We recently had an experience with Amberwood Gardens and found the care and environment excellent. My father in law was transferred to Amberwood from Kaiser hospital and we were anxious about what sort of care would be provided. Our overall impression could not have been better. The facility was clean and well kept up. They could provide exercise and rehabilitation services. The most important thing was the staff. We found them to be excellent. Down to the last person we had contact with the mood was always respectful, upbeat and helpful. The treatment of my father in law was very professional and extremely caring. We did not feel like he was just another person in the home but mattered to all who came in contact with him. They had an outstanding attitude and made us feel like we had found the absolute best place for his condition. I would not hesitate to recommend this nursing home to anyone.

I am an ICU Nurse at one of the local hospitals. I am well versed with the ins and outs of Skilled Nursing Facilities. This facility is a "trusted" facility for professionals at the local hospital accross the street. There are two of us nurses that have had our parent rehabilitaite here. These online reviews do not take many things into consideration. These elderly folks and their kids are mostly not tech savy therefore are unable to give reviews like some of us can. The online reviews also do not take into consideration that this place is a revolving door of patients coming and going. They can discharge up to 30+ patients monthly only to bring in just as many. My mother has been here on more than one occassion and she will return if more care is needed. I have a trained eye to see how skilled nurses are when caring for patients and these nurses at this facility are the best you can get in a facility. The rehab she recieved in house kept her on her toes and brought her back to the brink of loosing her mobility. It is an added bonus that this place has an affluent character. It is not only clean, it is polished with care. Tour these places and compare for yourself. There is a facility near Plum Tree that I wouldn't take my dog to because of the care patients recieved there. These patients came back to hospital with horror stories of how incompetent staff was. Please, tour these facilities and ask nurses and doctors for their help in selecting a facility. They will point you in the direction Plum Tree. Thank you to the staff their for all their hard work and caring nature.

Amberwood is one of the nicest facilities i have ever seen. My mother was in a couple different facilities before i found this one and this is heaven in comparison. The staff shows genuine care for every resident and are very helpful in making my mother comfortable and happy. Many thanks for this facility, stop by and see for yourself!

The staff is very good. The problem is mixing rudd patiants with short stay non rudd . minded people. Respect is the problem here. Some paitiants treat caregiver down right nasty, very demanding . My mato you get what you give. oh and the cook staff should lay off late nite partys. give me a break. change jobs PLEASE..

I had such high expectations for my mother here and it seems as if they gave up much too fast. No follow through with the promised care from our initial meeting on what was going to happen for the well being. Staff ignore alot of the call lights as I have had to go get them myself to tell them someone is in need of help.

Disappointed that this is the type of care offered for the elderly.

No wonder they have a federal 2 star rating.

My father was recently discharged from Vista Manor Nursing Center. I would like to thank the facility staff, especially the rehab staff who worked very hard with my father. The building is very clean and my father felt very comfortable there. I highly recommend this skilled nursing facility to everyone who are looking for excellent service.

There is a problem at your facility with the telephone used to contact patients. It appears as if the system is either outdated and defective. I have made calls to my relative there and the connection is horrible. One cannot exchange conversation without interference. The end result is not being able to speak to the patient at all. It would be beneficial if this problem could be corrected. The calls I make to your facility are long distance calls so I am paying for nothing.

The staff at Willow Glen are amazing.

They treat all residents with the upmost care, respect and love.

It is also very well maintained and clean facility. The food isn't bad either!

If you are looking for a place you should definitely stop by for a tour.

They are all very friendly and helpful.

We have no complaint towards this nursing home, infect, this is one of the cleanest place around the area, my Mother in Law( Jahan )was there for almost 11 months and I tell you they were all sweet, all the nurses were really nice on station 1. My grandmother has been in this facility several years after suffering a stroke. She has had nothing, but quality care since the beginning. They have always been very accommodating to her with any request that she has had. Even though she would prefer to be at her old home, this is the next best thing.

My husband has been at A Grace since 9/22/2011. He had a triple bypass in July as well as a colosmy, he spent a month at San Jose Regional and a month at Kendrid Hospital.When he was transferred from Kendred Hopsital he was on a nose feeding tube, air mist from ventulator and could not move his arms or body. He is now off the feeding tube and on a regular diet, no mist, can move his arms and push himself around in the wheel chair and is now working on walking. The staff is awesome and very helpful and always has a kind word for him. I could not have picked a better facility for his recovery.

