  • Texas Institute for Clinically Complex Care Houston, Texas 77064

This place is horrible.I don't understand how anyone would allow their love ones to stay at this facility. If has roaches, ants and other dead or dried up spiders and other insects in corners of the building. The short staffed and I understand why who wants to work in a flity place where when u finish cleaning you swear something was crawling on you.If u love your mother,fFather, daughter or son don't bring them to this facility to live or stay short term if horrible,it's a nightmare.

  • Solera at West Houston Houston, Texas 77084

  • Spring Branch Transitional Care Center Houston, Texas 77055

Trashy unsanitary rooms, uncaring staff. Everything in this place is not good. People should not be left in care of the staff, Smells like urine, one woman was literally screaming "nurse!!!.. nurse!!!" and no one showed up. Took my mother home after one day and a half . VERY DISSATISFIED . I'm not even sure HOW they are still open and helping our seniors. This place should be shut down. Smells horribly., elevators feel like you need to hold on to dear life.. staff didnt even help mother take a bath. IF I COULD GIVE IT ZERO STARS I WOULD> if you care about your loved ones please do not take them here to this facility

Husband was brought there yesterday with a broken hip from Memorial Hermann Hospital where I was erroneously informed this outfit was connected with Mem.Hermann. It is not. It's an old building that should have been razed a long time ago, clean, but decrepit, cheap furnishings. He did not receive his evening meds. it took transfer to 3 rooms to finally find one where everything worked. This am he got part of his meds, a slop of oatmeal he said was uneatable, they forgot coffee and he gave up on it. His comment - this place is terrible. Shame on his insurance, Texanplus HMO for dumping their members into such a place. I tried frantically to call him, had the old room no., called the front desk, listened to a lot of "your call is important to us", no-one at the nurses desk til the 3rd try. Husband could not reach the phone in his "new" room, says it takes a long time for anyone to show up. I'll if and what type of PT he'll receive today, check on his meds and bring him some edible food. What a dump!

I will not send my worst enemy here!! Please Please Please do not send your loved one to this facility UNLESS someone will be staying with them the ENTIRE time!! I repeat……Do Not Send Your Family Member To This Facility UNLESS Someone Will Be Staying With Them The ENTIRE time. Even if they are only there for rehabilitation.

My family member said that they never answered the call light, the primary nurse rarely came in, not only did they not adhere to the food diet.. it was cold when served, the staff members were rude(all except for two people), they act if they were "angry" when they had to assist with anything related to hygiene and bathing.

I thought that my family member was over exaggerating until I visited early one morning and stayed until 8pm. I was able to confirm everything that was told to me.

Insurance/Medicare should not pay these people until staff re-training is completed and additional guidelines are set in place. They have POOR Continuity of care!!

  • Jacinto Nursing & Rehabilitation Center, LLC Houston, Texas 77029

For those of you having problems with the staff talking about patients, you need to report them to administration and even take pictures or videos if possible. This actions that your talking about are in violation of patient privacy laws. Take pictures of their posts too.

  • Villa Toscana at Cypress Woods Houston, Texas 77014

I give them less than a 1 star! Horrible communication, they wont answer the phone! You can call all day and night for days on end, and either the phone keeps ringing, or they hang up on you. They refused to listen to me when I kept telling them something was wrong with my mother. They neglected her and chemically sedated her! She ended up hospitalized due to a very serious illness, and she was also dehydrated and malnourished. She remains in the hospital now almost a month later! They were even making fun of me in the hallway for causing "drama", and said that my mother wasn't sick, she was crazy! Don't send your loved ones there!

  • The Heights of North Houston Houston, Texas 77090

My Mom was admitted into the facility under Hospice. During her short stay, she totally regained her strength, changed her diet from pureed to regular, and started walking unassisted again. The only reason we are no longer there is because my Mother's health conditions required a different setting. Words cannot express the experience we had while there. During my Mom's stay, the staff couldn't have been more professional, courteous and attentive. During very trying times of my Moms health condition, the staff was very understanding. Anytime I had any questions or issues the D.O.N Sharon always addressed it immediately and professionally. Administration and all other staff was always very impressive the entire time.

