  • Life Care Center of Blount County Louisville, Tennessee 37777

Do not use this facility. As others state it is a beautiful, clean facility but the care is awful. My father was admitted on 2/2/19 from PNC (a wonderful facility for a higher level of care in Knoxville). On 2/3/19 in the evening I called to check on him and was told he fell but was okay. No one contacted us. My father had a small brain bleed in November and one in December which is why he was at PNC and why he was now in rehab here. Family visited him on approximately the 3rd, 5th and 7th and he appeared unchanged but very tired and doubled over in wheelchair. I contacted the unit nursing manager on the 8th and was told this was due to the intense therapy and they got very tired, etc. She told me he was complaining that his wright wrist hurt but it looked okay. She was ordering an x-ray to be sure. I arrived at the facility on the 8th mid-afternoon. When I arrived I was told by the ladies at the nursing station that he had another fall but there was no documentation and they didn't know what happened. I went to his room and he was lethargic, very confused, could not move his head or the entire right side of his body. His eyes were glassed over and his right arm was extremely hot and swollen. They told me they x-rayed his wrist and it was okay, probably just sprained and not moving, etc. was due to the pain. Ultimately I insisted on a transport to UT Medical Center where they determined he had a significant trauma to his head and emergency surgery was required to drill a hole in his head and place a port to drain the blood and relieve the pressure on his brain. After surgery he is currently still very confused and right side is very weak, although he can now wiggle fingers and toes minimally on that side. I do not know how much he will recover with therapy yet. I talked to the unit nursing manager and was told she had investigated and could not find proof of another fall. Further told their PA had assessed him and determined he did not need transport and that was really all she could tell me. They have no answers or charting for the condition I found him in. I asked for other information on escalating the issue and she gave me a name for the head nurse and interim administrator. I left a message for the administrator and she did not call me back. I received a call from a social worker who just wanted to let me know they would look into it further…..I am proceeding with formal complaint with the state. I do not want this to happen to someone else's family member. Had I not arrived on Saturday he would most likely have died from just being left in bed without care. The level of care is unacceptable at minimum. This is elderly abuse and negligence.

My dad was at this facility and we were amazed at the level of care and compassion that he received from all staff. When he was able to go home, he had a set back and was eager to return to this facility. Although he was placed on a different unit, he still received 5-star care from both nursing and rehab. The food was well-prepared and very appetizing. I purchased several meals myself. He was always invited to attend the many activities that they offered. The building was beautiful, clean and smelled good. I would definitely recommend this facility!

  • Life Care Center of Blount County Louisville, Tennessee 37777

The facility is certainly beautiful but the care level is spotty at best. I have spent one week here after undergoing surgery for a colostomy. I've received no training on how to care for my ostomy after discharge, and I'm leaving tomorrow. The food is generally well-prepared and tasty, but the kitchen staff can't seem to honor likes and dislikes and special requests, even when written on menus by their own hierarchy. They also seem to use no common sense concerning nutrition when it conflicts with personal preferences. If you are considering placing a loved one in this facility, I would ensure that a family member or friends visits frequently to run interference.

The facilities are beautifully impressive. For the most part the staff is caring and accommodating. The rehab staff is phenomenal. The downside is my parent was there for rehab and was virtually immobile. When the call button was used, employees would pass the room and not acknowledge them or their needs. The parent would be left with wet or soiled diapers without assistance. Sometimes left in the wheelchair for hours. The side dishes are good but the meats are too tough to chew.

  • Life Care Center of Blount County Louisville, Tennessee 37777

Facilities are lovely but service and levels of care are terrible. We felt as if we were sold a "pig in a poke". Plenty of promises to get you to use facility but no follow through. Father in Law was here for 2 weeks for rehab after surgery. Took several days for therapy to begin. Very poor communication between departments. Staff does poor job conveying information to patients as well. Patient had health concern and pushed call button. Took an hour to get response. Met with staff to express concerns. They didn't really seem to care and didn't even take notes. Food service is terrible. Quality of food is terrible. If you care for your loved ones do not bring them here. CANNOT RECOMMEND.

  • Concordia Transitional Care and Rehab-Maryville Maryville, Tennessee 37803

I was very pleased with the care my mother received at Kindred Tranistional Care & Rehabilitation- Maryville. The staff were patient, kind, and caring towards her and our family. My mother is diabetic and was happy with the meals offered. She was placed in another facility in the area previously due to a spiral fracture but was unhappy with their care. We are so thankful to find Kindred Tranisitional Care & Rehabilitation- Maryville. Our experience was wonderful. My mom really enjoyed activities and the therapy staff helped my mother overcome her fear of falling and make tremendous strides to return home. I'm thankful this facility exists where good quality care is provided in a clean odor free setting with compassionate staff. I would highly recommend this facility.

  • Concordia Nursing and Rehabilitation- Fairpark Maryville, Tennessee 37801

I chose Kindred-Fairpark because I know people whose experiences there had been very favorable, and because of the location is convenient for me. I have been well-pleased with my own experience there. I have found the administration to be professional, yet very personable. I am happy with the nutrition and the physical therapy. The main thing that has impressed me is the kindness of the entire staff.

  • Concordia Nursing and Rehabilitation- Fairpark Maryville, Tennessee 37801

The staff at Fairpark are doing a very good job taking care of my dad.He has settled in and seems to be comfortable. He likes the staff and says they treat him well.I ask him if I can bring him something special to eat and he says no,that the food is good and they bring him what he wants to eat.Dads appearance is good,he is always clean shaven, haircut when he wants it and clean clothes.Dad is paralyzed from the waist down and needs special care and attention and the staff has performed very well.I am notified of any change in his condition or medication.I have no complaints about the care he is receiving from Fairpark at this time. • Asbury Place at Maryville Maryville, Tennessee 37804

This is a dangerous place to have your loved one. It is on the Tennessee state watch list. They endangered my mother's life more than once. They were unresponsive to complaints. While RNs and mid-level supervisors were fine, there is a culture of not caring among several top administrators and the head physician that is astonishing.Additionally, CNAs, especially on the weekend, do not bother to check on patients. I have sat with my mother for up to six hours with no CNA entering the room. I have found her with bloody lips (dried blood)and in her own urine at 10am - no one checks patients many mornings. Their call buttons do not work properly in one wing, patients can lay for hours, perhaps choking, with no one coming to check. Sometimes, especially weekends, there is a cacaphony of at least half a doze voices crying "help, help" but no one comes (one lady does cry from dementia, but the other cries are real.) No one is watching call lights or even on the floor or at the desk. Trays for patients on special diets for their own safety due to difficulty swallowing are not checked. Nurses were told to give mother medicines that were on her allergy list. Mother's antibiotics were discontinued early by the house physician in spite of a diagnosis of pneumonia. Also, the facility repeatedly attempted to deny us the ability to see our own doctors and specialists. Please do not allow your loved one to go to this facility.

  • Shannondale of Maryville Health Care Center Maryville, Tennessee 37803

although Shannondale of Maryville is a beautiful setting with well kept buildings, it has the same problems as all other health care facilities. Lack of adequate employees to assist the patients, less than helpful and argumentative DON and Dir of Social Svc, unwilling to admitt to problems or try to work out care issues. Lack of staffing, causing problems with patient care and assistance to residents. Management is unresponsive to family complaints and issues.

This facility may be cleaner than most, but amount of money paid does not get better service
