There are a couple of nurse assistants that need to be removed from the facility. I witnessed a couple of things that broke my heart into pieces after hearing what I heard from a CNA…her name is Brittney…she was cussing up a storm and was upset that a senior was asking for her help to change clothes because she had "already changed her 20 minutes ago." I was in my uncle's room and could hear this girl from a distance…I poked out my head and saw exactly who it was who was complaining. To make matters worse, she was complaining in front of other patients, her coworkers and a lady who was a family member visiting a patient and I believe was also needing her assistance with something. Neither one of the nurses helped the patient….I believe an RN ended up asking another CNA from another hall who was working on her own group of patients…not exactly sure…but that Brittney and her partner in crime Esmerelda, are a very evil duo who need to be FAR AWAY from these seniors who are living a nightmare when those two are around. I cannot believe those two were hired in that facility….if anyone who has a loved one who resides in The RIO located on New Braunfels road in San Antonio, TX…I STRONGLY recommend you pay more visits to your loved one to ensure they are being properly cared for during the shifts of these two CNA's

My mother was a resident at Trisun Care Center for approximately 6 months. The staff was unresponsive on issue and the care seemed inadequate. Almost every time I went down the hallways it smelled of urine. One time I found an employee using the nebulizer hospice had provided but they thought it was ok since she used her own tubing. There were a few mishaps while there. A few times I found her naked in bed next to the AC unit. They would say she would take off her pants but there would be no pants laying around her. The last incident was when she had a huge hematoma on her forward with serious bruising all over her face and down her neck. They didn't file an incident report with the state until they knew I filed a complaint. Unfortunately our laws allow them to file it days after it happens as long as it is done before the investigator shows up. Don't be fooled by appearances. I would not recommend this place.

My husband did not receive good care here. His first night, he fell trying to get to the bathroom. Eventually an aide came along, said "I can't help you get up," and left; he crawled back to bed.
When they didn't have the food he ordered for dinner one night, they brought one small cup of pudding; that's all. I had to go to HEB to get him some food.
They don't check to see if the bathrooms in the rooms have toilet paper, you have to ask and then wait. He was also out of towels for three days; they were "waiting for a delivery."
After having been in the hospital for three weeks, he never saw a doctor while at Powerback, although they supposedly have one on staff.
When my sister-in-law went to get his belongings, she was given one bag that was labelled for another patient in another room, and wasn't given all of his things.
I would not stay there, or recommend it to anyone else.

Powerback is a very nice place but it is very short staffed. The food is nothing to brag about. One nurse for each floor at night. Long waits to and from the bathroom. I would never recommend this place.

I am lucky enough to have my loved one who resides at Mystic Park to be coherent and sane. I honestly fear how the residents are treated there if they're not in there right mind. My loved one has been robbed multiple times. It started small with her lipsticks and nail polishes then it turned into her sunglasses then it increased until they stole her necklace from her own neck when she was sleeping. We have tried filing a police report nothing has come of it. If you choose this center to in trust your friend or family I would highly suggest you do not bring their valuable personal items and visit often.

In order for me to write I had to click on a star they deserve NO STARS. I would NOT recommend this facility to no family. They let your love ones die a slow painful death. They neglect them when no ones looking. You report to state they show up. Facility is good and ready for presentation because everything they do wrong they sweep it under the rug when their not looking. So state doesn't not see the truth. They are good and hiding what they really do. THEY RETALIATE AGAINST YOUR ONE ONES. Pay attention here. They are good for nothing. They don't care about your love ones. They let them die. They are not professional nurses and wound care nurses. They enjoy seeing your love ones suffer!

