  • Preferred Care at Old Bridge, LLC Old Bridge, New Jersey 08857

    Preferred Care at Old Bridge (Rehabilitation Dept) was home for my MIL for about 6 weeks this winter. She was in there recovering from several health concerns that left her on oxygen for weeks and very weak. She is 90 and needed to rebuild her physical strength back up as well as her lung capacity. She was very well taken care of by ALL of the staff from nursing to food service. My husband and I went daily and each day she told us that everyone is so nice and praised her for all of her efforts in physical therapy . She was able to get her hair done. We added a few things from home to "cheer" her already remodeled- clean room feel more cozy.

    As anyone knows that has been through this before, it's the staff that makes the place. It's a smaller rehab, I knew the staff and and they knew me by name. They reassured me when I was worried and although my MIL left a few days ago we have been in touch for continued questions that I had with things that I needed to learn. The transiton was made easy by the caring nursing staff, all angels. My MIL is strong and independent again and a lot of the credit goes to the exceptional staff from Preferred Care.

    Elderly parent admitted to Alaris as a last resort. The hospital Social Worker & Alaris Social Worker both insisted it was a new facility. I was skeptical. If this place is where I think it is; then, it is not new. It is & I called them out on it too. This place changed hands at least 3 times over the past 15 years.

    The care was absolutely unacceptable as usual. As others stated, do not consider this place. If, you have Medicare Advantage; then, you will be relegated to inferior nursing facilities. I am probably not allowed to recommend preferable nursing facilities.

    Disgraceful facility -- my mother-in-law has been there a little more than 2 weeks and during that entire time she has been there she only had her own clothes on once, has not had her glasses or her dentures since she arrived even though we were told that there is an inventory that shows she arrived there with them. We were told by management that this is something that they are bad at and need to work on!!?

    My mother-in-law has seemed to have lost weight since arriving and today she mentioned that she actually has not been eating much because she hasn't had her teeth and there is no one to help cut her food so she eats very little. This is from someone who has always had a very hearty appetite. She also mentioned that she has been wetting herself because no has been available to take her to the bathroom.

    Staff constantly making excuses that others are lazy or new or under staffed. The lack of care of the patients and the lack of caring by the staff of our concerns I astounding. Management seems to have no control over what staff do since they seem to be constantly in meetings or on vacation as we have also been told.

    Phone calls go unreturned including from management. It was 24 hours before someone even reached out to us that she had arrived at the facility and this was only because we refused to hang up until someone answered the phone. When you call you constantly get transferred to extensions that have full mailboxes so no message can be left. Even the receptionist, who we witnessed in person, could not get someone in the facility to pick up a phone.

    AVOID AT ALL COSTS

    I think they are understaffed. Since it has been bought by ocean healthcare it has gone down hill. There Facebook page is nothing like the unit my Mom has been on for 5 years. Sometimes there aren't any staff members in the dining room. Weekend staffing is bare minimum. Some weekends have only 2 aides for the whole hall. When I bathe my Mom, I remove crud from her feet and naval. I have been questioned about bruises on her by ADON. They have treated devoted family members like criminals. I could go on and on! I ask for very little, and sometimes don't get anything. The unit manager changes often. Still waiting for follow up from 2 years ago!

    And the business office! I can't wait for them to be investigated.

    No stars. They deprived my father of all water and food and shot him with morphine, until his mouth, tongue and gums were bleeding welts from horrific thirst. He had no terminal illness whatsoever, I personally can't and don't condone torture. The administrators and van dyke hospice do. His death certificate said death from natural causes. A guillotine or bullet would have been merciful, in comparison to the unbearable suffering he was forced to endure. To this day, I can not get my hands on his medical files. My mother was the poa. These brutal people treated my father like a sub human, and euthanized and exterminated him.

