I give this facility a rating of 0. My mother entered this facility in 2/2016 after having a massive stroke. She could no longer walk or move herself and became blind from the stroke. My mother's stay started out with having good care so we thought. Every time the family visited my mother, she was always laying on her back just like the woman in the bed next to her. They were supposed to turn her every 4 hours but they never did. In July my mother became very ill and we found out that she had a full body infection. She was admitted to Shady Grove Hospital, and the emergency room doctor said he has never seen bed sores this bad. She had bed sores so bad that the tail bone could be seen. The sores were Stage 4 bed sores that takes 5-6 months to develop. Collingswood Nursing & Rehabilitation Center stated that she did not have them when they took her to the emergency room. During her stay at the hospital, they informed my dad that she could no longer return to their facility. They are terrible caregivers. The nursing assistants do not know what they are doing and aren't fully trained. Check the State of Maryland's Health inspection report on them from 2008-2015.

I have a relative staying there during July 2016 so I had an opportunity to observe the facility first hand.
The hardware is very nice. They offer semi-private rooms covered by Medicare, and private rooms for out-of-pocket charge of $50 per day. The building is well-maintained, clean and quiet. The outdoor garden is pleasant and attracts many residents weather permitting. Each wing has about 10 rooms, with its own dining area. This setting is good for the residents to get to know each other. They have a well-equipped PT facility and daily cultural activities.
However, the management is poor. The staff members are hardly available for extra help (call buttons are left unanswered for a long time). Over the day, many staff mills around. It is difficult for the resident to know whom is responsible for what (so they can ask the right person for help). Plus, many of the staff members do not speak English well, adding to the difficulty to communications. They are always in hurry and can be rough in helping patients to turn in bed or to go to bathroom. Communications with the families is ineffective. There is no single point of contact for the family. For different issues you need to call different people, and those calls and emails are often left unanswered. There are good, responsive staff members. However, there is not a good common practice.
During my relative's stay they dropped the ball a few times. When my relative was discharged from hospital, the doctor wants follow up in two weeks. However, the facility never scheduled it. After repeated calls to the head nurse, the visit was accomplished 5 weeks after discharge. The doctor prescribed pain killer but the family could not find out when and how much it was actually taken (the resident did not have good memory and people at the nursing station could not find record). On a smaller matter, they delivered three pieces of clothing to my relative's room, which does not belong to my relative. We kept sending them back to the nurse station and they kept coming back to the room for three times, before discharge. On an even smaller matter, the hand sanitizer dispenser at the building door is left empty 8 out of 10 times. These incidents show an ineffective operation management.
To summarize, this is a good resource to use if you have a clear mind and can pretty much take care of yourself. If you have to rely on the staff physically or mentally, it would be a risky undertaking.

Discharge planning was an abject failure! Said they would order a hospital bed to be delivered to her home--never happened. So was a week before could get home health agency to order the bed.

I wouldn't even give this place a 1 start review. The only nice thing I can say about this place is Im stuck in a corner next to my dads bedside because they let you stay here 24/7 because no one in their right mind would leave their worst enemy here. Its dirty run down and you can never get a nurse in a timely manner to do any thing. I could go on and on. Its only hit 24 hours being here and we are stuck in here because the Fairfax Hospital dropped us here at 9pm on a Friday night and you cant get any thing done on the weekend to get transferred. If my dad was a little more stable I would stick him in the car and get out of here. I wouldn't even feed my dog the food here. Patients wandering all hours of the night in the hallways and into other patients room. As a visitor you cant even get in or out of here after 8pm with out a nurse escort.

Collingswood was a horrible place for my loved one. As the prices increased, the care declined. After terrible care that I believe accelerated my loved one's demise, they are now threatening me with a collection agency. An awful experience.

I had a family member admitted to this rehab place and when ever i would call to see how he was i always got the run around and they seem like they do not care if yo want to know how the family member is or not they dont want you to talk to the family member until you get frustrated with them they are very inconcsiderate and it seems to me that they really dont want to take time with the patients I always thought that the rehab and health places was for the patients to help them so they could go home and be with their loveones I recently went to see my family member we asked to speak to a nurse it took the nurse 25 minutes to come speak to us when she did she was very rude and no beside manner to the patient this place really needs to be looked in to I have a feeling that abuse is going on in this place and the patient is scared to say anything because what could happen to them after the family leaves The county needs to looki into this

I think that Bel Pre Nursing Center is an excellent facility. It is not fancy and does not have all of the bells and whistles, but it is clean, has no odor and has wonderful staff people. The nursing staff is attentive and calls the family member or responsible party whenever there is a development or issue relating to the resident. The recreation director does a good job on what I am sure are limited resources. I cannot rate the food, as I have not eaten it, but I have not heard any complaints about it from the resident that I know there.

