Lebanon Health & Rehab Center gets a no Star rating from me. My advice to anyone looking at using this facility is to look elsewhere. My mother was left unattented in a wheelchair and fell out. She lost approximately 40 pounds and became malnourished. She developed bedsores which continued to worsen to Stage 3 before we were notified that she needed Hospital Care. She is now dying from malnutrition and bedsore wounds and is in tremendous pain. Please do not put your loved ones in this facility.
