Run. Run away from Vista of Boardman as far and as fast as you can. The "care," and I use that term loosely, was abhorrent.

I have been in a few nursing homes. Oasis has gotten much better since 2013! I have never seen any mistreatment. I am there 3or 5 times a week. From what I know personally, I know they are a pretty good nursing home. My mother has been there for 3 years. I have a pretty good relationship with the staff. God Bless them for their compassion and hard work. This is not an easy occupation. Sandy Tomlin!

The worst nursing home I have ever experienced. My mother was treated horribly at this facility. I ended up called in the state to investigate abuse and neglect. If you love someone DO NOT Put them here.

JCAH or whomever regulates nursing home/LTC/rehabilatation facilities in Ohio should tear this place apart with poor record keeping, improper billing to both insurance companies, medicare and private individuals, terrible nutritional needs assessments, and overall poor care. I would not let an animal stay at this place, let alone someone whom you love and care. LPN's, not RN's, are hired "on the cheap" and, with the exception of a rare few, they could care less about the physical, emotional, and spiritual needs of any of their current residents. If this rating system was able to provide negative stars, I would give the maximum negative stars. At over $200 per day, there are much less costly and many more worthy places to entrust the care of a loved one.

This is a wonderful facility. It's located close to everything so visiting was easy. The staff was kind and very helpful.

The facility is dirty. The staff is uncaring. It becomes obvious in a very short time that their only interest is money.
Look at the real reviews, not what is here.
Their own staff will tell you that the facility has greatly gone downhill over the past couple of years.

My grandfather stayed at Pembrooke Place for skilled nursing/ therapy after having knee replacement surgery. He always had to wait long periods of time before his call bell was answered. He had some pain following surgery and Pembrooke ran out of his pain pills! He had to wait over 24 hours for the pharmacy to deliver it! He caught C. diff while there which is a contagious bacteria that causes you to have horrible diarrhea and need antibiotics. His stay at Pembrooke ended him up back in the hospital because he was so dehydrated from all the diarrhea it took a week to diagnosis him. Do not go here for therapy. The Director of Nursing- Laura Rice is very rude (even the staff said they didn't like the Director of Nursing).

My mom was located on the first floor vent unit, the respiratory therapists were great with her, but I also LOVED the nurses and the fact is was 24 hr RN coverage on that floor! They were on top of everything and always very helpful! I also referred a friend whose dad needed a trach to go there due to the 24hr respiratory and 24hr RN! And the director of nursing was always so helpful and answered all questions immediately! Thank you valley renaissance and those nurses!!!!

ugh is all i have to say! the administrator actually dared our family to call state a few weeks ago! So i did! instead of addressing our concerns and putting our fears at ease, he just got right back in our face like he is untouchable! Most of the nurses are wonderful i will say, but they are terribly overworked, and the aides are overworked and understaffed, which might explain for their rudeness!!! They never have clean sheets our towels! I once asked where my mother's shirt was and the aide said 'lets check in laundry' and i saw behind the doors, laundry was actually to the ceiling and the smell was atrocious! i've heard them tour people and make all these wonderful promises….all lies! it used to be a great place, but in recent weeks its been horrible!

This is a horrendous place. Yes, they are remodeling, however, this is going on while residents are in their regular rooms and there is a lot of dirt and noise. Every time I went there to visit my husband upon passing a certain area the smell of feces was overwhelming. I witnessed residents being call "sundowners" to their faces by staff when they became confused. During 3 months my husband was there a total of 5 days, 5 days too many. One of those days I was called early in the morning to come to care for him because the nurse was overwhelmed and they were understaffed. The furniture there is old and not really safe. The care was minimal. My husband required a certain bed for pressure sores, one they said they could provide. Instead they provided one that was not exactly what had been ordered and additionally tried calling his attorney to received extra payment for this even though they had agreed to provide it and were required by the state to do so. In the end my husband was in a hospital and hospice became involved. My brother-in-law notices very little about this sort of conditions and yet when we were told my husband would need to be returned to Briarfield to die even he said it was not a place to die. If a place isn't good enough to die, and it isn't, it certainly isn't good enough to live.

This nursing facility is awful. They have carpet throughout the building from the lobby to residents room. The day time aides do half their job with residents yelling for assistance. You see them on cell phones and more so hiding then assisting the residents. Some of the nurses are ignorant and disrespectful to staff and as well the residents. I was tdld that the don plays shifts against one another to where they are always at war. They are also always understaff. If you have over 35 residents why is there only 2 aides? Or 1 stna to 20 residents. The residents suffer through it all. When our family notice the bullshit that was going down we took our family member immediately out of there. We also had reported them to state. They need to either shut them down or sell the business. Stacey M.

Ths is is a lovely facility, with many open spaces suitable for entertaining patients, yet few opportunities are available. Although there is a theater and a stage, I have seen no events scheduled there in 5 months. The snack bar is open a mere few hours weekly. Opportunities for social patient gatherings (board/card games, jigsaw puzzles, 40s
music recorded on something as simple as an ipod and broadcast thru speakers) are not available, with the exception of weekly bingo and a few patients who get together to play cards. TVs are blaring from all areas. The social events that are planned are not meant to engage the long term care patients, such as an old movie in the theater with popcorn or snacks. I attended a valentines daymusical event, where the pianist asked for requests, but knew none of the songs requested from the 40s and 50s and didn't sing while he played. Dont' ask for requests if you aren't skilled enough to fulfill--and this gentleman plays there monthly.
Both the rehab and long-term care sections are short staffed and cannot attend to the patient needs quickly. This definitely needs to be rectified.
