In past visits observed no parking lot lighting. Only lighting from inside bldg. Since on country road at edge of town, it is pitch black at night. Not safe for visitors, employees or residents sitting on front porch. Lights out in main hallways (flourescent). Residents moved and left in main lobby of facility nightly and during day. Never observed any activities, music, smiling faces, or positive facility amenities. Scarce employees. Old Carpets stained and dirty. Tile floors in resident rooms very old and every visit are nasty, have fecal material, smells bad. Employees, obvious smokers wreak of smoke, grumpy, extremely unprofessional and agitated if asked a question or directions. Front desk persons do not address visitors upon arrival and basically ignore anyone walking thru halls…not safe! Residents lie and sit in soiled clothing until worker can "make it around" to clean them. Sometimes taking an hour or longer depending on if mealtime. Resident waits for meds and/or breathing medications for extended time, usually because Med. person is eating or on smoke break! Management always gone or in meetings. Very under-staffed. If Staff are in dining room during meal prep/eating/clean-up the rest of residents are waiting extended time for meds and/or assistance and clean up from incontinent issues. Limited Staff on weekends and holidays. Observed Staff arguments and discussions in hallways and resident's room (very inappropriate). Repeated mistakes in ordering medications, proper billing to residents'preferred provider and/or personal accounts. Facility is in very poor shape and rooms are dirty on floors, bathrooms, window sills, temperature fluctuations, bugs on floors and window sills, resident personal belongings are stolen on regular basis.
