    This facility has been under new ownership for over a year now since the old ones filed for bankruptcy. To quote a longtime employee, 'this place is the worst that I have ever seen it'. Please don't send your loved ones there. State is called in frequently because there are so many complaints. It truly should be shut down.

    My Father fell and broke his neck at Montclair, because of short staff. They, then put him back in bed and just left him. That is how I found him a couple of hours later, with a bump, the size of a baseball on his head. I had him taken to the hospital, where he passed away, a few weeks later. We filled suit against Montclair, only to find that they had filled for bankruptcy. I see that they are still going. Avoid this place, like your loved one's lives depend on it….They do.

    I had my wife in this facility for rehab and wound care. They did an excellent job with her. They got her up and moving very quickly and healed her wound. The rehab staff was excellent and very friendly. The nursing staff was also very friendly and gave good care to my wife. Most of the time they were very prompt with answering the call lights and we had a great experience there. I would recommend this facility for anyone needing rehab.

    A family member was placed in this facility as a last resort, as the other facilities that we wanted him to go to had no beds available. Needless to say the rating they were given by the state and senior care people were far to kind. My family member was place in this facility for what was suppose to be rehab. and wound care. The rehab part was very well and did the best they were able to. The nursing staff and there wound care staff was awful, and on numerous occasions we witness them being down right mean and causing him pain on purpose, as my family member now has to have part of his leg amputated as his wounds ended up with gange green and the dead tissue down to his bones and the pain became unbearable, because of there negelance, he was also considered malnutrition when we the family could not see him like this anymore and the lies they were telling us we wanted him to go to hospital, Instead of calling an ambulance they used their van to transport him to the hospital, dropped him off outside the ER and left, without checking him in and wait for one of his children to show up.. Thank God one his daughter and son in laws were following them so he only had to sit outside by his self for a few minutes before they arrived and had to wheel him inside of the ER and check him in. We do not recommend the place for anyone who loves there family members and want what is suppose to be good quality care. PS they allow you to bring pets in on Sunday's, our dog would not even eat the food they served, he smelt it and walked away… that tell you and us why he was considered malnourished when he was admitted to the hospital.

    i agree with the other reviewer.this facility is a joke the do not care for there residents like they say they are going to do since it is a small facility.with that in mind you would think they are getting awsome care but not true.my mother has been in there for almost 2 weeks and i have complained almost everyday. I have went to the head of nursing and i got no response. most of the nursing staff is rude there was only like 2 of the nursing staff that is there cause of the people and not just paycheck. I visit my mother 2 times a day and I have found my other in soiled and or dirty pants almost on a daily basis.not cool. She is there to streghten up her legs but you cant do that if they arent making them walk instead of wheeling them around cause it is easier and faster for them. Thats not why they get paid the high dollars for them to be neglected. or ignored. please think before placing a loved one in the facility. cause it isnt what they make it out to be by no means…the only reason there is one star is cause it wont let me submit my review with no stars filled

    I would have to rate LCC of Elkhorn with 1 star. That 1 star is for the few whom I actually felt gave my mother the care she deserved. I spent most of my mom's 76 days at this facility there. I was afraid if I didn't they would not brush her teeth, wash her face, take out her hearing aid, etc. Call lights were on an average of 12 minutes. My mother would wet her pants waiting for help to get to her. The food is not good. I saw on line they have 1 full time cook and 1 part time. This would explain why the food looked unappetizing, tasted poor and was often times delivered to her table cold. 1 full time cook is not able to feed 114 residents. No one bothered to reassess my mother's meds. Found out when she was dismissed that she was on 8 pain pills, still had the pain patch and 1 prescribed pain pill at night that was as needed. My mother had not had pain in over 6 weeks but was still getting all the pain meds. She came home so overmedicated that once we weaned her from the meds, she slowly was becoming her old self. Though I was very vocal in my concerns, I felt as if I was a bother and they rather I not be there. However they do stress the importance of family involvement in their booklet as well as their website. I know she missed a bath on occasion, I know she was not gotten up at night on occasion and therefore was saturated in her own urine in the mornings. I know this because I brought all her clothes to be washed home with me. With no computer system, everything appears written out. The problem is shift 1 could not answer my questions from shift 3 as their claim is "I wasn't there". When my mom caught their gastro flu bug and was vomiting all day, her dinner meal was chili and lettuce salad. Too many concerns to list for this review. I went because they had an open bed when she was released from the hospital. Clear for me to see why there were "open beds". In good conscience, I can not recommend this facility.

    horrible! while my dad was there-3 of the employees themselves agreed that the place was not good! One said 'I know I hate this place' They never read notes that were given about the patient--miscommunication. Nothing in restroom-when they got him up to use toilet-set him there(he was a fall risk) and left him to go get a depend-then she was like-what does he use to wipe off?? asking me?? she had me had her a glove! When he first got there-he was wearing edema socks(that need to be taken off at night-he wore them for 3 days-no shower for 3 days-never took him to day room to eat(even with sign there-brought him in a tray and didnt help him. I was in room for 8 hours with him(they came in twice) he was suppose to be on a reg. bathroom assist schedule? We took him out--it was dirty(no hand sanitizer near room-no one even came in room until 9:15 in the am. food horrible! employee told us we should bring in our own food-its that bad! told them to put an alarm on his bed(they didnt until his third day there. I came in and he was sitting in a recliner and no call button was near him-yet he had on a braclet Fall Risk! Lack of communication there, staff not around, nothing in the bathroom-except toilet paper and paper towel(never even a hand towel or extra depend in the room? The p.t. nurses were nice and good at what they did(although he only had p.t once in the four days he was there. I called director of nursing 3 times and left message for them to call me. I was cut off several times when calling to see how my dad was--no communication-very depressing place-old everything-not equipped well. Dont go there-since then so many people have told me oh don't ever to to Nebraska Kills! (skills)

