We should have done more research of facilities when we were looking for a place for my husband. He wasn't there two days before we decided we were moving him somewhere else. The response time for calls for assistance was often 45-60 minutes… Full Review

DO NOT bring your loved ones here! Recently Provo Rehab and Nursing has undergone new management (new ED and Operations Manager). They're also currently lacking a DON, which has seem to make the nursing staff slack. Call light response times… Full Review

Sara Salazar helps with patient planning and things to help patients go home. She is fantastic! I have never had a better advocate helping with patients. She shows genuine concern and truly helps the patients.… Full Review

Do NOT take anyone to Provo Rehab that needs more attention than just short term rehab.(and I doubt that care is any good either.) I am not necessarily blaming the overworked, underpaid nurses and aids for the HORRIBLE care my mom received… Full Review

I have had several relatives who have spent time in this facility, and I strongly reccommend that you take your loved ones somewhere else. My Grandmother was here, and the care was so bad that we had to take her home and care for her ourselves… Full Review

No matter what your told this is not a rehab facility. We were told that they had the resources to rehabilitate our wife and mother. They do not put together a individualized care plan. The staff and care is disorganized at best. They failed… Full Review

The facility in the Tower is the best I have seen. But the nursing care is lousy. I kept begging the nurses to get my mother on antibiotics for a UTI but they drug their feet until she went into Septic Shock and is now in ICU. I thought… Full Review

DAY 1: No one there to meet us when being admitted. Family had to track down nursing staff for the room. Equipment for the family member to be effectively and safely transferred to bed was locked up and the nursing staff could not get to it… Full Review

The Doctor of this facility does not attend to his patients very well. If needs are required he does not respond. He rarley meets the patients. The other staff seems to care and respond well to the needs of the patients. The stars only… Full Review