If I could give it less than one star, I would. I have seen a lot of neglect. I have seen over-medication. There is a very bad patient/caretaker ratio. It's dirty. It smells. Phone and TV and cable are not provided. Everyone that accompanies me to visit my friend leaves feeling outraged and also shocked that people are treated this way. Look beyond the pretty picture they show visitors. Ask to see the units in the back, the units not included in the tours. Ask to see the rehab at it's busy time. Maybe they are doing their best….but it's not nearly enough. I have not met one resident that likes it there.

My mother was at the facility twice due to a fall. The OT was sufficient but the communication from staff, especially the social workers, was terrible. At the last minute they stated she was discharged, neither time organizing a care plan meeting. I would never recommend this facility. They ignored my requests to have my mother's hair done regularly and for her to attend activities daily also.

The staff who care for the patients are helpful and kind. The people who run the center can be a bit pompous. Therapy was put to a halt for my mother as soon as her insuranse stopped covering her. Then we found out that in order for her to stay at Layhill, it would cost 11,000 dollars a month - or roughly 360 dollars a day. This would exclude therapy. Outrageous for a center of this quality.

Correction: The owner of this facility owns three more facilities, fairly acquired when they were in distress, as this one was. They promote inexperienced new RN grads and nurses trained overseas at the LPN care level for two years and the third year is all about RN - IT, ie: "To maximize the full capability of reimbursed items due each admission then released to any available community shelter when benefits end."

Friends tell me that / like in any industry / there are some employees who offer excellent care and attention to their duties, and there are some who are not quite so good……….
This facility has at least one person who gives my Mom special care and attention every time:
She is a nursing assistant named Petrina. Apparently, this woman makes sure my Mom's teeth aren't falling out, she gets help going to the bathroom and getting cleaned up after meals etc. and she helps my Mom with all the moisturizer etc. the typical old lady needs for dry skin, etc. Any of us would think such things are obviously part of the job and not really negotiable. However, there are obviously some employees who avoid such things and prefer to just skate through their shift, leaving the less fun tasks for the next shift / employee to deal with (maybe). Overall, this is a great facility that tries pretty hard to keep everyone happy, but it's important to visit and pay attention to these "obvious" things, if you hope to get your loved ones taken care of properly / all the time.

My Dad was in this rehab center in Nov. 2012. At first, we thought the facility and the staff seemed very pleasant and we were pleased overall.
However, during the days that he was there, things quickly deteriorated.
He had been transferred from a nearby hospital. Communication was poor. We left messages that were not returned at all.
He was overworked in PT and was left huffing and puffing alone in his room. He needed oxygen and I had to get help which was slow in coming.
He was left alone in the bathroom even though he could not move around unassisted. He was having a crisis (and died that day) but my sister had to search for help when he shouldn't have been alone in the first place.
I don't know what caused this facility to operate so poorly - lack of staff, poor management…?? I would not recommend this center.

Our Manorcare (Potomac, MD) experience was exceptional. They took fantastic care of my mother. The staff is professional, friendly and responsive. The facility is lovely, nicely decorated and clean. There is a lovely dining room and beautiful patio area. Everything was so well done, so nicely done and so thoroughly done. It was such a comfort to know our Mom was being so well cared for.

My mom was sent here by her primary care physician and we didnt know anything about the facility except the fancy brochure. I went to see her the next day and was speaking with her about her medication. She said she had no medication and I asked the nurse why. Her reply was that she didn't know because she wasn't here in the morning. She was not alarmed in the least that a patient was off her meds for seizures.
Avoid this place if you love your family member.

This facility is a dump and the staff could give a rats ass abt your needs. The food is swill, something you feed pigs. This facility is an abomination and should be shut down. Collectively, this facility is staffed by uncaring, inep and surly staff. The front office folks are not much better, stay away from this dump. I was transferred there after major surgery and they didn't have have my medications. For pain, all they had was generic Tylenol. Upon arrival, the MD who said he would be there wasn't and the young lady who took me to my room didn't know how to use the scale. My wife showed her.