    One of the worst facilities that I have seen. Disorganized,lack of communication between facility and clients.Misrepresented facts.Relative there 2 months,broke ankle,ran over their foot in wheelchair,hit patient in eye,lost articles of clothing.Patient put in bed upside down.Left patient and did not change for 12 hours.Patient was left at Hospital without notification to relatives which was a directive to do.Patient moved 3 times do to smell/disorganization of rooms. Dietary needs were not met many times after review with facility many times. All facts with pictures and documents.

  • Advanced Subacute Rehabilitation Center at Sewell Sewell, New Jersey 08080

    Hallways are clean and clear of patients sitting in wheelchairs all day. No funky odors when you walk in. That's the GOOD news.

    THESE ARE THE PROBLEMS:

    1) Place must be short-staffed, as "I'll be back" is common response when you use the call button. Then nobody comes back for 1…2…3 hours.

    2) Website says they're certified in my problem but all that means is certified Dr/nurse stops in once a week to check you. Staff nurses do their best--sometimes w/ good results, sometimes not. Then you have to wait until the next shift comes on, if you're lucky, a nurse on that shift will be able to resolve the problem.

    3) MANY (NOT ALL) AIDES BEHAVE VERY UNPROFESSIONALLY. They are loud and impatient, rush you to move, stand, walk, to point you could sustain new injuries. They inappropriately vent anger/frustrations on patients (e.g.-AM shift aide never returned to empty bedpan. Next shift came in and exploded that she "shouldn't have to take care of somebody else's shit"--she was referring to aide from previous shift and she was right about that but she should take it to her supervisor, not to patient who;s already mortified they're using a bedpan.

    4) KITCHEN PERSONNEL ARE APPARENTLY INCOMPETENT AND ILLITERATE. Either they weren't cooking what the menu offered or they couldn't read who ordered which foods. VERY FEW MEALS arrived containing foods that were actually "ordered" from their menu sheets. It was borderline-incredible that it could be so consistently wrong. When you complained it took a very long time for a reply and often "the kitchen [was] closed" or was out of whatever you actually ordered. The meats were steamed and gray-- repulsive and mealy. Very bad food service, which, that should be the least of the problems. Any short-order cook could help them. One aide shared on a weekend "Oh those young kids, on weekends they just want to get done and get out early." Who's running the asylum? And one final thought--- grilled cheese sandwiches consistently were made with one homely slice of American cheese. One slice??? Is the budget that tight? That is something mgmt should be ashamed of and correct immediately.

    Overall it was not a happy experience. The setting was OK but the people and procedures and the way they performed their work left a bitter feeling of "GET ME OUT OF THIS PLACE!"

    My mother was a resident in the Alzheimer's unit for three years prior to her death in 2012. My husband and I visited every week and intentionally did not always come on the same day so the staff would not know which day we were coming.

    My mother was always very well cared for, dressed nicely and sitting up in her wheel chair. She was not able to communicate; however, she was always smiling and the staff seemed to adore her. They were always hugging her and were very friendly with my husband and I.

    My mother went downhill quickly when the time came for her to pass. The staff were coming in and out of her room to say good-bye, with many crying, hugging her and hugging myself. I am so thankful that my mother was so well cared for as all during her life she was meticulous in her dress. I did her laundry and would return it with each outfit together on a hangar. When they dressed her, they never deviated from the specific clothes that were on the hangars.

    The only issue I have is that a good deal of her clothes went missing. I don't know if certain staff stole them as they were very nice clothes, or other residents took them unbeknownst to the staff and they were lost somewhere within the home. I do have to say that quite a few items were lost and I did report the incidents, but it was virtually impossible to find out where they ended up.

    One last item was stolen, and this was devastating. Although I gave them a very high rating, there are always people that steal and there is nothing that anyone can do about it if they are not caught. This goes on everywhere, unfortunately. My mother had a 14 karat gold Blessed Mary medal around her neck that her sister gave her for her 75th birthday. My mother was almost 91 when she passed so she had that for quite some time. The last week of my mother's life, someone stole it. Probably because during her last week she was going downhill and was pretty unresponsive and would not have protested if someone was trying to take something from her. That really upset me as my intention was to then wear the medal myself. So having this treasured medal of my mother's stolen is the only reason that I gave a 4 out of 5 rating.

