Some staffs are very disrespectful and rude to other staff. Caregivers, and nurses are quitting do to be disrespected, and rudeness from the head people. Nurses talk people business, and when it comes down to telling the DON or the administrator nothing done about it. They tell famil members anything just to get them in the building. The nurses does not believe in answering call lights. They will sit and wait on cnas come out of a room, and tell them they have a light on. I recommend families, and people that’s looking for a job or a place for your family this is not the place for you. They always working short handed, because they have a person that does not know how to do the schedule, and she refuse to help out on the floor. She had told patients she do not change people anymore , because she’s the boss now….. BEWARE OF THE HEIGHTS AT TOMBALL!!!!!

Our experience with Heights of Tomball are so much like a previous review.

The waiting in soiled diapers or holding yourself and waiting 1-3 hours to get cleaned up.

New meds showing up without any notice to family.

No answering the call button during meals, 3 times a day.

In our case, there was a couple emails however not from the "business staff".

I do not agree that ALL the nurses and cna's really care. In our case, it was all about, "the duties they didn't mind doing" versus the ones not wanting to do.

Beware, if your loved needs a lot of help.

Daytime Nurses and Aides do a good job. Nights and weekends suffer from constant staff turnover. The rude administrative staff will enter a room and go through patient's personal belongings whenever patients are away at meals or therapy. Nanny cams are prohibited for"privacy" reasons resulting in widespread thievery. Do not be surprised to see more shocking details on upcoming local investigative newscasts. There are far better options than Park Manor.

My family member has been there since 2008. There have been lots of changes to continue to improve and strive to meet the needs of the residents. I applaud the administrator we have now for having an open door policy for family or residents. I have learned that if you get to know your floor aides and work with them to come up with a workable solution for any concerns you have you will have a successful relationship that will benefit your loved one and you. Park Manor Tomball has truly been a blessing for my family. All family members should consider getting involved in Family Council and the residents need to get involved with Resident Council. I have learned it really helps you navigate the whole long term care system. But it doesn't hurt that the staff are so helpful!

My friend RM admitted to Park Manor Tomball from Tomball Regional Hospital, for physical therapy and wound care.

Without going into detail, we were unsure if she'd be able to return home. RM was assigned to a semi-private room, with another woman, BB. The room was spacious and clean! She became very close with her roommate BB, it was a blessing in disguise as they were each other’s greatest support.

She did therapy three hours a day for three weeks, her strength and confidence was remarkable. Everyone was so encouraging and helpful.

After she met her needs in therapy, Park Manor Tomball assisted in helping her access her Medicaid benefits. Today, RM is a full time resident. Although on a different hall, she still shares a room with BB. Their room is decorated with photos and family heirlooms, it’s so homey!

Thank you Park Manor Tomball – you brought my sweet friend strength and a lifelong best friend. I’d highly recommend Park Manor Tomball for anyone needing therapy, iv’s, wound care or long-term care!

The following reflects our experience at Heights of Tomball from 5 Nov 2014 to 16 Dec 2014, during which time my Mother was admitted for rehab following surgery for a broken hip.

The caregiving staff (nurses, CNAs, aides, therapists, etc.) at Heights of Tomball are great! They work extremely hard and most of them genuinely care about their residents/patients. They go above and beyond to take care of your loved one. HOWEVER, during my Mother’s time at the Heights, the response to her call button averaged over 20 minutes and there were several occasions where it took almost an hour for anyone to come to her assistance. We were told by one of the CNAs that they are specifically prohibited from helping patients with toileting during meal times. Considering that each meal time lasts 90-120 minutes, your loved one could lay there in soiled diapers or in toileting agony for well over an hour… three times a day.

The leadership team is all but inaccessible: they rarely return phone calls or respond to email. In fact, I was told by the Administrator that "we do not do email." While you are visiting your loved one (weekday, morning, afternoon, evening), the likelihood of being able to actually talk to the Administrator, Finance Manager, Director of Nursing, Director of Therapy, and/or the house Doctor is slim to none. They spend their time in meetings and behind closed doors. If you do manage to find one of them, they smile and nod and are very agreeable regarding whatever issues you are concerned about… but there is rarely actual delivery or follow up, and the communication flow to the care giving staff is very poor.