I bought the sales pitch and fell for the curb appeal, but made the greatest error in entrusting Coronado at Stone Oak to assist in caring for my father. Care is exactly the opposite of what was received. Upon being admitted, my requests began for a wheelchair so that I could take my father down the hall to the cafeteria or outside to get fresh air. After three days of repeatedly asking for that wheelchair, I was confronted by a nursing assistant who complained of having her break interrupted in order to assist me. She was ugly, rude, and unprofessional. A bath was almost as hard to come-by. My requests that someone assist my father in bathing began on a Wednesday. He was not bathed until Friday. Despite his inability to walk, meals were placed at the front of the room beyond his reach. He ate when family members were there to ensure that the tray table was placed within his reach. I could go on and on. I think the greatest insult, though, was when my father passed. We got the call early in the morning and rushed over. We were saying our good-byes when their billing agent, Alma, came into the room - completely unannounced and without even knocking- with her clipboard to discuss my payment. My calls to the executive director have not been returned.

The facility is beautiful and clean although the semi-private rooms are tiny. If you did not need any skilled care you might be okay. It broke my heart to see these elderly people roll themselves out into the hallway for help because no one would answer their call light. If you are confined to your room expect a 2 hour delay with your tray unless you have a family member go directly to the kitchen to get it. I had to transfer my parent out because of the lack of care. The aides were nowhere to be found when they were needed. I do not recommend.

I am extremely pleased with the treatment my husband received while staying in the rehab portion of The Heights on Huebner. He was taken by ambulance on 1/5/15 at about 4:00 pm to his room . When I arrived, I was told he was in a different room. It was confusing when my husband was not in the designated room. But in about 1 minute, the staff did move him in to the correct room. The rooms were very pleasant and roomy enough to not feel crowded with two occupants. His roommate was very pleasant and courteous.
Throughout his entire stay the nursing staff, physical and occupational therapy staff, and maintenance/custodian staff were very pleasant. They were all compassionate, sensitive to my husband's needs, courteous, and it seemed, always in a good mood. My husband was kept on a consistent schedule. He had daily therapy which lasted 2 hours in the morning and 1 hour in the afternoon. He was encouraged to move about the facility whenever he wanted to get out of his room. First in a wheelchair because he was unable to stand on his own. But within 2 weeks, he had progressed to walking with a walker, then a cane. By the 3rd week, he and I were taking walks up and down the hallways and outside the building, as weather permitted. His improvement was readily noticeable. There were had daily activities which he could participate in if he so desired.
Of course, the world being as it is, there were some negatives. The medication schedule was confusing and sometimes incorrect in the beginning. The nursing staff and/or doctors started giving medication as prescribed by the hospital, which was not what my husband had previously been taking for over 6 years. They continued with this schedule until we left . I was required to bring in medication that they could not provide at the necessary dosage. When we left the rehab, all medication was returned after a very thorough review of what was still available and what had been given during his stay and an indepth discussion of what was discontinued and why.
Another negative was not having ready access to the doctor on staff. A new medical director was hired but had not started her rotation at this facility. When he left the hospital, I was not given any information on his condition nor any information of the medication he was taking. I had informed the hospital of all his medicine types and dosage, but what he was getting in the rehab was much more and of a different type. Very confusing. I was told by the hospital that all the information I desired was given to the rehab center and that they would explain everything to me. It took 3 days before a doctor made any effort to visit with me and explain my husbands condition and medication schedule. She was very thorough and did help relieve some of my fears, but it was very frightening not knowing his current condition and/or current medication dosages.
One other problem issue concerned updates to my husband's condition, what processes he would complete and ongoing activity . They admin staff had two meetings with my husband without my knowledge or attendance. I had explained that I needed to be in all meetings because my husband had a traumatic brain injury 3 years ago and had no short term memory or ability to readily understand information given to him concerning his health. Even determining when he was to be released was an ongoing question in my mind. It wasn't until 2 days before he actually left that a definite date was given to me by the Social Worker. This left me unsure on what was my husbands true status on his gaining mobility and function. I had to be the one to keep asking rather than the a staff member coming to me to let me know exactly how he was measuring up to his abilities and functions. With so many patients and residents, I can understand the slowness in giving me this information, but it was disappointing that they did not have a set program for this informational exchange.
Even dealing the these negatives, overall I highly recommend this facility. The nursing staff and therapy staff are above average in all their work ethic. They are friendly, courteous and always ready to help.

