This used to be a quality facility, but has been declining for several months. Constant staff turnover translates to no continuity of care, seem to be consistently short staffed, poor staff morale, changes are made to resident recreation/leisure activities without resident input or explanation, facility maintenance deferred, communication between CNA's, LVN, and RN(s) poor. Check out the Medicare website rating…..not good!
