The place is an absolute disgrace. It looks nice and that's about it. My father was forced to leave with a UTI and bedsore. Social workers are a disgrace. Exec Dir. pretends to be interested and concerned and she isn't. HIPAA violations discussing my father's finances and treatment. Don't send anyone you care about here

This place is a disgrace. My family member's clothes were stolen, the social services folks were rude, condescending and not knowledgeable of my family member's medicare rights. They had absolutely no idea of what they were talking about. In addition, the social service person, along with other Powerback administrators loudly admonished me regarding my family member's options in a common area. They did not ask me to their office nor my family member's room- HUGE HIPAA VIOLATION. The therapist had very little interest in the progress of my family memebr. I chose the facility due to its location which provided me the opportunity to see my family regularly. It was not worth it at all. Do NOT send your family member to this facility

The nurse practioner told my husband to sign to have her none rescuscitated paperwork done because that is what is best. How is saying oh well shes a lost cause end her life and move on?
They also didnt give her your cpap machine that keeps her going either and stated that the day before she refused it. Let it be on her if she refuses not an oh well you refused one day so we dont have to give it to you for none of the days
i plan on writing the state with a through letter to inform them
ps they dont document anything but if your loved one gives an once of problems they will most definitely document that and then black ball your loved one and give very limited services so be ware

This place is just okay. The CNA's only do what they have to. My family member was only taken care of when I asked them do something. My family member had asked many times before to get his nails cut but that didn't happen until I showed up and said something.

Do not take your loved ones to St. Monica's especailly to the St. Edwards section.The staff there have no compassion for the patients under their care. My stepfather was there for 3 months and he sufferd greatly. A stroke victim, on the 3rd day there I found the begginings of a bedsore on his heel. That morning I go to perform his a.m. cared around 7:30 ; I find him in a twisted position with his affected leg under him, later that evening I found the sore. The staff is rude and I was not allowed in the room, his caregiver to be, when peforming ADL to the point of them locking the door. I can go on, however I am hoping others get the picture. Please, do not send your loved ones to St. Monica's unless they can care for themselves.

Our mother just finished up a stay at this facility in March of 2015. She was there for just under six months. We were very happy with staff and the care she received. The only downside was the lack of parking, which was worse on weekends as opposed to weeknights when we would come visit.
Highly recommend.

My mom was in the hospital after suffering a massive stroke. She needed rehab (physical and occupational therapy) and speech therapy. She received therapy at St. Ignatius and they were great with her. Also my family was very stressed with going through all of this and the staff was sympathetic and made us feel welcome to come and visit my mom all the time and even when she was getting therapy. It is a very old building that has been around a long time but it is filled with kind and caring people. thanks be to God.

Today I go there for a food delivery the receptionist nd his female friend make me go back nd forth to the establishment in this cold weather . Very time he let his friend answer from the reception desk to confirm the order nd when I get there she say this is not her order then finally after laughing brought he call the customer from upstairs to pickup her food .

The Angela Jane Pavilion may have a nice looking rehab center & clean looking facility. But as the saying goes, Don’t judge a book by it’s cover. This is a typical nursing home.
The in-house living care in this place is a JOKE! The aids, nurses (read: LPNs, the RNs have very little to no patient interactions!) and very few RNs are actually staffed, doctors (read: doctors who treat based on a patient’s chart, rather than walking room to room and physically looking at the patient) and the rest of the staff are very rude. Long patient waiting times when call bells are rung, do to short staffing. The 11pm-7am staff is extremely rude when a patient asks for help.
How do I know this, I had to spend every night with my mother while she was there for 4 weeks.
The staff will be all smiles and friendly when family is present. Once family leaves the staff changes their tune.

This place needs to be shut down, I use to wonder why the turnover rate was so bad at Tucker House and now I am beginning to see with my own eyes. I walked into one of the bathrooms by mistake where the patients receive showers I thought it was a bathroom for anyone to use. I see a patient just sitting there fully clothed in a shower chair, I thought it was rather odd but I didn’t make much fuss about it. Then I overhead a nurse talking to another staff member about how one nurse places patients in the bathroom for showers but never actually gives them, she just pretends to. I was horrified. I thought if that nurse is doing that to a patient then what’s going on with my father? I hinted to his social worker but she assured me that he was well taken care of. Another thing I didn’t like I seen a EMT and a nurse come out of one of the rooms giggling and laughing like they just got finished fooling around.; That was all I needed to see for one day. The next day I came out there it was during lunch time and the nurses were all sitting around talking very loud having personal conversations about their outside life. You would think that once they seen me walk in they would stop. My father is still in there but I’m trying my hardest to get him moved to another facility or possibly home. I know no place is perfect but Tucker gives me a creepy feeling. And the staff just doesn’t care,

My sister had our mom admitted to Chapel Manor back in the early 1980's. Communication with mom showed she considered my sister a "witch" for placing her into a dump. I've had no communication with my sister since 1983. I've written to Chapel Manors director who has ignored my requests. There is no record of my mom's death anywhere---evidently so my sibling could collect any and all inheritance. Sister's deceased hubby once stated to mom, "Who gets your SS benefits after you and Minnie are gone?" Mom said, "It goes away! What kind of financial planner are you?"

This place needs to be shut down! My cousin has been there for a year. the place isdirty, the staff is abusive. they stole my cousin's clothes, phone, sunglasses, etc. One nurse tried to slap him. the elevator is unsafe, his wheelchair was stuck in a gap between the floor and the elevator. there was no emergency button to call for help. the food is horrible. they gave him a spoonful of leftover veggies and two pieces of dry bread for dinner. My aunt has to bring him food. I'm reporting them to the state.

I've never been to such an unorganized place in my life, the management doesn't know what they're really doing and the nurses on the floor seem to be confused also. Every time I asked for something I got an attitude and it took a long time. People run around the building cursing and complaining all day. When I did get a chance to speak to the director of nursing he was short with me and he seemed a little off. I pulled my motet out of there as soon as I could (her social worker was very nice). I would not refer anyone to this nursing home. It's unorganized, ghetto and ran down. I was looking around wondering why all the employees looked so misrable but if I had to deal with that unorganized management I would look miserable also.

REPULSIVE. Extremely rude staff. One lady actually asked me what I was doing there when I was going to my grandmother's room and told me to "go sit in the day room" as if I was her child and she was sending me to my room….I don't recommend this place at all. I feel uncomfortable knowing my grandmother has to stay there with people like that.
