  • Life Care Center of Haltom Haltom City, Texas 76117

My father chose this facility for rehab because my family was going to be out of town the day he was released from the hospital and for a further 2 days. He picked this location for convenience. The BIGGEST mistake of his life. Literally. The lack of compassion from the nursing staff, the lack of making sure he was properly fed and had something to drink was unbearable. The only thing that was good coming out of the place was the PT(physical therapy). Every time I came to check on him after work, he said they come around sometimes, but not enough. They refused to wash his clothes because he was only a rehab patient. He was in there about a month and a half. The DAY before he was supposed to be released to come home, he had suffered a massive heart attack. He called me telling me he wasn't going to be coming home, he wasn't feeling good, and that they can't do anything about it until Sunday(it was Wednesday), when the doctor would be able to come by and decide yay or nay on his release. Needless to say he was rushed to the ER by ambulance because my father's girlfriend was there that day, and she demanded it. When he was admitted, he was so dehydrated that all his organs were shut down, and had suffered one, possibly TWO heart attacks that week. When I went to gather his belongings from his room, I knew he wasn't coming back, the receptionist said "No need to worry, my dear, I'm sure he'll be checked out, and sent right back to us", like that's a common situation or something there. That boiled my blood. I got into his room, there was evidence of something amiss. Blood stained napkins and pills allover the floor. Like someone didn't care to see him take them, or bother noticing that something was very wrong. He died one week to the day he was hospitalized. Please people don't send your loved ones to such a thoughtless facility. It might just cost them there life.

  • West Side Campus of Care Fort Worth, Texas 76108

My mother was at this facility for less then 2weeks. We had to move her out due to the social worker Leslie. Leslie was greatly concerned that my 81year old, stage 7 Alzheimer's mother, " just couldn't be reasoned with ". what - her exact words!!Many great staff members at this facility that seem to care, but if Leslie is part of the staff, I would say avoid at all costs.

  • Trinity Healthcare Residence Fort Worth, Texas 76108

My brother, Tom Staples, was in this facility from late April until his death on July 1st. The staff in the Pulmonary Unit was friendly, caring and professional. They addressed our questions and responded quickly to any need… even to the point of remembering to charge his cell phone each evening. Their coordination with the hospice staff that was called in on June 21st was also appreciated.

The administration of the facility is sorely understaffed. There was no full-time receptionist so phone calls rang many times until someone finally decided to answer. The business office, among other things business related, is charged with answering questions about Medicare and Medicaid. Ms. Mxxx Lxxxx is the only employee in the business office. She was of great assistance in submitting the Medicaid application and following up with me and the Medicaid folks. She was great to work with.

  • The Harrison at Heritage Keller, Texas 76244

My mom was here for rehab after a hip injury. She enjoyed the therapy staff. She did not receive a dinner meal on two different occasions. Despite the kitchen missing her ticket the CNA's did not notice they failed to pass her a tray. CNA's were rude. My mom had a nose bleed that sent her back to the hospital. The facility did not clean the blood from her bathroom which she shared with a room mate. Upon returning 4 hours later I asked for wipes to clean it up myself. They gave me facial wipes and said they had no antibacterial wipes. She got blood on her fabric chair and it was never removed for cleaning. I am sure anther persons loved one is sitting on it now. My mom is an independent woman who went back to her home after 10days. She was uncomfortable there and did not wasn't to use her call light because staff was rude. Medication usage at discharge was confusing as the discharging nurse wasn't sure what all the medications were for.

I would not use this facility again, my father was sent there for rehad, after a feeding tube was put in. He's also a diabetic with amputations, mild dementia. The CNA's there are very untrained and do not care one time I was leaving his room and the door knob was covered in ointment! the stuff they put on his behind!!, he developed a bed wound and they never reported it, we found it covered in feces, blood and ointment. He was unresponsive for a full week, before they decided he needed to be in the hospital, disregarding our concerns… Now my father is in the hospital with c-diff bacterial infection and is very septic, he's been unresponsive for 2 weeks. I'm positive he was contaminated at this facility, I would not take my loved ones here ever.

  • Senior Care of Stonegate Fort Worth, Texas 76109

'm giving Stonegate Nursing Center in Ft Worth two stars which are for the nursing and care staff, who really deserve five.

