Ask how many falls, near falls, found on floor, lowered to floor events. Don't just ask for "falls" as in medical-wise words that means witnessed fall to the floor only. What is the nurse to resident ratio on each unit, as well as aide to resident ratio. Have you had any state inspections, including visits due to complaints. Staff is burned out, high turn over rate, no mgmt on site except during bankers hours.