    So, in conclusion, my mother received excellent care, the facility was clean, the food was standard fare, and I always felt that my mother was loved. Obviously the stealing was an issue, but there are always people who seem to have no soul and take advantage of every situation.

    I feel confident in recommending this facility to anyone in the area that is looking for a place for a loved one.

    I chose this facility for my rehab after a bad TKR surgery (after seeing good reviews on the web) and stayed for 25 days and my experience has been totally unflattering and scary:

    1. Food is of poor quality and often served hours late in the rooms;

    2. Poor response to emergency button calls:

    a. They most of the time ignore the calls even after passing thru' the area when the light in front of a room is ON;

    b. Staff shift changes occur at hrs inconvenient to patients: around 7 AM when many are on IV for 1-2 hrs and the machines keep beeping for several minutes after the IV is over and no one bothers to disconnect them;

    c. Once when I was on IV on a wk-end AM, I felt like going to the toilet and it took 40 mins to get some one to disconnect the IV to enable me to rush to the bathroom!

    d. While the Rehab dept. was good (4*), I won't recommend this to anyone!

    This was my second time at this facility. which is warmly and professionally decorated; very large rooms with a large sofa and love seat to accomondate your guest. I received EXCELLENT treatment! The Nursing staff were very PROFESSIONAL and FRIENDLY, as well the Aides and very attentative to all my needs. The Therapy Dept. had me up and moving. They are very dedicated to their profession and gave warm encourgagement of a "you can do this" attitude. There is a Beautiful large garden where you can go out and relax; watch the Silver Blue Birds, Red Robins along with a large 5' rock water fall which flows into a bed of all types of beautiful fish. There are all types of religious services and many activities guided by the Activities Director. You are never bored or along during your recovery, the aides are always stopping by to check on you which I really appreciated the attention.

    I recommend this facility highly and give it a 5 star rating.

    Horrible facility. Removed my father, lack of care. Always find a NON-profit facility. End result, Kidney disease stage 3 upon arrival at hospital. No previous Kidney disease or issue's. Also keep an eye on billing, they double-dip. Be aware of what insurance actually covers and time frames covered. Manorcare will bill insurance and patient for covered fees.

    mwouny huband contracted one infection after another in this place . no one knew what they were doing. had to be there almost 24hrs daily. one nurse and one aid were the only ones that knew anything the wound vac treatment he needed . I took care of him myself ,cleaned him changed his bedding and also learned the wound vac treatment. the last infection he acquired in that placed put him in the hospital were he died. the place needs a complete overhaul.

    I'm a 57 yr old male who was transfered to Subacute Rehabilitation after major back surgery at Jefferson center city. Arrived 1/27/17, 1pm. I was told I didn't have any meds yet including my pain meds but did receive a shot of a very important blood thinner called heprin. Nurse informed me Jefferson was giving me it in arm which was incorrect and gave it to me in my left side abdomen.The following day I was notified my meds were in and received any pills along with a second shot of heprin on my right side by nurse Megan. That night 2am I hit nurse call button and told aid I needed pain med as I only had 1 since getting there, waited two hours called nurse again at 4am, aid said she would let nurse know again, 6am called again aid said she would let nurse know. Finally got pain meds at 7:15 am sun. Nurse Megan showed up with another heprin shot which I instructed her to no give it in stomach muscles as she did the day before. After shot my girlfriend noticed I was supposed to be getting shot every 8 hrs not every 24. At that point things got a little crazy, meaning they accused me of being out of it during the missing doses and that I was asleep. I know how many needles I've gotten in my stomach in my lifetime let alone over 3 days, at that point head nurse JP showed my girl a doctored up card with the added doses. This could have been more serious and could have caused death.Finally discharged 1/31/17 I'm a young man and çould not imagine my 85 year old loved one going through this. Jefferson hospital sent me there because of the 5 star rating. I found out they changed their name and added an addition. Please, do your loved ones a favor, use Kennedy's Rehab or Atrium

    This is by far the worst facility that I have ever seen. My loved one is getting substandard care. The facility is understaffed and the majority of the staff are downright rude and unhelpful. Our family is disgusted with our experience here. Not to mention, the patients are underfed cold inedible food.