The office staff deserve negative stars, which is what brings it down so far. From admitting to billing, they were condesending and almost completely rude. The director was dismissive of my concerns about therapy. Therapy, with one exception, appeared lazy. My mother, unfortunately, was having her heaing aids repaired when she needed to be in the center, the PTA told me she could hear her, when Mom has a profound hearing loss. That's just an example of the many problems I encounterred there.

  • North Pointe Nursing and Rehabilitation Lp Fort Worth, Texas 76148

I have been visiting family at Northpoint for 5 years. Things I like, the staff is very responsive. And consistent. The kitchen is is agreeable to special meals. The food is about B+ quality restaurant akin to a Luby's cafeteria. Which is amazing considering state budget allocations for Medicaid patients. Definitely NOT standard institutional food. And the go all out for holidays to make sure residents are not missing anything they might get were they able to be at home. The nursing assistants, who provide the bulk of care, are conscientious towards the individual requirements of my family.

What could they do better? This is the 21st century and people have electronic gadgets. It would be nice if the rooms were updated with more electrical outlets and Internet access.

  • Marine Creek Nursing and Rehabilitation Lp Fort Worth, Texas 76106

Marine Creek Nursing Rehab. can be a better place if they care more about the patients. The staff need to work together as a team. As a visitor, I noticed the facility was unorganized and the room wasn't cleaned. Vacant rooms beds was unmade. This facility need much improvement.

  • Green Valley Healthcare and Rehabilitation Center Fort Worth, Texas 76148

When we first moved our mother there, we found it to be clean, bright and soon were overly impressed with the staff. The Administrator Tom was amazing, great philosophy and great rapor with his Director of Nurses Matt, and the entire staff! Well…that was then! Tom's father took ill and he transferred to an out of state facility to be near him…then Matt left…then our mother's nurses..Megan and Asleigh left…then many others followed. No longer was this a place where you felt the unity of it's employee's and management..it was quickly heading in a downward spiral of turmoil, disgruntled and many times incompetent staff. The management (or the lack there of) was inconsistent, and it showed in the revolving door of CNA's, and nurse's. We never seemed to have the same nurse or aides for our mother, and once we thought there was a good one..they would be turning in there notice and going elsewhere. I do not have enough space here to list the plethora of neglectful and incompetent treatment my mother, as well as many other residents received. I went numerous times to those in charge (even Corporate) and expressed my concerns as to the care and well being of our mother. I was given basically what ended up being "lip service" …in regards to things getting better. I was more than understanding about transition..and gave them the benefit of the doubt numerous times…but to my regret, I should have followed my gut and moved our mother to another facility. She is bed-fast, cannot move at all on her own, yet they claim she fell out of bed. The CNA picked her up and put her back in bed, and passed it off as a minor skin tear on her head and elbow. Our mother is very frail and brittle boned, and also takes Cumiden ( a blood thinner). A "supposed fall" as they claim should have warranted an immediate 911 call, NOT moving her back into bed without stabilizing her neck or back. It was not until the nurse coming on duty later that night determined she should call and report the "fall" to the facility Dr/Nurse practitioner. THEN they decided to call the EMS and have her transported to an ER to be checked. It was then discovered that our mother had suffered a fractured C1 and C2 of her spine…of course at her age, and due to her degenerative bones, surgery is not an option…so now she is to be in a cervical neck collar for most likely the remainder of her days…that is IF she even recovers from the trauma. She, along with anyone else whom I know or love will NEVER step foot in this once amazing facility…and if I could shout it from the rooftops…no one else, nor their loved ones would either!! There are NO excuses for neglect,…. under-staffing, improperly trained staff, poor management, all lead to this …bottom line…the RESIDENTS, many whom are alone and disabled and cannot speak for themselves…THIS is who suffers…and it's just NOT RIGHT!!! I hope those who read this review and who might have consider this facility,…will think again! It may look like a rose, smell like a rose…but it's most definitely NOT! If your loved one is by chance there…search and find another place to move them…or else be there every minute you can, ask questions and demand answers and change! I only wish I had of listened to my gut, or seen a review like this first..it may be too late for our mother now!

  • Fort Worth Center of Rehabilitation Fort Worth, Texas 76104

I checked with my lawyer and he said it is not libel if it is true. This is true.