AP was admitted for a head injury and a broken arm. 53 days later she was discharged with the arm not repaired because the facility failed to get a timely medical clearance for the surgery. They would have had to refer her out and pay for the consult, which is their legal obligation under Medicare part A; instead they waited for their contract MD to be available. The arm will have to be rebroken and set, another month of pain and rehab at huge extra cost.
Medications 3-4 hours late.
On my way out, I glanced into another room and found a patient on the floor; no fall precaution notice on the door; I called a nurse.
The administrator stated that she was satisfied with the level of care they had provided my friend, and that they had met their obligation…
Her bill for out of pocket was over $3000.

My father entered the facility for rehabilitation after suffering a stroke. When entering the facility he was walking using the parallel bars, however in the time he was in this facility he just deteriorated to not walking or using his wheel chair at all. I'm not sure they ever did physical therapy as contracted because EVERY time I visited he was in bed, never out of bed in his wheel chair or out and about. My perception of this facility is they sit back and collect insurance, but do the bare minimum for the residents and not what they are hired to do.

This nursing home is in bad shape my friend has been there for 3 years. The first time we complained about a leak they moved the patient to another hall. Another time the patient fell and the patient and roommate kept pushing the button and hollering for help while she laid there bleeding again patient was moved to another hall. The room the patient is in now has leaks in the roof they just moved furniture out of the way and just left it when I went to visit patient the room looked like they haven't cleaned in a few days the towel dispenser was messed up I asked 3 times to get it fixed and the lady said it needed batteries. My friend said there was a leak in the room and the bathroom don't they even care there is mold growing which can cause respiratory problems. I think the roof has a problem. Because why would they have leaks every time it rains. I had to ask the cleaning lady to clean up some spills before they slip and fall at least 3 times and after she cleaned it was sticky. I want to make a complaint to the right people so it can be taken care of not just move the patient around the head lady of the nursing home acts like your just being picky well what are they there for I thought to care for the patient and not themselves speaking of the things that disappear and the residents have chains on everything so it doesn't walk off and the nurses say we looked for it but can't find it when their name is on there stuff and it can't be found anywhere well where could it of gone

The following is a copy and paste of my review of Trisun health care in Windcrest, TX. I will be posting my review on every website available to me: If I could rate zero stars I would. All of the employees, from janitorial to supervisory, have a lack of respect, compassion and empathy for the residents. I am unsure what the education/licensing requirements are for employment with Trisun, but I suspect they use Craigslist to advertise any job openings.I would hate to be any of these employees when independence and self care are no longer an option.

THIS YEAR THIS FACILITY FORCED MY MOTHER WHO HAD DEMENTIA TO WEAR A FILTHY CATHETER UNTIL SHE SET UP SEPTIS AND DIED. WHEN I CONTACTED DEPT OF AGING AND DISABILITY, THEY TOLD ME THAT THEY WERE SORRY FOR MY LOSS BUT THAT SENIORS PASS AWAY ALL OF THE TIME, AND DIDN'T DO ANYTHING TO THEM FOR THE NEGLIGENCE THAT THEY INFLICTED ON MY MOTHER.
THE LAST TIME THAT MY MOTHER WAS SENT OUT TO THE HOSPITAL, THEY FAILED TO SEND HER DNR WITH HER, AND WHILE AT THE HOSPITAL, SHE CODED ON THEM , AND THE HOSPITAL NOT KNOWING THAT THERE WAS A DNR, THEY REVIVED HER AGAINST HER WISHES.
I SAY THAT TO SAY THIS, UNLESS YOU DO NOT LOVE NOR TREASURE YOUR LOVE ONE, PLEASE DO NOT SEND THEM TO THIS PLACE. CHANCE ARE, THEY WILL KILL THEM ALSO!
AND WHEN YOU TRY TO CONTACT DADS, NOTHING WILL BE DONE ABOUT THE CRIME! THEY ACTUALLY CONDONE WHEN NURSING HOMES KILL SENIORS, BECAUSE THAT IS ONE LESS SENIOR THAT THE STATE HAS TO TAKE CARE OF!