  • Solera at West Houston Houston, Texas 77084

My grandmother was only there a week and had to be taken to the ER. She is oxygen dependent and has a history of congestive heart failure. She had been telling the night nurses she can't breathe and they told her there was nothing they could do about it. When we arrived in the morning, her face and abdomen were extremely swollen. The nurse said she heard no fluid in her chest. We asked for an ambulance and after an hour, they finally arrived, which I know was not their fault. The facility moves very slow and was in no hurry to get her the care she needed. The EMS worker said she is full of fluid as soon as he put the stethoscope to her chest. Test results at the ER confirmed this of course. She also has a catheter and we had to point out to the nurse that the cap was left off, causing urine to be draining onto the carpet in her room. Many times throughout the week she was there, we would push the button for a nurse and it would be 30-45 minutes before we would see anyone. When she is released from the hospital, she will not be returning there.

Although I do believe this facility has a very small handful of people that actually care and are caring, in retrospect and as advice to anyone looking for a facility for a loved one, particularly in a skilled nursing capacity, RUN AWAY FROM THIS PLACE. I would not board my dog there. MCHRC was partially responsible for the decline and ultimate death of my mother. Staff is either overworked, understaffed or just plain apathetic. I had to constantly be there to make sure her needs were addressed and in the end, I was the one who saw her in distress and demanded an ambulance be called and her taken to the hospital. This occurred while she was laying in a pool of melted ice cream of which she was incapable of feeding to herself. A simple horrible and depressing place. English is not the primary language for a large population of the staff at MCHRC. Nursing staff is seldom in the room unless a relative is in the building, which is why it is imperative to be there as often as possible. Overall, with a ranking of 10 being superb and 1 being deplorable, I would rank this facility as close to 0-1 as possible.

  • Park Manor of South Belt Houston, Texas 77089

The hospital admitted my mother here after her hip surgery. The very next morning I went to visit she was laying in her own piss, cold without a blanket. Hadn't had breakfast or medicine and it was 9am. She complained she called for help but no one came to her room the entire time she was there…14 hours!

When I told them I was taking her home they said whatever. Don't send loved ones there! The surgeon at clear lake should be fired for sending her there.

  • Park Manor of South Belt Houston, Texas 77089

I read the reviews prior to placing my father at Park Manor of South Belt. They definitely turned me away; however, my dad’s doctor is well connected there. Had it not been for his doctor’s suggestion that he be admitted there, I had already done my homework by picking out and visiting another place altogether. But since the reviews were all over a year old, and things do change, I have a favorable review:

My dad was there for 2-3 months over the fall/winter 2014. Both he and my mother (she has since passed) previously spent time at another place in Pearland. His time at Park Manor was for the most part positive. I’d like for the staff (and those looking to place their relatives) to know how I rated this experience. I especially want to give accolades to the CNA’s who helped him keep up with his hygiene, as well as a woman who took him on his daily walks. So very important are these people. If I were to suggest one improvement, it would be that every resident have their own (wireless) headphones. Because many residents (short/long term) are hard of hearing. This would alleviate any uneasiness amongst roommates, staff, etc. Oh, and one more suggestion, a diaper wipe warmer (like newborn babies have). Nobody wants to have their butt wiped with something cold!

    Rooms/Bathrooms (good size)

    Hot Water (instant)

    Bed Linens (routinely changed out)

    Doctors’ (kept family informed)

    Staff (big plus)

    Meals (good variety and yummy)

    Air Quality (fresh)

    Lobby (comfortable and inviting)

    Rehab/Work-Out Regiment (strict)

    Activities/Celebrations (plenty)

Nurses Station (needs improvement--acknowledging vs. ignoring us laymen)

Call Lights (sometimes he had to fend for himself)

    Weekends (short staffed)

  • Woodwind Lakes Health and Rehabilitation Center Houston, Texas 77040

I would give them NO stars as well. My mother has alzheimers and can not care for herself at all. The caregivers there that I have encountered are negligent at best, criminal in my opinion. I have to ask for them to come and change her diaper EVERYTIME I visit, the stench is overwhelming and I have actually vomited. If you ask them to come and clean her up they cop an attitude or disappear. There is ALWAYS feces all over her hands and arms, if they change her at all they refuse to clean her hands and I guess she is expected to feed herself with feces all over her hands. She can no longer feed herself properly and there is NEVER anything for her to drink in reach, her cup is placed across the room. She can not use a fork or spoon on her own and is left to try using her eye glasses to try and eat. She has been sent to hospital at least four times severely dehydrated and or her blood sugar dropping and labs coming back "horrible" and she has only been here since September. It is kind of weird that the last hospital she was sent to was different from where she was sent the first three times, maybe people are trying to cover the fact that she keeps getting in the same condition. Gee wonder how that keeps happening. It is because she does not get the basic care that a diabetic alzhemers patient requires. I think they should be closed down permanently and prosecuted for negligence. Do not send a loved one to this torture chamber!

