    Disgraceful facility -- my mother-in-law has been there a little more than 2 weeks and during that entire time she has been there she only had her own clothes on once, has not had her glasses or her dentures since she arrived even though we were told that there is an inventory that shows she arrived there with them. We were told by management that this is something that they are bad at and need to work on!!?

    My mother-in-law has seemed to have lost weight since arriving and today she mentioned that she actually has not been eating much because she hasn't had her teeth and there is no one to help cut her food so she eats very little. This is from someone who has always had a very hearty appetite. She also mentioned that she has been wetting herself because no has been available to take her to the bathroom.

    Staff constantly making excuses that others are lazy or new or under staffed. The lack of care of the patients and the lack of caring by the staff of our concerns I astounding. Management seems to have no control over what staff do since they seem to be constantly in meetings or on vacation as we have also been told.

    Phone calls go unreturned including from management. It was 24 hours before someone even reached out to us that she had arrived at the facility and this was only because we refused to hang up until someone answered the phone. When you call you constantly get transferred to extensions that have full mailboxes so no message can be left. Even the receptionist, who we witnessed in person, could not get someone in the facility to pick up a phone.

    I too have had a bad experience with Care One in Moorestown. Yes, it is a beautiful place but like the other reviewer pointed out, don't be fooled by the gorgeous exterior. The type of care our mother received is cookie-cutter, one-size fits all care. She actually fell and broke her pelvis during her stay and ended up being far worse off than when she entered this facility. I will point out that there are many good nurses and aides who have been very kind to our mother and they are angels on earth, and she absolutely adores her physical therapist. However, this facility arranges for services that she doesn't even need such as speech therapy. My mother is recovering from a bout of pneumonia and needs to build up strength - she doesn't need speech therapy. Again, it's this one-size fits all 'care plan'. I hate to say this but I truly feel this place milks the patients for all their worth. It's terrible but I agree with the other reviewer who mentioned this place holds your loved one hostage. THEY decide when you will be released and what type of therapy you'll receive regardless if it's needed or not. Another thing I want to mention is that my family and I tried many, many times to reach our mother's primary doctor and left message after message. Her doctor NEVER once returned any of our phone calls. That is terribly unprofessional. Just be warned this place is not all that it appears to be and if possible, try to do your homework when searching for a rehabilitation facility BEFORE it is needed.

    I think they should change the name to Care Zero, or better still leave the Care off completely. My wife asked to be taken to"Care Less" because her time in Moorestown Care One was terrific five (5) years ago, There is no comparison. Voorhees operates in spite of having NO management on site. If they are on site the are not controlled by upper management. Of course there are people who care but not many. They don't bother to address the patients with any respect. I would like to mention two people who went out of their way to help us The two are, Evytte & Levi. For the most part they are the only two that stand out. The call button with no one responding is useless. The claxson horn that sounds forever is asinine.I finally took my wife out of this facility at her request, only thing I did wrong after going there in the first place is staying too long.

    Resident Never available for phone calls-My family member was admitted to the hospital on December 26 2014, I am also her power of attorney. On January 9 2015 I received a call from a Physician at a local hospital asking me to make a decision on discontinuation of care and instituting comfort care measures. I was never informed that she had been admitted to the hospital. The Physician stated that I was second on the notification list and gave me a name of someone I had never heard of. Long story short I was misled that she was still mentally capable of handing her affairs and a staff member from Care one had contacted an Attorney stating that I was unwilling to handle her affairs, in turn he hired a Person to help her sort her mail. The attorney charged $2500.00 a pop and the helper charged $225.00 to $250.00 weekly. Neither ever contacted me and now my Step Mothers money is gone. If you have a family member in this facility and you live in another state LOOK OUT. I was told by the Administrator that my name was taken off and they could not locate my Power of Attorney-after a few phone calls to her is reappeared. TO LATE!

    Level of care & organization is very poor. Team meeting only once a month, there does not seem to be follow up or accountability. I asked how often my loved one would be checked & if there was a chart to ensure accountability, the response was 'sporadically' & no chart. Staff have evident low morale & there is a disconnect between nurses & health aides. The level of rehabilitation is bad, therapists do not communicate with each other, families nor do they appear to follow a schedule.

    Stay away - there are better, safer places for your loved one who deserves better.