This is by far one the of the worse Rehab centers I have seen!!! I would not recommend this place to anyone. They provide poor quality care and has a disregard to patient care and safety. The staff will allow more then 30 minute to answer a paitents call. If the patient has to go to the bathroom they should know that they will wait a very long time for assistance. It took them more than 1.5 hours to respond to a call from the patient before entering the patients room. I find that this facilty provides unacceptable care and if i was you I would never consider this location.

My sister-in-law just came to this facility 1 month ago. I am an ICU RN and have fair but high standards. The care she has received has been very good. She likes the food and the staff caring for her. There are too few nurses but the aids are very attentive. The facility has been very clean and mostly odor free everytime I have visited her.

Nursing homes have a bad rap and I think this facility is the example. The nice entrance does not reflect the actual care that the patients receive. Nurses are basically useless as they depend on CNA's and respiratory staff to go see what the patient needs. There are call lights on all over the place. They pass right by them, never bothering to see why the light is on. There are too many patients for the number of employees. These are not the regular patients as the other floors have. They require different, special care. I am just talking about this 400 hall that is supposed to be a special hall for ventilator patients. It's filthy. The 'housekeeping' mops all the patient floors with the same dirty water..no disinfectant either. It's just disgusting. It's obvious the owner doesn't know anything about the medical fiield, just the bottom dollar.

As previously stated by another review, the outside appearance is good and initial presentation sounds promising, but the facility is understaffed, throughout. You know the facility is understaffed when the RNs/LVNs are working in the dining room, assisting in serving residents during the meals. The CNA's are so overworked; they do not have the time to assist residents adequately, their immediate supervisors should be on the floor, hands-on, assisting the CNAs and acquainting themselves with the residents. Nursing staff is also lacking and are unable to meet each resident's needs, they do not make adequate rounds to check on the bed-bound residents. The nurses normally stand in front of their computer, possibly too much computer-work is required..? Numerous times, I've had to point out my Mother's need for serious medical attention; otherwise it would have been undetected. Many bed-bound residents are seldom moved from same position; they are left to sleep straight through, from one mealtime to the next, without being given a drink of water or their diaper changed. Many residents complain about the food and will not eat. I have noticed full plates are tossed out because the residents are unable to eat due to most wearing dentures or not having teeth; this is wasteful. The facility claims to provide in-between meal snacks, I have never seen any offered to my Mother or her roommate; these residents have to wait until the next meal is served or until the following day for breakfast, which makes it very difficult for the high amount of diabetic residents. Also, the activities provided are not varied, there's too much bingo or horse racing; it is possible that these are the only mind-stimulating activities provided. The activity director must become more creative or is in need of training so residents do not fall asleep. Religious services are not provided to accommodate various denominations. During my visits, I have not seen any church choir groups visit, something the elderly enjoy (suggestion); they love to hear gospel hymns and the only music I've heard is Elvis or Country. This facility is fairly new and will quickly decline should the same procedures remain in place. I have noticed that the staff will go overboard when the State comes in for their annual inspection. Their attitude is so obvious, I am sure the State can see beyond the deceit. We are presently looking for another facility where residents are treated as aging elderly with special medical concerns and not ignored or just routinely taken care of, as part of the business or way to make a living..

This place is smoke and mirrors. Its beautiful interior design will bring a false sense that your loved one will be cared for. Unless your loved one is either in bed all day or the perfect patient, this is NOT the place for them. The Director of Nursing has no idea how to handle a patient will sun downers or with mild dementia. My loved one was labeled as a "troubled patient" yet she had a UTI which can cause issues with a patient’s mental status. Also for WEEKS I'd been asking for the Dr or Psychologist to evaluate her for mental issues b/c she seemed to be getting WORSE not better. I would get calls 2 or 3 times a day about something that would happen and was told I needed to do something. I am not a medical professional. I asked for help from staff. No one did anything until she lashed out and then I had to have her taken to the hospital to get true treatment. Once she was taken to the hospital her finger was swollen and bruised yet I had received no phone call about her injury. I have since found a new facility that can properly take care of her. Please do not take your loved ones here. Do not be fooled by this place.
