Skilled Nursing:

1. resident personal care place entirely upon aide with little supervision by nurse.

2. resident sitting in soiled diaper for longer than 2 hours

3. resident not repositioned in wheelchair or bed every 2 hours

4. resident head-of-bed raised as medically prescribed

5. resident call button not answered for 40+ minutes

6. nurse does not assist when aide is busy

7. problems reported to Director of Nursing with little or no change in procedure

8. all this for the fee of $233 per day = $7,000 per month!