    My mother was there a year ago, and it was a terrible experience. It is very painful to write about what my mother went through, but she was neglected and almost ignored by many staff. She had a lot of bruises, and they didn't report the bruises, they just told me that it happened on another shift and that she was prone to bruising. This was absolutely not true. The bathrooms were extremely unclean. They moved my mother into another room which had a giant puddle of dried urine on the floor the day she moved in. I would come to visit and find the small trash can in her room filled with her roommate's dirty diapers. They moved residents around with no regard to their feelings. I saw a case when they moved a resident from a room, with a long time roommate they had bonded with, and put them in a room with a women who hated her, and screamed at her day and night. In another case, they put a women who was very stressed and needed calm, into a room with a nearly deaf women who blasted the TV at full volume, making the other women cry, and the staff just became angry at the women for "acting up". Why not put two deaf women together? I could list many more examples of why you should not bring a loved one to this home.

    This place is holding my Mother hostage… She fell and broke her femur in August. They had her in and out of Marlton in under a week. She was at first given too much pain meds in Moorestown and then refused to take them except at night. They have repeatedly told me that it is unsafe to bring her home but she is showering, using the bathroom, and walking with a cane on her own. If I sign her out AMA then they will not set her up with in home therapy that she needs. Bottom line the only thing other than therapy they are providing for her is food and I do all the cooking for her…. I have been fighting with them since last week. The social worker Nancy gives absolutely no reason as to why they feel it is unsafe. They will not answer the question "what are you doing for her there that I can not do for her here?" She does not hear well and can not hear the phone in her room, so none of her friends have been able to speak with her. She is crying on the phone that she wants to come home and her emotional health is suffering. My advice RUN from this place.

    the care here was top notch. My father was a patient and the care and compassion he received from the nursing staff and therapists was remarkable. The director of nursing was extremely hands on and tended to my fathers wound personally. The care he received at this building allowed him to return home with his wounds healed.

    I consider this facility Burlington Co. best kept secret. I have become very close with the activity ladies. I plan on coming back utilizing my cane not my wheelchair to see them all. KAtie from the kitchen is the sweetest hardest worker I have come across in my stay any where. I am being discharged next week and will always have a place for the people who helped me progress and get back to my full potential.

    My loved one was here for "rehab". Care one's definition of rehab is 1 and a half hours per day of occupational and physical therapy not beginning the day the patient arrives but three days later. (Well yes ofcourse they need to keep them longer to get a bigger check from the insurance companies.) This facility is a caotic zoo! As you walk through the front room there are elderly people in wheel chairs spread out everywhere and in the way of everything. Everyone is on oxygen due to the fact that the nurses do not varify any of the medication that is rx to new patients coming into the facility, so if there are patients with breathing issues, instead of giving them there rx inhalers they hook them up to oxygen. Let's not forget to mention how my loved on sat in her own urine and feices for hours before a family member noticed she was wet. She even received her breakfest before she was checked. There policy is only to give a shower 2x per week. That's really nice especially when you leave her in that mess. Places like this make me sick. Everyone that is apart of this facility or in conncetion with this facility should be a shamed of their selves. DO NOT EVER SEND YOUR LOVED ONE HERE UNLESS YOU FEEL LIKE THE DESERVE TO BE NEGLETED. Unfortunetly I had no say in the matter of my loved one going here, if I did this would have never happened because they are all the same.. discrasful.

    P.s. if anyone wants to know the code to open the door for the employee exit let me know one of the aids gave it to me to get a soda out of the vending machines, anyone could walk into his place.. ABSOLUTLY NO SECURTY AT ALL unbelievable.

    If you care for you loved ones STAY AWAY! They line them up in front of the nursing station from moring to night so they do not have to do their jobs. It's sad ! And unnessary! Just cause they are lazy. They complaint they are understaffed but saw most laughting and jokeing at the desk My mom was a fall risk and had a bed alarm anyway they had no clue I was in the room when the alarm went off She was fine the alarm was placed wrong on her bed so it was going off while she was sleeping but it took them 15 minutes to check on her but only after I went to the front desk to request assistance! Glad she wasn't on the floor needing thir help. Oh one day mom asking to go to the bathroom and her aid pulled me aside and said you do know your mom has a catheter in. I'm like really cause that was taken out a week ago. The staff had no clue!!! If you put you love ones hear check on the regularly!!!!

    I am not sure if the other reviewers' loved ones where staying at the same facility as my mother? We had an excellent experience given the situation at hand. Mom has been there two times now. I did my due diligence and I had more word of mouth referrals to this community than anywhere. The nursing staff and therapists all did a great job. The second time she came back the staff made it feel like a coming home party.

