  • Van Duyn Center for Rehabilitation and Nursing Syracuse, New York 13215

They left my 92 yr old grandmother in bed for 6 days in pain with her leg. They said they took an x-ray and there was no break. They never contacted the family that she fell. After seeing her we requested that she go to the hospital and they put up a little argument about sending her. Turns out she had a shattered pelvis. For 6 days AND they kept trying to move her!!! She ended up having to have surgery and we are still waiting for her recovery. She will not be returning to this facility.

In Dec 2015, my 91 yr old father entered The Cottages as a self pay due to a severe muscle tear in his leg. He spent 2 days in the hospital, but did not qualify as a Medicare referral. He had to pay $6000 ahead before admittance to cover a $425/day charge. He asked and was told any unused portion would be returned. After 5 days the pain was tolerable enough he felt he could live at home w/family and PT help. Of course, the admin did not want him to leave as they were getting paid the full amount. It was very difficult to get administration to come to discharge him and have the papers completed. He was ignored really for 24 hrs after making his request known. The administrators were very unpleasant to him. Finally he was discharged, but 3 months later he is still waiting for a refund for his unused $6000. Despite numerous phone calls and letters initiated by him, no one has bothered to even reply. This has been very stressful for him. I guess they figure, at 91, they can just wait him out. This is a warning that you may find it hard to escape from here, even if Medicaid is paying!

The Cottages have the potential of being warm, homey, and the type of surroundings conducive to healing. However, the administration of these lovely cottages leaves much to be desired. The food is brought in from not sure where, but at times is really awful. There are alternatives but they are not much better. The nurses and nurses aides are also the kitchen help and serve the food.

I was fortunate enough that I was mobile and could get around on my own with a walker, but those who were dependent on waiting for someone to help them were left alone way too long. Call bells were not answered in a timely fashion. At night the charge nurse shared services with 3 cottages so if you needed pain meds during the night you may have to wait 2 hours or longer.

Physical therapy and occupational therapy were received once a day. The therapists and staff were very friendly and helpful. The PT I received at another facility for a knee replacement was much more aggressive and I was out sooner than I was here for a hip replacement.

Overall I would not recommend this facility nor would I ever return if I needed short term rehabililiration. Much room for improvement at a lovely environment.

  • The Crossings Nursing and Rehab Centre Minoa, New York 13116

I have to say that the best part of the crossings is there physical and occupational therapy department. They are an outstanding group of people. The nursing staff and the aides are a whole different story, for the most part the nurses are good but there are a few that are very rude and verbally abusive. There is a nurse who loves to make fun of the residents who really can't communicate with people to tell them what is going on. They usually have dementia pretty bad he will mock them out, get in there face and repeat what they are saying over and over again. It really made me sick to see that people could treat people that way, I said something to this person and he thought it was funny. I went to the higher ups and nothing was done about this situation. The following day he was still laughing about the same person he made of the day before and then continued the day mocking out more residents. This is not the care that our family members should be treated. Then you have these aides they are to busy to take care of the residents because they are either on their cell phones, taking multiple smoke breaks, in rooms with their fellow co-workers some have been caught fooling around with each other or even doing the deed, and management does nothing to them. I have tried to turn these people in for what I see and management ends up doing nothing but telling the people that I turned in and the in turn they retailiate on me. There is so much more that I could go on about. All I have to say think twice before sending your loved ones here.

  • Central Park Rehabilitation and Nursing Center Syracuse, New York 13205

I have experience to say that Central Park Rehabilitation and Nursing home is a horrible place. There are never enough nurses and aides on hand to help patients and there is not a Supervisor on head on the weekends. How do they really expect LPN's and CNA's to handle a major emergency! The place smells like feces and it is unclean!

When the bell is rung for assistance, it takes a good 15-20 minutes before someone comes to assist you.

Because of lack of assistance, my sister fell in this place and broke her hip. The outcome was an operation. As a result, she had to remain here for a longer period of time, and things just seem to get worse! I had once asked the Nurse Manager if there were enough staff on hand and she said yes. Well, from speaking with others and observation, I would say no; there are not enough staff on hand to assist the patients and if the staff are there, they are not doing their jobs!

The rating for this Nursing Home should without a doubt not have a rating. It is definitely a zero! There is room for improvement and I hope for the patients' sake, it happens soon!

    Thank you

If the pink MOLST form that contains end-of-life instructions is completed there make sure a doctor reviews it with you not a social worker. Make sure a doctor signs it also, it's not valid or legal without that signature. I would like to know how many patients both past and present have legal MOLST forms

The absolute worst place to ever send a family member in the WORLD. It's like HELL!!! Nobody cares and that starts at the top down. Call bell not answered for over an hour. FILTHY all around. Uneducated not my problem staff DON'T EVER GO HERE!!!! over 100 degrees in the summer, no airconditioning working. Place stinks and food is a grade 1 out of 10… 10 being the best. December 14, 2010, The Fed sited them with 19 The fine Only $4615 is related to a March 9 state Health Department inspection of Rosewood, which found 19 deficiencies. EVEN THE FEDS DO NOT CARE!!!! GO ANYWHERE ELSE. The Aids stole money from my moms purse twice! They are physically abusive. The whole place needs to be emptied out and burned to the ground!

Safety and cleanliness issues. Limited security, basement entrance open and can move in and out with no questions asked, residents could leave undetected.Dirty floor and brown spots spattered one resident ceiling above bed. Limited or no response to patient bell calls.