My wife came here the day after surgery for wound care. It took 24 hours after admission before all my wife's medications were available, including her pain medicines. I had to medicate her from the medicines we brought with us. I once found her bandages soaked - they had not been changed for 2 1/2 days despite a high infection risk and a doctor's order that they be changed daily. One Saturday evening, they ran out of her primary pain medicine. Ran out! When I went to get our own meds out of her shoulder bag, they had been stolen. It then took them 24 hours to get the medicine to the hospital and get her a dose because of their "protocols", per the hospital director. Common RX available at any neighborhood pharmacy!!! The Chief Nurse later told me running out was the fault of the wholesale pharmacy that serves their account, even though her people gave the last remaining pills and took no action! The very next day, my wife did not get this very same pain medicine again because the nurse "couldn't find it". (They were at the other end of the hall in the wrong medicine cart.) They served fish to a patient on strong medicine that had bones in it. I provided all her meals from outside after that. Another patient's medical files were left on a table in the family room on a Friday afternoon, laid there all day Saturday and Sunday, and were not removed until mid-morning Monday. When I took that info to the hospital director Monday afternoon, he said that was an isolated incident and he would check on it. I told him if he wanted he could go right now and find unattended medical records on a table in the communal eating area, another isolated incident I'm sure. I have photo's to prove this.

Why did we stay? The wound care nurse and the PT people are top notch. They get one star only because of this. We also stayed because I was able to be present most of the day, everyday. Do not leave anyone you love in this facility unattended.

  • Life Care Center of Haltom Haltom City, Texas 76117

very very bad facility i have never seen that poor management facility in my entire life, administrator always want to cut staff so he can put money in his pocket and let patients suffer , very high rate of falls, and poor skin treatments, plz never put your loved one there, administrator is a monster and the ADON is worst than any body.

  • Arlington Heights Health and Rehabilitation Center Fort Worth, Texas 76107

My family member was in this facility for less than 2 weeks and developed a Stage III pressure ulcer and his bowels were impacted. When we reported his negligent care to the state, the facility immediately retaliated and kicked him out of their facility. The DON repeatedly lied about the care my family member received. Would never let anyone else in my family be sent there.

  • Marine Creek Nursing and Rehabilitation Lp Fort Worth, Texas 76106

My sister was in Marine Creek for rehab after heart surgery. The staff were very attentive to her, even to staying up with her at night when she first got there. Se was in the center for a few months and they did a good job getting to her do needed physical therapy. In fact she is now going to them as an outpatient in their physical therapy unit.

  • Life Care Center of Haltom Haltom City, Texas 76117

I have performed music several times at this nursing home and my great-grandmother is a client here.

I have read other reviews and it seems that the cafeteria is not Ruth's Chris Steakhouse, but it also isn't awful. I typically eat with the seniors following performances and was satisfied with what was purchased. There may be an item on the plate that was unpalatable, but the variety ensured that there were plenty to consume and nourish those who ate there everyday.

The care was satisfactory. I never saw a client that was in need who wasn't being helped. The staff was always busy and working their ways down the halls to complete their tasks. The staff is evidently reciprocal to clients who are friendly and often let them indulge in their stories or other facts of life.

I believe this to be a nice facility, but I have personally not dealt with the administrative side of things at this establishment, but my grandparents who deal with my great-grandmother's finances have expressed displeasure seldom compared to satisfaction.

Also, my great-grandmother has said that she is very happy there. That's good enough for me.

  • Life Care Center of Haltom Haltom City, Texas 76117

While visiting our relative a staff member came to assist him to his wheel chair, the way she lifted him caused her to lose control & if we had not been there he would have fallen. He was just recovering from a previous fall requiring stiches in the head & arm brace. he has suffered multiple falls in the past. How are these employees trained or tested to make sure of proper handling of residents to avoid injury. who is responsible for staffing????

  • Trinity Healthcare Residence Fort Worth, Texas 76108

My daddy ( Richard Martin ) was there for about 2 weeks. They treated him so wonderful, and the family. When ever we needed something they was there question's anything. Myself and my family can't thank them enough for the care of my daddy. The day he passed they still was there. NEVER once we was treated bad. If we needed something it was there. Thank you, to ALL the STAFF at the Courtyards in Ft. Worth. God Bless all of you.
