Review of Four Seasons Nursing and Rehabilitation Center

Hi Everyone,

I just want to make clear the experiences with Four Seasons Nursing Home. Please don't place your trust in the reviews of the Department of Health on the patient care and information you get from this Government Agency. I would like to comment on the type of care that is given. I would say the the place is considerably understaffed in regard to the aids that administer the readied care of your loved one. There is only one aid one the wing to handle more than 20 patients. The food is not inviting at all. I would not even give it to my animals for dinner, as I had been there many times to help my loved one eat. If you are not there enough help to feed your loved one, then it is quite possible that they will be hooked up to a feeding tube. The daily activity is to get you dressed and put you in the day room or in the hall with a wheelchair to spend the day there. There are movies and a few activities for those people who have it together. If incontinent and unable to state that your diaper needs changing, it will be on until you go to bed.

The Social Worker never answer your calls. There is only ONE SOCIAL WORKER for the whole facility. The supervisor of the Social Worker does nothing to return calls either. They made my Mother In Law sign a Health Care Proxy while she had Dementia. Was supposed to contact my wife and myself upon her transfer out of the facility to another, never did and I was lucky enough to meet in the elevator upon her leaving with a transport van. They sent her without a wheelchair, which if I was contacted would have brought my own on a 60 mile trip to Spring Valley. seated on a bench seat in the van. My wife sat next to her and held her from falling from the seat while traveling. Mind you now that my mother in Law is 96 years of age with dementia. She could have fallen to the floor on many occasions during this transport. This facility would have had a major law suit against them because of their neglect, negligence, and just plain stupidity of not informing us of the move and was told by the illegal Health Care Proxy to call us when the move was to take place. Prior to the move knowing what date it would be they sent her clothes to be washed and I had to make 2 trips to pick them up from the facility. I would have been the common sense to hold them until I arrived to visit and given them to me to wash. Very Dissatisfied with this facility.
