  • Signature Healthcare of Warren Warren, Ohio 44483

    This place is the worst thing I have EVER seen. They should be shut down immediately!

    My mother, was sent to Signature Health in Warren, Ohio after she had ankle surgery on December 17, 2015. Since she arrived we have had nothing but problems with this facility.

    She was scheduled to have a PET scan the following week, which I called and gave Signature all of the dietary restrictions as well as transport needs. They assured me they would set up the transport with their people and even confirmed the times with me. On the day of the scan, my mother called and said no one came to get her and they were blaming it on the hospital for not coming. After making 5 attempts to reach someone, I finally spoke to the director of nursing who apologized and promised they would get it taken care of. Finally, four weeks later, she got her PET scan. This PET scan was because there was a spot on her lung they thought might be cancer again. Great care, right?

    In her first week there, not a single doctor came in to see her, despite our requests. Also in the first week she was there, she was not given a sponge bath or even able to change her night gown. She physically could not move on her own and was relying on their care, which seems to be bare minimum at best.

    At one point in time, a nurse was trying to roll her over and the blanket was bunched up in the way. After being asked to stop, the nurse forcefully rolled her over so hard she left a bruise on her arm.

    After three weeks in the facility she started complaining that something felt wrong in her cast. When we called the surgeon, they said she needed to go to the ER. I personally called the facility (after they completely ignored what my mother was telling them) and told them they needed to call the surgeon's office to get the request, but he wanted her back in the ER to get checked that day. Nothing ever happened. She sat there until the following week.

    Beyond the major complaints that are risks to her health, medicine is consistently given late if not missed entirely, meals are served and random hours (lunch at 2:30 and dinner at 5?) One night dinner was buttered noodles. Literally.The response time when a call buzzer is activated can take 5-10 minutes, which for someone dependent on them for bed pans and emergencies is a long time. The place absolutely stinks of urine as well.

    After seeing the way care was administered, we requested a transfer the first week of January. After hearing nothing for a week, I called on January 8th to speak with the social worker. Again on the 9th I called, but got no response. I emailed the administrator, who promised he would launch an investigation and also work on the transfer. I have been in contact with them multiple times in the last month and it seems like it takes about a week to even get a response from Signature to requests. In the meantime, she isn't starting the physical therapy she needs to heal and they're continually coming in and trying to talk her into staying. My gut instinct is that this place is simply trying to suck up Medicare money by having her stay every day they can.

    I cannot believe a place like this is even allowed to be in charge of the care of the elderly. It is truly awful and I would say even a dangerous place for seniors to be. I cannot put into words how absolutely awful this place is. Disgusting is about the best I can do!

    If I could give this nursing home zero stars, I would. This is the WORST care facility I have seen in my life and the establishment should be shut down. We recently removed my grandfather from this facility due to the lack of care.

    Where to begin… the "so called nurses" pay no attention to their patients. Instead, they give them sedatives to make them sleep all day so they do not have to be bothered with them. There was no reason for my grandfather to be subscribed sedatives, which in turn was making him turn into a vegetable and be incoherent every time we visited him. Also, when walking down the hallway, I heard several patients say "Get me out of here." What does that tell you?

    The nurses are ignorant. I witnessed one of the male nurses wheeling a bed down the hallway and while wheeling it he banged the bed off of the patients that were congregating around the nurses desk. Instead of another nurse moving them aside so they would not get bumped, they just stared and did nothing. I took it upon myself to move the patients out of the way.

    The odor … I have NEVER smelt a more sickening and disgusting place. I wouldn't even put my worst enemy here. The reason for the foul smell is because they allow their patients to lie in bed in their own urine, not take them to the restroom, not tend to them when needed, not clean up the mess in an appropriate manner.

    If the patients weren't sick before they checked in, they are sure to get sick here. One day two of the nurses were sick. Instead of covering their faces with a mask to prevent anyone else from getting ill, they were coughing, blowing their noses religiously in the rooms and at the nurses desk. Very inconsiderate staff. If you ask for help, for more water for your family member who is staying there, etc. it seems as a bother to them. It is the "nurses" job to take care of these people, instead they are killing them.

    As for the physically therapy dept., my grandpa was checked in here a couple of months ago. Before he was checked in, he was able to walk but was somewhat losing his strength. In order for him to regain his strength, he was advised to go to therapy. Well, come to find out, the nursing home has refused to take him to therapy for the past month. My grandpa now has no ability in his legs.

    If you have family members in this facility, get them out! Word of mouth is the biggest form of communication and I will be spreading the word about this establishment.

    My best advice is to shut this place down!

    terrible! they don't give med's properly! they don't help resident get dressed or ready for bed,they leave them in their clothes and shoe's day&nite to sleep in!the administrator is very rude as well as social worker brandy. i watched an aide drop a resident out of wheelchair on her head outside on sidewalk leading into nursing home!!she had a helmet but it was not on! she was thrown back in chair and told to shut up!really???? my sister lost 60 lbs while in there,had bldwk done cuz she was doing so bad,there was no med's in her system at all she is now in a better place,wish i would of known about this sooner! they never schedule her any dr appts i told them she need's mammogram&pap,toenails were like knifes&she had to beg for shower!!!! i was told by hospital& other nursing facilities to report them to state and i am! if u care for ur loved one beware of this facility!!!hard to get out of to,they gave me a real hard time but i won!!!

    My father was a patient of Shepherd of the Valley in Howland and it is such a beautiful, clean facility with the kindest employees!!! He stayed on the therapy floor which was nice because you're surrounded by other people all going through the same thing and discharging home after therapy is finished. He made friends quickly during mealtimes. He is having another surgery in June 2013 and wants to return to Shepherd of the Valley in Howland. NO NURSING HOME ODORS!!!!

    I have a friend who has been at Orange Village for 3 weeks and I couldn't ask for a friendlier staff! They are also extremely accomodating, and have always done their best to answer questions. I appreciate the way they take such good care of her, going the extra mile, and I know my brother does too. Thank you all so much for your prayers and kind words and thoughts. I never knew a Nursing Home would be the ideal place for such a young person recovering from an accident. It has been a difficult 11 weeks, but the staff at Orange Village gives us hope that she has a future! I cannot thank you all enough! God Bless you for the work you do and please know that we are eternally grateful for everything you've done, not just for this young woman, but for her family and friends, who anxiously await her return home!
