalescent Therapists discharged me against orders of both my Drs.
Therapists decided I should leave after 12 days. Both Drs told them I should stay full 20 days. Therapists lied, told me Drs ordered my discharge.
Because I could scarecely walk, hit by a car a few hours after discharge. Pelvis and both legs smashed. Disabled for life.
Soon as pacconvalescent found out about accident falsified records to say I walked out AMA. Discharge instructions disprove that lie.
Falsified records to state my Drs gave telephone discharge orders. Letters from both Drs state they never ordered discharge. Pacconvalescent claimed my son removed me from facility against medical advice. He was 300 miles away.
Medicare and pacificare have paid $700,000 so far. I am 69. My family is very longlived. Some of my Grandmother's cousins lived past 100. Except for most of the bones below the rib cage smashed, health is excellent. Blood Pressure from 100 to 115.
I have 20 to 25 more years to live. Medicare and pacificare will pay millions in continuing care. For instances, because of the head injuries the neurologist wants MIRs,$3,100 a pop every 6 months.
RN Pete Calara told me I could get oxytocin and vicodine without prescriptions. Not true. He wore an RN badge. I doubt a real RN would state after I asked him 3 times that I could get highly restricted schedule 2, 3 and 4 drugs without prescriptions from a discharge instruction medication list.
Records falsification obvious. Therapists, social workers, nurse's notes had conflicting falsehoods. They didn't bother to coordinate lies on the medical records.
My Dr has not sent a patient there since I was disabled.
If you value your patient's lives, don't send them to pacificconvalescent.
