My father chose this facility for rehab because my family was going to be out of town the day he was released from the hospital and for a further 2 days. He picked this location for convenience. The BIGGEST mistake of his life. Literally. The lack of compassion from the nursing staff, the lack of making sure he was properly fed and had something to drink was unbearable. The only thing that was good coming out of the place was the PT(physical therapy). Every time I came to check on him after work, he said they come around sometimes, but not enough. They refused to wash his clothes because he was only a rehab patient. He was in there about a month and a half. The DAY before he was supposed to be released to come home, he had suffered a massive heart attack. He called me telling me he wasn't going to be coming home, he wasn't feeling good, and that they can't do anything about it until Sunday(it was Wednesday), when the doctor would be able to come by and decide yay or nay on his release. Needless to say he was rushed to the ER by ambulance because my father's girlfriend was there that day, and she demanded it. When he was admitted, he was so dehydrated that all his organs were shut down, and had suffered one, possibly TWO heart attacks that week. When I went to gather his belongings from his room, I knew he wasn't coming back, the receptionist said "No need to worry, my dear, I'm sure he'll be checked out, and sent right back to us", like that's a common situation or something there. That boiled my blood. I got into his room, there was evidence of something amiss. Blood stained napkins and pills allover the floor. Like someone didn't care to see him take them, or bother noticing that something was very wrong. He died one week to the day he was hospitalized. Please people don't send your loved ones to such a thoughtless facility. It might just cost them there life.

I would not use this facility again, my father was sent there for rehad, after a feeding tube was put in. He's also a diabetic with amputations, mild dementia. The CNA's there are very untrained and do not care one time I was leaving his room and the door knob was covered in ointment! the stuff they put on his behind!!, he developed a bed wound and they never reported it, we found it covered in feces, blood and ointment. He was unresponsive for a full week, before they decided he needed to be in the hospital, disregarding our concerns… Now my father is in the hospital with c-diff bacterial infection and is very septic, he's been unresponsive for 2 weeks. I'm positive he was contaminated at this facility, I would not take my loved ones here ever.

I have been visiting family at Northpoint for 5 years. Things I like, the staff is very responsive. And consistent. The kitchen is is agreeable to special meals. The food is about B+ quality restaurant akin to a Luby's cafeteria. Which is amazing considering state budget allocations for Medicaid patients. Definitely NOT standard institutional food. And the go all out for holidays to make sure residents are not missing anything they might get were they able to be at home. The nursing assistants, who provide the bulk of care, are conscientious towards the individual requirements of my family.

What could they do better? This is the 21st century and people have electronic gadgets. It would be nice if the rooms were updated with more electrical outlets and Internet access.

When we first moved our mother there, we found it to be clean, bright and soon were overly impressed with the staff. The Administrator Tom was amazing, great philosophy and great rapor with his Director of Nurses Matt, and the entire staff! Well…that was then! Tom's father took ill and he transferred to an out of state facility to be near him…then Matt left…then our mother's nurses..Megan and Asleigh left…then many others followed. No longer was this a place where you felt the unity of it's employee's and management..it was quickly heading in a downward spiral of turmoil, disgruntled and many times incompetent staff. The management (or the lack there of) was inconsistent, and it showed in the revolving door of CNA's, and nurse's. We never seemed to have the same nurse or aides for our mother, and once we thought there was a good one..they would be turning in there notice and going elsewhere. I do not have enough space here to list the plethora of neglectful and incompetent treatment my mother, as well as many other residents received. I went numerous times to those in charge (even Corporate) and expressed my concerns as to the care and well being of our mother. I was given basically what ended up being "lip service" …in regards to things getting better. I was more than understanding about transition..and gave them the benefit of the doubt numerous times…but to my regret, I should have followed my gut and moved our mother to another facility. She is bed-fast, cannot move at all on her own, yet they claim she fell out of bed. The CNA picked her up and put her back in bed, and passed it off as a minor skin tear on her head and elbow. Our mother is very frail and brittle boned, and also takes Cumiden ( a blood thinner). A "supposed fall" as they claim should have warranted an immediate 911 call, NOT moving her back into bed without stabilizing her neck or back. It was not until the nurse coming on duty later that night determined she should call and report the "fall" to the facility Dr/Nurse practitioner. THEN they decided to call the EMS and have her transported to an ER to be checked. It was then discovered that our mother had suffered a fractured C1 and C2 of her spine…of course at her age, and due to her degenerative bones, surgery is not an option…so now she is to be in a cervical neck collar for most likely the remainder of her days…that is IF she even recovers from the trauma. She, along with anyone else whom I know or love will NEVER step foot in this once amazing facility…and if I could shout it from the rooftops…no one else, nor their loved ones would either!! There are NO excuses for neglect,…. under-staffing, improperly trained staff, poor management, all lead to this …bottom line…the RESIDENTS, many whom are alone and disabled and cannot speak for themselves…THIS is who suffers…and it's just NOT RIGHT!!! I hope those who read this review and who might have consider this facility,…will think again! It may look like a rose, smell like a rose…but it's most definitely NOT! If your loved one is by chance there…search and find another place to move them…or else be there every minute you can, ask questions and demand answers and change! I only wish I had of listened to my gut, or seen a review like this first..it may be too late for our mother now!

very very bad facility i have never seen that poor management facility in my entire life, administrator always want to cut staff so he can put money in his pocket and let patients suffer , very high rate of falls, and poor skin treatments, plz never put your loved one there, administrator is a monster and the ADON is worst than any body.

