My loved one came into this facility after suffering a diabetic incident-he was supposed to be receiving 'rehab' for confusion. Instead they did ignored the fact that he was 80% deaf and when they spoke to him, he could not understand and would strike out-they called him 'agressive'. They ignored the physician order to elevate his feet off the bed because that causes heel ulcers. He ended up in another hospital for surgery to both heels. They ignored the fact he was celiac-gave him gluten diet AND sugar-to a diabetic! I could go on but this is probably enough. The reception folks and Bruce the PT were excellent but some of the staff is quite inadequate for any patient with medical needs..

This resident was a 61 year old Caucasian male admitted to Brookwood Gardens on 08/28/12. He was admitted for short term placement and the discharge plan was for him to go home after completion of rehab. He displayed episodes of socially inappropriate behavior, i.e. screaming, yelling, agitated and restless, and needing constant redirection and supervision. His responsible party (his ex-wife) was contacted by a social worker to discuss behaviors and discharge plans, as she reported not being able to care for him at that time. She mentioned being his ex-wife and the fact that she was in a new relationship. She was upset about the behaviors and stated to the social worker that the resident was not like that a couple of months ago. She could not understand his behaviors and changes in cognitive level. He was seen by a psychiatrist a couple of times to evaluate these behaviors and monitor the behavior’s frequency. This resident assaulted one of our CNAs and therefore needed to be transferred to a more acute setting. We provided great care and met the needs of the resident until this event.
