My mother was recently in Transitional Care for 5 weeks and we were very impressed with her care. She met all of her rehab goals of gaining wt., strength, mobility, and gaining self assurance. The team included my dad and the home situation, but did not make them feel pressured. Skin issues were totally resolved in a timely way. I am a RN of 40 yrs and was impressed.