I have performed music several times at this nursing home and my great-grandmother is a client here.

I have read other reviews and it seems that the cafeteria is not Ruth's Chris Steakhouse, but it also isn't awful. I typically eat with the seniors following performances and was satisfied with what was purchased. There may be an item on the plate that was unpalatable, but the variety ensured that there were plenty to consume and nourish those who ate there everyday.

The care was satisfactory. I never saw a client that was in need who wasn't being helped. The staff was always busy and working their ways down the halls to complete their tasks. The staff is evidently reciprocal to clients who are friendly and often let them indulge in their stories or other facts of life.

I believe this to be a nice facility, but I have personally not dealt with the administrative side of things at this establishment, but my grandparents who deal with my great-grandmother's finances have expressed displeasure seldom compared to satisfaction.

Also, my great-grandmother has said that she is very happy there. That's good enough for me.

While visiting our relative a staff member came to assist him to his wheel chair, the way she lifted him caused her to lose control & if we had not been there he would have fallen. He was just recovering from a previous fall requiring stiches in the head & arm brace. he has suffered multiple falls in the past. How are these employees trained or tested to make sure of proper handling of residents to avoid injury. who is responsible for staffing????

Nursing staff is O.K., but all seem fearful of losing their jobs at any minute. Administration is awful, with little direction and total emphasis being on generating most money for least cost. Food for clients was dismal. Facility it ‘self is very pleasant, and appears well kept, but it’s just a façade to cover the shady dealings by administration. They repeatedly lied to me, and threw up roadblocks to prevent insurance from paying and to keep my relative on private pay (from which they get more money). This one should be avoided at all cost.

The horrific journey we have had since our mother began needing "Assisted living", "memory care" and now "long term care" had us at the end of our ropes…that is until we had the pleasure of her now being at Green Valley Healthcare on Rufe Snow in NRH, Texas. We cannot tell you what a pleasure it is to have "peace of mind" in regards to how our mother is being cared for here. The facility is bright, clean,…NO smells other than fresh and clean. The staff is amazing! Our mother has done a 180 in regards to her alertness, pain level, and happiness since having moved to Green Valley. While she is on a "modified food diet", it still tastes good and she is eating much better here than any other facility. The nurses take time to truly "know" their patients…Megan and Ashley and the weekend nurses are wonderful and are always informative as to how our mother is doing. We are more than thankful and relieved to have found a place that is a wonderful as Green Valley is to care for our mother! IF there were one single issue that I could say concerns us is that her laundry (which the facility does free of charge) does not always make it back to her….even though it's all labeled…still is sometimes an issue….but other than that….We HIGHLY recommend this facility to any family in need of excellent, attentive, thorough and loving care for their family members needing nursing, re-hab or long term care!

Can things get any worse here. Today when we visited i brought pictures & left them on the bedside table of resident. When we returned from lunch someone had drilled holes in the wall for a light fixture & didn't cover the table. The pictures i brought had tools & dust all over them & dust was all over a tissue box, dental equipment, ect just to name a few of the items. The cafeteria experience was awful. One women was coughing so hard she was she was gasping for breath before anyone responded. Another women was feeling her way around in her wheelchair banging in to other patiences while the help looked helpless. Total disorganization among the cafeteria help, i've repeatedly had to remind them to bring milk that is clearly listed on the residents menu. One resident was all bent over asleep in his wheelchair, his head & neck a purple color, obviously had been in that position awhile. Surprise visits would be highly recommended. While speaking with other visitors they have voiced similiar concerns.

There is a serious need for the cafeteria service to be looked into. Today they were serving undercooked bloody chicken. The residents are not being provided with the help eating that some of the very obviously need. There is an atmosphere of the right hand not noing what the left is doing in slow motion.

This will make the second complaint on treatment of patients in the cafeteria. After noticing a patience with no food after lunch was about over i question why she didn't have any food or drink. I was told i guess we missed her even though atleast 3 employees were standing around just talking. This problem needs to be Addressed.

While visiting a resident there last week in the cafeteria there was a loud outburst and arguement between two employees that involved yelling across the cafeteria and ignoring the patients. This lasted several minutes before another employee intervened. It was a very frighting situation and the Director was made aware of it, i hope this was dealt with for the safety of the patients.

This facility needs to educate their staff on their assigned patients. Never under any circumstances should a resident or rehabilitation patient ever have to be sent to the hospital for neglect. It is unacceptable in every sense of the word. I would never ever leave a loved one at this facility. You are supposed to trust that the staff here will care for your loved ones. I am disgusted to know that my family member was sent to the nearest hospital due to dehydration. The word "healthcare" should be erased from this establishment. The food they serve is horrible to say the least. I thought broccoli was supposed to be green not brown mush. I would not even feed this food to my dogs. I mean these establishments are being paid good money by insurance companies/families for well being and treatments. There residents and patients deserve a whole lot more than what they are being given now. It makes me sad to have seen all that I did. There is no accountability. The residents and patients here have no say. Most of the nurses do not have good bedside manner. The CNA's were much more in tune with my family member more then the RN's. That in itself is a problem! If you love your family member please please do not put them in this facility. Do your homework and pick a place that will fit the needs of your family and especially the patient or resident. By the way my family member spent thus far 3 days in the local hospital due to neglect period! I wish I had something better to write on this facility but I do not. I can not urge you enough to do your homework.
