Quality of care and discharge planning with emphasis on how to function outside of this facility is poor bordering on NEGLIGENCE! my family member was admitted for rehabilitation. individual staff were friendly and helpful. overall there is a serious problem with staffing, staff utilization or response time to the call button or verbal requests. my family member was continent upon admission. due to 15 minute, 30 minute, or longer waiting for some one to respond to the call button, my family member soiled their clothes and bed numerous times. this was humiliating to my family member. i watched staff at the station where the call board is located ignore the beeping and go about other duties. there is no phone in the rooms. the phone number provided to speak to the family member is useless. the base is too far away from my family's room for the handset. no assistance was given during meal times. a tray was placed on the bedside table and the tray was removed after some time. mh family member was not offered to participate in the activities of the facility. no one offered to take her or inform her of activities. rehab did not show, train or discuss how to assist my family member upon discharge. physically the facility is clean. individual staff were friendly and helpful. Quality of care and discharge planning are poor bordering on negligence.