    Excellent staff, caring and attentive. I have observed my father's stay at another rehab, and the difference is substantial. At Virtua my father is getting physical therapy every day, twice a day. At another facility it was hit or miss, and his progress was much slower. The CNAs and nurses answer any request quickly and with a smile and compassion. This seems to be the culture there, and I have not seen any impatient or less than kind staff. Highly recommended. 5 stars.

    This place gets one star. They are inattentive to resident needs, the "aides" rough up the residents and throw them around like a sack of potatoes when getting them up and dressed and they play with their cell phones when no one is around while they should be on duty. In addition, the food is TERRIBLE, like what one would expect to find in a prison cafeteria.

    They do very little with respect to helping residents get a shower, brush their teeth, wash their hair and so on. The facility itself is antiquated with bathroom sinks at such an elevated level, a resident in a wheelchair cannot even reach the sink to brush teeth or get a glass of water. if a resident is incontinent, they can lay in their own filth for many hours before someone decides to change them - and they do not even get a shower, only a "sponge bath" if they are lucky!

    It is extremely difficult to imagine how on earth they obtained a 5 star Medicare rating, unless those stars come with a price! I am a family member of one of their sub-acute "rehabilitation" residents.

    The place doesn't smell disgusting and the the residents appear clean and unsoiled--if that is the only requirement for nursing homes, then I can understand their 5 star rating. Outside of the above "attributes" that center should be rated one star. Their staff are poorly trained, the supervisors are completely unable to coordinate staff schedules to meet the needs of the residents, and the so called "rehabilitation" part of the center is laughable at best. It is clear they know nothing about the complexities or needs of stroke survivors, the have no knowledge of people who suffered debilitating neurological events, and therefore, are completely unprepared to care for and rehabilitate this particular population.

    Residents in the short-term "sub-acute rehabilitation" section of the center are treated exactly the same as the long term residents of the nursing home. There is no focus on rehabilitation, and the infrequent one hour rehabilitation sessions they do receive is sub-par and noneffective.

    The rest of the day, those in the "rehabilitation" section are left to flail on their own, alone in their rooms, with absolutely no staff supervision or stimulation.

    There is also no effort to help or educate the families of the "sub-acute rehabilitation" section on how to best help their loved ones regain the abilities they might have lost due to stroke or other neurological events.

    I am convinced their sole purpose is to gain a profit off the health insurance of the most vulnerable members of our population. As long as they don't smell and the residents are kept clean, they think they've done their job. They have no business being in the sub-acute business.

    Five star rating from the government is a healthy stretch. The nursing care is adequate. My mother is a resident there and generally she is clean and there are no sores or open areas on her body. I check her daily and I visit 2-3 times a day. The rehab is questionable. The rehab department is very quick to say someone has reached their max and the medicare is cut off. It is useless to appeal the decision as the reports sent to medicare are slanted their way and there is not a visual inspection by medicare they only go by the written report from the nursing home. When the actual rehab gets to where they have to actually do physical work with the patient the pt. has reached the most she can do.

    My grandmother was here in the rehab section to build up some strength after she fell and was unable to get up or get herself to the phone. The idea was to allow her to return to her home and live independently with daily non-medical help. While I was only an infrequent visitor, it was remarkable to me that sometimes her speech and affect were close to normal, but other times she was unable to speak clearly, slurring her words, speaking much more slowly, and appearing dazed. I strongly suspected that she was given too much of her tramadol (which she had been taking for years for pain with no such effects), or perhaps being drugged with Seroquel or other psychoactive meds. Staff assured us otherwise. She was not permitted to get herself out of bed or her wheelchair to use the bathroom. If we had not requested that she be allowed to shower (although never alone, never washing herself), she would have only been bathed in bed. Personal hygiene, toileting, occasional cooking, walking from room to room were activities that she could handle before, however slowly. After 2 months, she changed from a reasonably healthy albeit frail lady to a near-invalid because she was not permitted to care for herself at all. After a brief daily physical education session, she was returned to her room, either in her wheelchair or in her bed for the night. On my last visit, she was put in her nightgown in her bed at 4:15pm because she had been unable to get off the toilet. Her physical therapist informed us that she was "regressing". Her speech became barely intelligible. I strongly believe these were medication effects. If this facility's mission is to return people to normal or near-normal functioning, they are failing miserably.

    I would not recommend this facility. Very dirty- housekeeper came in & said they were going to clean bathroom - he put some blue liquid in toilet & left. Beds & overbed tables were dirty. Short staffed and workers who were there were unfriendly & gave rude answers to questions asked. I had private duty caregiver for my father & no one came in to check on him. Nurse was told he needed bandage on wound on 2 different days - this was never done ,she was also told that my was constipated and nothing was done.
