According to state inspection reports, from 2009 to 2011 the state found a variety of deficiencies at fIELDSTON lODGE, including failing to maintain records, unsanitary conditions, and not enough visits by the facility's doctor. But during its latest inspection, the state only found 25 deficiencys, and it was NOT corrected at the time of inspection.