They are not friendly. Nurses specifically pick VITAS hospice for all clients. They never let you pick your own hospice agency because they get kick backs from VITAS. Their care is very little. Poor service, poor care and the administrator is not willing to meet with families.

I brought my mother to this facility for nursing care and rehab after she had a fall and fractured her hip. The staff - nursing assistants, nurses, therapists, social workers, and even housekeepers seem to enjoy their work. My mom received excellent care and came home excited. The staff at White Blossom focus on their residents and make a difference. I would recommend this home to anyone.

My husband (Ramon) was at San Jose Medical Hospital for 60 days with a TBI (traumatic brainin jury). On May 19, 2010 he had his trach tube removed. The hospital transfered him to a grace one day later. My husband still had his feeding tube and could not talk, walk and very limited movement. When we got him to a grace the staff & social worker were very professional & kind. My husband had intense therapy per my request and I facilitated this with the therapist and his doctor. All of our family visited him every day and gave him courage. The staff were awesome, I must say. Their care for my husband was in my opinion above and beyond. I could not have done this alone and am very grategul for the care my husband received. When I took my husband home on June 18, 2010 he was able to communicate with nodding to yes and no questions and he was off the feeding tube. He was able to help with his transfer from his bed to the wheelchair. My husband is now able to walk, dress himself, shower on his own, able to turn on and off the TV, and he is able to talk. I believe that the care at a grace was part of his healing and recovery. Thank you to all the staff at A Grace, may you keep up the great job your doing. I appreciate all that you have done for us!

My 93 year old mother was admitted to White Blossom Care Center, San Jose, Ca. Oct. 2010. She had fallen and fractured a rib, along with a pre-existing heart and kidney condition. She received the best care you could ask for. The staff is stretched thin but do one heck of a job tending to all of their patients. They exhibit patience, understanding and warm attitudes toward all including family members. Only had a run in with one CNA who was being verbally abusive towards our mother and she was promptly dismissed. Food could be a little tastier especially for those on unrestricted diets. I would recommend this care facility to anyone.

The staff were very nice. Everyone was happy and showing their smiles. The nursing care was very good.They have one person dedicated to be concierge who comes by each day to problem solve and show her care and concern.They specialize in short term rehab and have over 14 therapy team to support the recovery process. Therapy was very good. Caring and professional at all times. Everyone seemed to enjoy coming to work and caring for residents.

In April of this year, my mom (Susana To'omalatai) was submitted into A Grace Sub Acute and Skilled Care as a patient. Before she was submitted, she was recovering from a very high risk surgery that doctors were shocked she survived. As weeks gone by, she was gaining her strength and appetite back which allowed her to go to the next step. With a lot of research and great referrals from the nurses and social workers, we were able to hear about A Grace and how they offering everything we were wanting to provide for our mom. From the first day of my mom being submitted, she was a little nervous and scared because this type of care and living was completely foreign to her. She was coming in with her own notion that A Grace was just another hospital. But, it was in the gentle services of the rn's/cna's/rt's, the daily physical therapy, the weekly patient activities, etc…that activated new life in my mom that was lacking in her for over 12 years. When I look at her now, I no longer see a woman that was imprisoned for over 10 years with a sickness that is no longer evident in her life through the love and care of the whole staff of A Grace Sub Acute. She now appreciates life and is smiling like never before. If there is one word I could use to describe about the DNA of A Grace, it would be: Transformation. Thank you staff and social workers at A Grace for an outstanding work.

The staff here is very friendly and compassionate. Each person I have met shows a true love for elderly care. I don't think that quality is found often. I appreciate everything they do.

I'm feel sorry for yesterday's parking lot crash. I think an accident is an ACCIDENT and hope that they are able to recover as a community. It's really unfortunate, but felt relieved with the care my (uninjured) mother received post incident.

My 96 year old dad broke his hip and stayed at White Blossom for over a month. The nursing, PT and OT care he received was top-notch. I honestly believe that he is coming home in better shape than when he got hurt. The nursing staff appears unresponsive if compared to hospital care but they do a super job considering that the ration is 14 patients to each nuse and nurse assistant. My dad is washed thoroughly each morning, his medicine is carefully managed, he is kept really clean even though he is continent and receives PT and OT 5 days a week. The food could definitely be improved but the cook visits my dad regularly and responds really well to my dad's complaints. I would recommend this facility willingly.
