On dementia unit there is overcrowding with 3 patients to a room. No mental stimulation. Patients able to express themselves state being bored, nothing to do but sleep. Poor sanitary practices, i.e., fecal material found on floors, urine on pillow cases, incontinent patients sitting on dirty, smelly, wheelchair seats, filthy wheelchairs. Nurses and aides do not familiarize themselves with individual patient needs and put patients at risk for fractures, etc. Patients with dried food around mouth and food caked under nails, not cleaned after eating. The list goes on…………
