I would not recommend this facility for anyone needing assistance with activities of daily living and mobility nor for anyone needing rehabilitation after a prolonged illness. Although most of the staff seem to be caring individuals, the processes throughout the facility are poor. The phone is seldom answered when called for an update on resident status. The facility is old with peeling paint and uneven flooring. Housekeeping practices are generally unclean, dirty stained carpeting that is seldom vacuumed and only minimal bathroom or bedroom cleaning is done daily. There are bugs crawling on the high ceilings. Meals are seldom correct according to the menu completed and often cold when finally delivered. Call lights often take 20 min. or more to be answered which leads to incontinence and safety issues.There is minimal coordination of care and very little rehabilitation provided. There is no carryover of rehabilitation from the nursing staff. Therapy staff spend most of the therapy time socializing and very little time on actual skilled therapy services. Wheelchairs and other equipment are in disrepair. Orders from physicians are not carried out and physicians are not notified of changes in status. Outside doctor appointments often take a week before the staff call to make them leading to delays in scheduling. Clothing is lost when laundered. Complaints to administration go unresolved.
