Repeated requests for water to be provided to my loved one who was totally dependent on staff were made but not always provided, infrequent brief changes causing skin issues and urine to drip on floor from brief overflowing, not providing meals that someone without teeth could eat, poorly monitoring of residents ie. only coming in room to see if resident ok twice a shift, staff did not know how to safely and properly use lift equipment sometimes placing resident in dangers way, staff not providing education regarding medications, poor to no communication with residents and families even when families are present and involved in cares.