    I would not recommend this Nursing home. My grandmother was admitted to receive end of life care until her passing. We had *specifically* asked to be notified directly (I had even asked if they needed our home number and they replied that they had it already) when they thought my grandmother might be close to passing so that family may be with her when it occurred. They did not call, however, and we were told through a third party, instead, and that call did not reach us because they used a different number than the one we gave the Nursing home. And when I asked the attending nurse why we had not been notified I was replied to with "Well you got the call either way, didn't you?" So because of this, my grandmother died alone.

    We are new to this facility and were very pleased despite the reviews,like all things there will always be dislikes, nothing is perfect we should all know that. The ladies at the front desk were very friendly in helping me out with all the many questions,overall i had no complaints, the staff all seemed very helpful, we feel this was a good choice home away from home - thank you all!

    The first reviewer is absolutely correct. The owners and management go to great lengths to put a very pretty PR picture together, all the way from soliciting their own employees to vote for the facility to be Omaha's Best, to hiring inexperienced new grad RN's, just so they can say they have RN's on staff. They have a beautiful property but horrible management .

    Do not let the pretty outside fool you their is a lot of ugly on the inside!!

    Very unhappy regarding the care of my father. He appeared to be uncleaned and upon questioning staff, found out that there were too many call-ins and that the bath aid was pulled to perform other duties. This happened several times. Gave my dad bed baths to clean him up, also shaved him myself. He was losing weight. Again, the staff were too busy to ensure that my father ate. The food did not look appetizing, nor did it taste good. My family brought in food and healthy snacks for him. Too many patients and not enough staffing to give the care that my father needed. We made arrangements for other care and brought him home.

    My mom has been at Omaha Nursing Center for over a year now and we have been very satisfied with the care she receives. The staff is caring and attentive and they address any questions or concerns we may have. Mom likes the food most of the time (which is quite a victory, since she was such a good cook herself) and she enjoys visiting with other residents. The building could use some updating, but it is kept extremely clean, which to us is much more important. I recommend this facility.

    The nursing staff were great- very friendly and attentive. They went out of their way to make sure my mother had everything that she needed. They even offered therapy on the weekend and had lots of activities in general, to include live music and always the daily paper. The food was much better than we expected compared to your typical hospital food. Mom was very happy with the care. I'm surprised by the comments posted by others- we would recommend Nebraska Skilled to any of our friends.

    I don't know where to begin since no one deserves being placed in this facility. Small hall ways for wheel chairs; call lights on for over an hour and no one comes; if this is a rehab center, where is the rehab equipment; when I visited an individual being rehabilitaed - there was one piece of equipment, even though this piece of equipment was to move an individual from chair to bed, etc; where is the staff when you need them - big turn over of staff; no activities for residents; the facility is being renovated but needs to be trashed. Safety issues abound. I don't understand how this center passes code? Please look at this facility before you place your loved one there! Animals are in a better facility than this.

    My mother was getting worse at this facility because they let her lay in bed all day and only get up once for physical therapy. She was brought her meals in bed. She was not required to get dressed. Her new facility requires her to dress, attend 3 meals in the dining room, and attend 3 sessions of PT/OT/ST in the exercise room. 6 times out of bed is way better than 1. She is doing much better since we moved her.

    this place is the most disgusting place imaginable. i wouldn't leave my dog there. the stench of urine is overwhelming from the front entrance throughout the entire facility. the mattresses are old and God only knows how many people have died on them. i removed bedding to look at the mattress in my mothers room and it was covered with urine stains. the staff there are unprofessional, rude, incompetent, loud and mouthy. most don't have any answers to questions at all. a white trash facility and staff. i have filed complaints with the attorney general, bbb, consumer reports and dhhs.

    This place is a wreck. My elderly relative was in recently for what we thought would be twenty days of rehabilitation. We are taking her out after less than a week because of the pathetic care of the staff. Nurses were inattentive and left her to lie on urine-soaked sheets for hours. My relative did not receive a bath the whole time she was there because "they didn't have enough help." Nebraska "Skilled" Nursing ran out of my relative's painkillers, which she desperately needs, for two whole days. When I went to the nurses' desk to see if they could possibly clean up the room or move my relative from the wheelchair, they ignored me for at least three minutes each time while they chatted about their personal lives. It took HOURS for anyone to come in and assist my relative. The food is disgusting - we had to bring in our own. The staff only paid attention to my relative once we hired a day-care worker at our own expense, to monitor the situation. This place needs a complete overhaul, and I sincerely hope the state gets involved. This is a travesty.

    This is supposed to be the best home in the area. It may look nice and be clean, but a majority of the staff are unprofessional, lazy and could care less about their patients. They focus on everything but the patient's care and attention. My mother needed to go to the restroom with assistance, they were rarely available. Their solution was to put her in a diaper and put an alarm on her bed so she wouldn't get up. If they would have answered her call, it would not have been an issue. My mom was there 4 months and didn't even know the employees names. When her medication messed up her mind, they abused her. I wouldn't let my worst enemy stay at this place. I beat myself up for putting my mother there for as long as she was. Don't judge a book by its cover. The people make the place.
