I would rate this very unskilled nursing facility a negative 0 if possible.

Please consider any other facility before you even think about placing your loved one here.

This was the only place available at the time our family member was discharged from the hospital and we were pressured to go ahead with the placement even though my first impression was negative.

The nurses are very rude, and uncompassionate.

If at all possible do not send your loved ones to this facility. The nurses are hateful and uncaring. Just as an example my mother refused medication and they held her down and gave her the shot anyway. I have spoken to a few nice people that work here while I was trying to get my mother out because she hated it. The admin cares about nothing going on except when they will receive payment.

    Horrible! Horrible! Horrible!

Short staff (but say they're not). Too many double shifts worked by employees. Too many PRN workers. Personally witnessed midnight staff sleeping while call lights went unanswered. Bedridden residents do NOT get bathed regularly and their hair is not washed. Supplies are not routinely provided to residents even though their monthly rate includes these items. Staff is allowed to carry and use their personal cell phones anywhere and anytime. Residents that are mobile, walking or wheelchair, are not allowed free range within facility common areas. Staff constantly directs them to their room. Residents are rarely offered a drink of water by the staff. The water pictures are filled with ice and placed in their room, but no assistance is given with drinking. The water pictures are too large and therefore too heavy for most residents to handle (dehydration and UTI's). Makes me wonder if this facility is given an operating budget and if they come in under budget somebody gets a big bonus?? The shower chair is too old and the cut out area for their bottoms is too hard and sharp for their fragile skin.

One more important thing, you can't count on the state investigators/surveyors to be of assistance when you file a complaint on behalf of your loved one because no matter how much documentation or evidence you have (even broken bones and horrific bruising). Amazing fact is the facility never seems to know how it happened. They report it as "unexplained", or the resident did it to themselves. It's a C.Y.A. operation. The state always decides based on documentation provided by the facility; employee interviews and nurses notes. Doesn't matter what you tell them you witnessed. You have to visit your loved one everyday and at different times. Never let them know what time you're coming. That's the only way you can help protect and ensure their well-being.

When the family finally decided that mom needed to be in a nursing home, we had hoped for assisted living, but her health required a little more care. This facility was the only one with an available bed that would work with Medicaid pending. This facility is substandard, old, not well taken care of, not clean, the food that is supposed to be served hot is cold, and not very nutritious. Mom was crammed into a small room with 2 other women and shared 1 bathroom with 5 others! The bathrooms were not kept clean, much less sanitized. If mom needed help and rang the buzzer, it would be 30 minutes to 90 minutes before someone responded. Most of the staff didn't speak English! Her medical care was neglected resulting in hospitalization and she almost died. I would not recommend this facility to anyone! They need to tear this one down, rebuild and restaff totally. It's a shame people working at this facility don't have more compassion for the sick and elderly.

If I could give a Zero, I would. My mother was at this nursing home when it changed hands and became part of Unihealth. It got so bad there that I moved her to another location. Patient rights are ignored, food is deplorable, the Administrator even told the residents at one meeting that they needed to eat their food because they were wasting her money. All my requests for a change to her room were ignored and she was placed with 2 alzheimers patients, even though she didn't have alzheimers. They ignored early warning signs of an intestinal bleed until she was unresponsive, at which time Emergency Medical personel had to intubate her and she spent 6 days in intensive care.