    This facility is disgusting and the staff should be ashamed of themselves. It starts with leadership and trickles down to their staff. I requested my dad be moved to this facility based on the ratings on the medicare or medicaid site. (I do not remember) and either those ratings were severely outdated or no one ever visited the facility to see what actually goes on. My dad was transferred from the hospital directly to the facility with his cell phone, keys and glasses. My first visit- he had all his belongings. Cell phone and glasses were on the table- his wallet was in his pocket. I know- because I asked to see his wallet to make sure he had it. Cell phone & glasses aren't as big of a deal- obviously. A few days later- I spoke with his nurse, Jennifer, and she informed me that my dad was irate because he said his roommate stole his belongings when he left. Per Jennifer, she LIED to him and told him I had his belongings to calm him down (which it did). She stated she would have another aid look for them as she was too busy. I followed up and could never get a hold of Jennifer - because she never had time for family members. Either they have too high of a ratio for patients to caretakers or she was creating more work for herself. Either way, I could never get a response or update. (I'm also power of attorney)- She did let me know that they are not responsible for patient belongings… (but yet he brought them from the hospital- so what did they suggest he do with them?) Lets move on.. I call Dani who is the director (big big joke). We are now at Thanksgiving Eve… she proceeds to look for his belongings and call me back. Well- she does get back to me and tells me she found his wallet and put it in his drawer… sounds like a lie- as people were looking for it for a week now.. anywho- I visit him on Thanksgiving- the very next day… Do you think he wallet is in his drawer? Absolutely not.. because they probably never found it and probably didn't think I was stopping to visit.

    On another note- my dad was currently in a wheel chair- with medical staff all around- his wheelchair is alarming as he is trying to stand.. a group of 5 nurses standing around.. no one seems alarmed and one even says "you really shouldn't let him stand up" - my reply "I don't work here- you do." I'm confused on what they are really getting paid to do. This is completely mind boggling at the moment.. but moving right along.

    At this point- I don't know who Dani is (director) however- another patient leaves the facility without signing out and apparently he has a urinary tract infection. How do I know this? Because the nurse mentioned it to Dani and her response was "good I hope he keeps it" WAIT, what? Still- didn't know that was Dani (director) .. until she introduces herself to me in about an hour… you could imagine the look on my face.. complete and utter disgust.

    Moving along- my father gets transferred back to the hospital.. he's in there for about another month.. they are ready to send him to rehab and try to send him to Hammonton again- JOKE? I think not. I told them it was clear it was not going back to Hammonton. So the director of admissions calls me- Hershey Alter- I tell him everything I have just explained to you all… he pretends to be completely shocked that his director and staff would even act in such a manner. And he was going to look into the lost wallet- at this time.. I may not have expressed much concern about the glasses and phone as much as the wallet. However, I heard nothing back. I had to follow up with him and Dani quite a few times until I received a response.

    Here Here- now I receive a response from Hershey… at this point NO ONE knows ANYTHING about the cellphone and glasses (huh- but ok… he has a new pair of glasses and the cellphone can be replaced) and they want to know how they can compensate for his wallet - EVEN THOUGH THEY ARE NOT RESPONSIBLE… Oh my have the tables turned by Mr. Alter. Trying to persuade me to send my father back to the facility he was very accommodating- now that I sent him somewhere else- he doesn't believe they owe anything. Interesting. I am now waiting on information for the board of directors- which I was told there isn't one… but there is a complaint hotline.

    This should be interesting.. Just steer clear of this place.
