    This tiny place in NE Minnesota is tidy. But it lacks caring and compassionate managers and staff. It's a real pity. For nearly two weeks after surgery the stay was unbearable. We very much hope people will only use this place if a more reputable one is not available. Otherwise insist on being elsewhere in Chisholm, Cook, or in Virginia at St Michaels. Those are much better choices, despite maybe not having a single room.

    Understaffed. Underpaid. Perhaps, however, that isn't the biggest issue going on here as our loved one suffered terrible pain here. And despite the books saying she was given her meds, the pain was too great for that to be the case. Medicare should pop in and investigate this facility unannounced during morning med rounds and evenings after management has gone home.

    We question the level of professionalism at Cornerstone. On the surface everything appears to be in order, however, get there and you quickly find out it is not and resident care suffers. There is high staff turnover. Nurses and staff regularly have to work double (16) hour shifts due to no shows leaving them tired, cranky and forgetful. Management is highly emotional and out constantly. Response times are quite long due to understaffing and sometimes no followup is provided. Stealing is also an issue here as are bedsores. Room temps vary and can be regularly below 71F on weekends when nobody in Maintenance is available. For the money, there are better places.

    Think twice! Not suggested for short rehab or long-term. Althought it is a newer build, management is poor, unresponsive, and delay notification of problems or necessary communications. Doctor appointments are missed. Many caregivers are related to each other, to management and residents. Staff including PTs openly talk about going out and getting drunk. Issues with stealing watches and wallets. Turnover of nonrelated staff is very high. Morale is low. All rooms are singles, all bathrooms are not ADA compliant. Cannot regulate temperature in room. Very dry air. Meal options are either eggs or a starchy alternative. Salty. No fresh vegetables here! Meats are so dried out. Bedding is 100% POLYESTER atop a plastic mattress promoting sweating, various red rashes and sores. If you have a serious rehab goal, this place should not be used. No TVs in rooms.
