I give stars to you . The inspection report from 2010 to 2012 is on target I have witness it all at Carolyn Boone Lewis Nursing Home.. But nothing has been done to make show these same issues will not exit .. can't wait to see to 2013 report…. 3-strike your out and this nursing home is truly one that should be Shut Down……

This is a poorly operated facility. They do not take pride in their jobs over here and do not provide adequate care for their residents. Once a resident speaks out then the staff decides to take the residents dignity and pride by bad mouthing them and mistreating them. The staff comes to work for an easy check and that's that. If your care about your loved one don't send them here because they are guaranteed to die here. They are very negligent and I have experienced times where a resident has fallen out the bed and they asked me to help them. I refused because of civilian suits that can be brought against me for helping and something goes wrong. They are THE WORSE PLACE TO BE. I AM RATING THEM WITH NO STARS DESERVED!!!!

As of most of the nurses are very good to the patients while others are very sassy and Mr /Ms know it all….they should allow new employees the chance to want to learn the (New people are much better) while the older ones are stuck in there ways but overall [good work] with care of residents.

I was not happy with the care of my mother. Her room wasn't clean, the dirt was built up around the walls in her room. The front office staff wasn't responsive to the needs of my mother and other patient as well. The majority of patients complained about the food. They received chicken cooked differnt ways 5 time out of 7 days a week. Patient that were not able to ambulate well set strapped in their chair unattended. On patient was blind, plate was set in front of her and they didn't ensure she was able to feed herself because she didn't fully understand how to manuver now that she was blind. There was 1 or 2 CNA staff that worked hard. Unfortuatnately they couldn't complete the 24 hour shift. My mother doctor informed me that he wasn't aware of her sleeping continuously prior to her discharge. I would've of thought that something as serious as that would've been documented in her files. They are not qualified enough to care for my mother again!! Our Family will search for another facility if ever she need long term care again! I truly rate this facility as a 3.

The facility is bright and clean. They take excellent care of my grandmother who has lived there for about 8 years. She says, "This is the nicest place I have ever lived!" If she is happy, I am happy. I am grateful for the staff and the attention they give her.
