The building is out of date. Too small of rooms. Very filthy. High turnover of staff, including supervisors. Food is awful, especially on the evening shift. Portions are smaller than state allows. Staff is undertrained, especially for dementia, and rehab patients. 900 wing on the evening shift doesn't allow patients to do their nursing care /rehab programs due to lack of communication of rehab and nursing.

Too many hospice patients admitted to rehab to get extra money for "rehab" patient, when patient has less than 6 months to live.