  • Park Manor of Westchase Houston, Texas 77082

This place is very, very pathetic. The office staff is incompetent including most of the nursing staff (except for 1 or 2 nursing staff persons). The social worker is absolutely ignorant regarding her responsibilities and has no idea what she is doing. My father was there for a couple (2) months during the 1st month my father's feeding tube was dislodged. he spent an entire week there at a nearby Hospital to have another feeding tube surgically imputed. upon discharge from the hospital park manor would not fulfill their responsibilities of transporting my father from the hospital back to their facility. I was notified by one of the office staff members that I would have to transport him back to there facility. Please do not take your love one(s) to this horrible place.

  • Silverado Hermann Park Houston, Texas 77004

Terrible!!!! Run from this place as fast as you can as they are unprofessional and uncaring! Your loved one will be far better not going to this facility! The Nurses and CNA's do not consistently feed the patients or change them. They never answer the call button (red flag). The entire staff is unprofessional and they fight constantly in plain view of patients and their families. To fill their facility they admit homeless patients. The Administration is always in meetings and they never fully address your concerns/grievances. Filed several grievance reports and am still feeling the effects of their unprofessional behavior post discharge. Not only that, but fighting with Pharmacy that is billing my ill mother thousands of dollars for meds that should have been covered by Medicare but wasn't because of intake errors by Accel. Did I mention that I held the elevator for a mortician who was taking away a dead patient? If you want your loved one better, find somewhere else to send them.

  • Solera at West Houston Houston, Texas 77084

My grandmother has dementia and had hip surgery, which landed her a stay at Solera West Houston (2101 Greenhouse Road, Houston, TX 77084) for 23 days. I am beyond appalled at the “care” she received during her stay at Solera. The overall obliviousness from the entire staff, including management, is completely unacceptable. Nurses had to be reminded daily of the fact my grandmother could not feed herself or communicate. Anytime we spoke with a manager, whether in person or by telephone, they had no idea what was going on and never did anything to resolve the issues. Anytime we would ask questions we would be responded to with a thick level of attitude and essentially be blown off instead of giving a response. By day two, someone from our family was with her during all feedings and at least twelve hours per day. Staff members would just leave her meal on the table feet away from her bed. A week later, they put my grandmother in a wheel chair and left her by the nurse’s station unattended with no supervision. She fell out of the wheelchair and was laying on the floor face down helpless for a minimum of fifteen minutes; which resulted in a bump on her forehead and a large cut on her forearm. Every time a family member arrived to visit my grandmother, we would find her lying in her bed alone in the dark with no one in site. We would find my grandmother every time with her diaper soiled and needing changing and cleaning, and would only be done when we firmly recommended to the staff to do so. The smell of urine in the facility and hallways was horrendous. Staff members always seemed to be gossiping or on their cell phones. The entire 23 days, my grandmother did not receive a bath/shower and by the end of her stay developed a Urinary Tract Infection. The UTI was not even noticed by the staff at Solera, her regular care facility discovered it the first day she returned. The day before her doctor’s appointment and the doctor cleared her to go back to her regular care facility; a social worker called her regular care facility and told them she was going to become a permanent resident at Solera. This was in no way true and I’m amazed that a mistake like this could happen and some people unfortunately do not know any better. My grandmother was transferred back to her regular care facility with no records of the care she received and no hand off report while at Solera. I am astonished that Solera would discharge a patient and not include medical records when the patient was there for twenty three days. Solera desperately needs to be looked into and you should be very weary about leaving a love one here. It baffles me that an elderly rehabilitation facility could display so little compassion for those whom are not able to care for themselves. It is a shame and disgusting.
