Really, hiding behind a name change? This place used to be called Benchmark and it is far from a 4/5 rating given by this website; that is ridiculous. Ceilings collapsing, run down. Not one working bath tub in the whole facility, they are all non functioning. Shower rooms have tubs and whirlpools but not one works. Staffing is terrible, the staff turnover is some of the worst I have ever seen in a skilled nursing facility. Perhaps the nurses wouldn't be so stressed out from work if they spent less time standing outside the back door smoking with the administration? If you can't find any staff just look out that back door and that"s where they will be.

My Husband was there only four days. He received the best care ever. I will highly recommend this vacility.