Heights of Tomball “manages” difficult patients (anxiety, fear, etc) with drugs. The Administrator told me we had to give them permission to put my mother on antipsychotic drugs or she would have to go elsewhere. After that conversation, we moved her out of there as fast as we could – it took us about a week. Interestingly, within 24 hours of being in her new facility, my Mother’s anxiety and fear settled down to the lowest it’s been in well over a month – and with no antipsychotic drugs.

Heights of Tomball is a new and shiny, well-designed facility. The staff will give you a tour that will knock your socks off but BEWARE OF CURB APPEAL. If you do put your loved one there, be sure to visit every day and check the Meds list as often as you can… it is amazing how new Rx appear with no discussion with the family. For example, I had a conversation with the Nurse Practioner from Behavioral Services regarding additional anxiety meds for my Mother. The NP very clearly told me that she would not add any additional meds as she wanted to observe Mom for a few more days and perhaps adjust some dosage levels. That night, just a few hours later, a new med was added (without discussion with any of us) by the house Doctor. It seems that members of the medical staff at Heights of Tomball don’t talk to each other.

Oh, and don’t expect to have a care meeting with the staff more than once every 90 days. Yep, once every 3 months they will give you 15 minutes of their precious time to discuss the care plan for your loved one. Isn’t that great?

Heights of Tomball is a Touchstone facility. I would be careful of all Touchstone facilities as it appears, at least to some degree, that the practices and culture at Heights of Tomball is a reflection of the Touchstone corporate practices and culture.

One star is still too generous. This place is absolutely horrible! They dropped my 86 yr old grandmother on the floor and then tried to cover it up. They also neglected her medical Carr. She was running a fever, was in terrible pain in her lower back and refused to even apologize for their negligence. The nurse on duty whose name is Angela, RN was very rude claiming she was tired of arguing with people. She apparently gets paid handsomely to practical malpractice. This place is insane with incompetent morons running it. Do yourself and your loved one a favor: NEVER allow them to go here for any reason, and if you do and find they have mistreated your loved one, tell everyone who will listen…including the police and a lawyer

  • Grace Care Center at Northpointe Tomball, Texas 77377

I have had my mom at grace care now for 3 years the staff knows my moms every need. They are attentive and listen to any concerns I have ever had addressing them at once. The administrator is always willing to listen and we have never had any issues we loved her there from another local nursing home due to the poor living situation she was in and couldn't be happier! Thank you grace care for all you do for mom

  • Park Manor of Tomball Tomball, Texas 77375

A few good nurses and aides there. Administration in quite inept to say the least. If a life emergency arises the response to the room by the call button is quite lengthy more often than not. Ants are in the rooms to get on drinking cups, clothes etc. the van driver that takes patients to appointments etc is always on the cell phone instead of giving 100% attention to her job. Therapists do not pay full attention to family set goals, let alone the given Drs specific orders. Food trays quite often are given with drinks spilled all over tray and no attempt to clean it up. Personal possessions disappear quite often from employees taking them. Merely my opinion but I believe they administer prescriptions generically to patients that really put them out of it unnecessarily. They do not pay daily attention to individual needs such as oxygen assist if needed. Initially upon admittance they didn't even look at records to see such things. Quite often I had to jump them cause patients oxy level was down to 70%!

Great folks!!! Unbeatable staff!!! They cared for my Mom for over 7 years, and we NEVER had issues. No odor in the room or hallways. Always attentive. The staff REALLY DOES care about their residents. Stop in for a visit and walk around a bit, you'll see the building may be older, but the care is compared to none!

  • Grace Care Center at Northpointe Tomball, Texas 77377

The administration of Grace Care is not helpful, nor caring for your love ones. I had to repeat myself over and over several times on a lot of different concerns i had . They are very rude if you or patient's family ask questions about anything, which is very silly. I had to remind them, which is sad several times that they are being paid for the services that they do there in the nurse home and that it wasn't free.I had enough, of the poor services from the staff. I had to remove my father out of that nurse home and absolutely the best decision ever!! I agree don't let the prettiness fool you!! Not worth putting your love ones in that "nurse home" and not worth paying for the services in that place with that staff to intrust your love ones well being in that place!!!!!!!!!!!!!!!

  • Grace Care Center at Northpointe Tomball, Texas 77377

Do not have your loved one live or stay at Grace Care at Northpointe ! I had a family member there and the care and the majority of nurses and staff were horrible. There are some good nurses and CNA's, but most of the staff, including nurses, Unit Managers, The Director of Nursing, the Assistant Director, and most of all, the Administrator, are rude, uncaring, confrontational, lazy and disrespectful to patients and family members. The Administrator is one of the most disrespectful, rude, mean and uncaring persons I ever met. There are not enough nurses, which contributes to the poor care and the turnover of nurses is very high. The food and the activities are bad. When residents and family members express concerns and make complaints, they are criticized, ignored and made to feel like they are in the wrong. The Administrator she cares so little about anyone - and yet she is still allowed to work here - at a care facility. The Corporate Office also is not interested at all in caring about the well being of the residents. They have received numerous and ongoing complaints and concerns from our family and others, and they do not even look into these concerns, much less make improvements. Our family member became extremely malnourished, and so our family had to demand that they start him on a high calorie - weight gain diet. For the health, safety and well being of your loved one - please do not let your family member live here.

just want to say this facility does not care about the residents. They are really on the ball when they think the state is coming for inspections or if someone calls the state. Something most of happen wrong for

This home to be denied Medicare or Medicaid payments by the government on 01/10/2013. That's to close for comfort everyone needs to beware where they are putting there family/friend if there care about them . I would NEVER put a loved one in this facility. They talk a good talk but don't perform.

  • Grace Care Center at Northpointe Tomball, Texas 77377

i would never ever recommend this facility to any body. It would be a shame to do that. My family member was admitted to this facility's rehab, to get better with her health and to be rehabilitated. Instead she got worst and ended up again in the ER and has been hospitalize. Don't be carried away of how this facility looks from outside. Inside the service is rotten. I would not even give a zero rating for this facility.

  • Park Manor of Tomball Tomball, Texas 77375

DO NOT let the pretty pictures fool you! I wouldn't wish this place on my worst enemy! The mattresses are like marshmallows, not at all supportve. The vast majority of patients are so heavily drugged that they don't so much as know where they are. The poor excuse for a physical therapist is NOT qualified to work in a professional position anywhere! The food is the worst I've ever seen in any facility of this kind and I have visited many patients throughout my life. They are quick to drug their patients so as to convince them to sign over their power of attorney to the unit. This place should be investigated by authorities and shut down!

  • Grace Care Center at Northpointe Tomball, Texas 77377

My mother has been a resident at Grace Care for over a year and has received good to excelent care and attention. It is well maintained and the staff is friendly and well trained. Most of the residents seem content and are kept active. My mother likes it but has never been in another nursing home. I would and have recommend it. Facility laundry could be improved.

  • Tomball Rehab & Nursing Tomball, Texas 77375

Nursing home staff is lacking,short-handed All Of The Time!! There is a lot of turn over in staff from nurses to cna's can't seem to get good people to stay and work there. Morale in nursing home is low. State should come and inspect More!! Wait times to get patients in and out of beds take longer, cause there are not enough people. Upper management seems clueless in keeping nursing home running smoothly.

  • Grace Care Center at Northpointe Tomball, Texas 77377

Extremely disappointed in the lack of care and disregard for the residents health and well-being in this facility. From administrators to nurses, doctors, certified nurse assistants, the Rehab department and even the kitchen staff there is absolutely NO COMMUNICATION between each other. Administration from the top down need to get off their behide and help when they are short staffed Resident end up without care , the certified nurse assistants are runing around doing all the work while everyone else sits around at the desk or in the office talking and goofing off And i feel sorry for the certified nurse assistants cause they don't even have that many of them to do the workload.. so if you went your family member to be well takeing care of don't bring them to grace care you will regreat it !!!!!
